9 Introduction to the Home Care & Hospice Resource Guide The 2014 Home Care & Hospice Resource Guide is produced to provide a comprehensive directory of Ohio Council for Home Care & Hospice s provider members in Ohio, as well as a useful source of information. It is intended to facilitate referrals for home care and hospice services. USING THE GUIDE The resource guide is provided as an OCHCH service. One copy of the guide is distributed free to all OCHCH members and persons who make referrals to home care and hospice. Additional copies are for sale. To order copies, use the order form in this section of the resource guide, or visit the OCHCH website at Ohio Council for Home Care & Hospice members have voluntarily submitted information included in the resource guide. Home care agencies are listed both alphabetically and by county served. Hospice agencies have their own alphabetical listing and are included with the home care agencies in the county listing. Agencies that join after publication printing are found on the OCHCH website under the Find a Provider section. The Provider Search is designed to help you find home care and hospice agencies based on county served, services provided, and payor sources accepted by the agency. The information is frequently updated and includes new agencies and current agency changes. 11 Definition of Terms Used in Alphabetical Listings of Home Care Agencies and Hospice Agencies This section provides information to help you understand industry terms, a listing of additional services offered by agencies, and the standard abbreviations. USING THE GUIDE Accreditation Accreditation is the first reference under the agencies contact information. ACHC is Accreditation Commission for Health Care, Inc; Phone: (919) CHAP is Community Health Accreditation Program; Phone: (202) The Joint Commission; Phone: (630) Agency Structure Institutional Based (Hospital or SNF) agencies are organizations whose agency is part of a hospital system. Official agencies are organizations whose agency is government-sponsored. Non Profit These agencies must provide skilled nursing as well as one or more additional therapeutic services, including home health aides, physical, occupational or speech therapy, and medical social services. Medicare Certified home health agencies typically provide skilled services on an intermittent basis to Medicare beneficiaries or other individuals with acute conditions. Services Additional specialty services can also be provided beyond the Medicare Certified service. 12 USING THE GUIDE Definition of Terms Used in Alphabetical Listings of Home Care Agencies and Hospice Agencies Private Duty Nursing: is a continuous nursing service that requires the skills of and is performed by either a registered nurse or a licensed practical nurse at the direction of a registered nurse. Hospice Program: is end-of-life care provided by health professionals and volunteers. Medicaid Waiver: The Ohio Home Care Waiver: is designed to meet the home care needs of individuals under the age of 60 whose medical condition and/or functional abilities would qualify them for Medicaid coverage in a nursing facility or hospital. The Transitions Carve-out Waiver: is only available to individuals age 60 or older that were either enrolled on the Ohio home care waiver, HOME Choice program, or on an Ohio Department of Aging waiver. These individuals would be determined to have a nursing facility level of care, and if this waiver were not available the individuals would require hospitalization or institutionalization in a nursing facility. PASSPORT: (Pre-Admission Screening System Providing Options and Resources Today) is administered by the Ohio Department of Aging. Home care agencies mainly provide homemaker, chore and personal care services; however, they may provide other services like home delivered meals, non-medical transportation, or emergency response systems. DD (Developmental Disabled): programs are administered by the Ohio Department of Developmental Disabilities under the home and community-based services waivers for nursing waiver and personal care services. Bureau of Workers Compensation (BWC): authorizes home health nursing, aide and therapy visits. BWC also authorizes hospice care. Department of Veteran Affairs (VA): authorizes home health nursing, aide and therapy visits. The VA also authorizes hospice care. Hospice Listing All hospices must be licensed in Ohio. Information includes if the hospice is Medicare Certified and/or accredited. A hospice must be certified if providing Medicare services. Hospices provide a specialized interdisciplinary program designed to provide palliative care to the terminally ill and supportive care for the primary caregiver and family members. 15 Defining Home Care and Hospice What are Home Care and Hospice? Home health care and hospice are health care options that allow individuals of all ages to remain in their homes alone or with their families rather than in an institutional setting. Home care is a health care option available to individuals with acute, chronic or debilitating health problems. Hospice is not only caring for the terminally ill, but also a philosophy that affirms life, dignity, and choice until the end of life. USING THE GUIDE Why Do People Choose Home Care and Hospice? Home health care services promote independence to individuals of all ages. Home care and hospice can provide a viable, cost-effective health care environment outside the institutional setting of a hospital or nursing home. What Types of Services are Available? Advanced technologies now enable health care professionals to use sophisticated, high-tech medical equipment and procedures in the home to promote healing and/or provide comfort and pain relief. Home health care services include care provided by skilled professionals as prescribed and directed by the patient s personal physician, as well as supportive services including assistance with walking, bathing, dressing, meal preparation and light housekeeping. A home health care or hospice team may include: nurses, social workers, therapy specialists, dieticians, home care aides, chore workers and volunteers.
