2 What s the Problem? U.S. health care system is the most expensive in the world and has mediocre health outcomes. Spending is projected to double by 2020 ($2.6 trillion to $5.2 trillion). By 2020 health care will consume 21 percent of the GDP. 46 million Americans are uninsured or underinsured. As much as 30% of spending is on wasteful, ineffective care. There is tremendous unexplained variation in health care spending.
