We have exciting plans for the campers this summer such as awesome field trips, swimming, yummy snacks, traditional and new crafts and art projects, tremendous staff and all around good fun! Here are your registration forms and our summer program information. As some camps fill quickly, please complete and return the forms as soon as possible. Each week is $300.00, except week 3 which will be 4 days due to the July 4 th holiday, the cost for week 3 is $250 (or $200 if signing up for 4 weeks or more). A $50 discount is allowed for each child who is registered for 4 or more weeks making each week a total of $ A non-refundable, non-transferable down payment of $50.00 per session, per child is due with registration (the $50.00 down payment will be applied to each session s fee). 2 PROGRAM DESCRIPTION Our summer Swim & Gym day camp program is dedicated to providing excellent opportunities for children 1st through 5 th grade to grow and develop into productive, responsible people. Through a well-planned program, we promote activities that help build healthy relationships, increase competence, and explore various ageappropriate interests. The staff uses the resources of Peninsula Covenant Church s campus to meet recreational, educational, social, cultural and spiritual needs of children. Daily Schedule: Our hours are from 8:00am to 6:00pm. Children participate in a variety of activities including swimming, organized games, playing on the playground, outdoor sports on the lawn and fine arts. Each child will spend part of their day with their Home Group, and part of the day in activities of their own choosing. Swimming: Kindergarten through 2 nd graders will be scheduled for swimming one hour a day at the PCC pool they will be offered semi-private swim lessons twice a week. Children in 3 rd grade and higher will have 2 hours of swim time at the Sequoia High School pool. (Sequoia pool is subject to change due to approval and availability.) Field Trips: We will have off campus field trips on Thursdays. Children must be at PCC and signed in and in their opening rooms by 8:45 on field trip day. First and Second graders in room 15 and 3 rd and up in room 104 across the footbridge. There will be no on site childcare on field trip days. Kindergarteners: In order to be enrolled in our Kindergarten camps your child must be registered to attend, or on the wait list to attend, the PCC After School Camp for the upcoming school year. Kindergarten Swim & Gym is limited to children registered for PCC After School Camp for the 2015/16 school year, and is only available the first 2 weeks of August. Snacks/lunch: We will provide two snacks each day (morning at 10:30 and in the afternoon at 3:30) and hot lunch on Tuesdays. The lunch menu will vary throughout the summer. If you think your child may not like the snacks/ lunch we serve or they have allergies, please send snacks/ lunch with them. Be sure your child brings a nutritious lunch each day. If your child has food allergies please send snacks with your child to ensure their safety. What we believe: Each person on our staff has expressed a personal desire to follow Jesus Christ and this relationship is foundational to our summer program. Each day during the summer we will have music and story time that will present material from the Bible and each child will have the opportunity to explore what it means to follow Jesus. Although all religions and belief systems will be treated with respect, the emphasis will be on the Christian faith. Staff: Our staff is a combination of our year round After School Camp staff and college students. There may also be some high school students that assist. Each staff person participates in extensive training that includes planning for children, safety, first aid and child development. Cost: Each week is $ A $50 discount is allowed for each child who is registered for 4 or more weeks making the price $ each week. A non-refundable, non-transferable down payment of $50.00 per week is due with the registration forms. Payment is due the Monday (one week) before your child is enrolled to come to camp. For example, if your child is coming to Week 1 then payment is due June 9 th, If your child is coming week 7 then payment is due July 21 st, Cancellations must be made 10 days in advance. If payment is not received by the due date, your child s space may be given to a child on the waiting list and a $50.00 late payment fee will be assessed. We accept personal checks, credit cards (Visa & MasterCard), money orders or cash. Please make payable to PCC.

5 BEHAVIOR EXPECTATIONS It is our expectation that children will behave well. Being respectful, appropriate interactions and learning selfcontrol is part of a process. At PCC Swim & Gym, we work to promote positive, healthy relationships with self and others. Please discuss our expectations with your child and have them sign below acknowledging that they know what is expected and will try to do it. Our staff will work with parents to develop an action plan to reduce or eliminate unhealthy behavior. If the action plan does not produce the desired results, a conference will be requested with parents and other staff members to determine next steps. Swim & Gym does reserve the right to terminate a child s enrollment in the program if there is an inability to resolve problematic behaviors. An Incident/ Accident notice will be on UNACCEPTABLE BEHAVIORS INCLUDE: the sign out clipboard if there was a defying authority or deliberately disrupting the group behavioral issue or your child was hurt assaulting another person by a verbal or physical threat or action during the day. Please check daily to see if you have a notice. Transportation will be provided by the Peninsula Covenant Church vans and drivers or by contracted by a licensed transportation company. Peninsula Covenant Church employees will not be liable during transportation, or at the field trip destination, in the event of your child s accidental injury, unless negligence is proven. 6 PARKING AGREEMENT When dropping off or picking up children who attend the PCC Swim & Gym, it is imperative that cars use the 3560 Farm Hill Boulevard parking lot (church side). Cars are not to enter the parking lots at 3623 Jefferson Avenue as these parking lots are for the exclusive use of the Peninsula Community Center members. Parents are to park only on the church side of the facility. I hereby agree to only use the 3560 Farm Hill Blvd.(church side) Parking lot for dropping off and picking up my child attending PCC Swim & Gym. I understand that using the 3623 Jefferson Ave. parking lots (upper or lower) could result in my child losing his right to attend the PCC Swim & Gym Program. Children must be at PCC and signed in by 8:45 on field trip day, Thursday. There will be no onsite childcare on field trip days. The first week your child attends please come to the SACC office on Thursday morning, where t-shirts will be handed out. Camp Shirts must be worn on field trips. If your child has two homes, or loses things easily, we recommend that you purchase an additional shirt ($10.00 each). To order additional shirts, mark the quantity on page 1 of this registration form and include the payment with your registration deposit. If your child doesn t wear their field trip shirt on a field trip day, we will provide them with an additional one and your account will be charged $10.00 for that shirt. Kindergarteners will not be going on field trips on Thursdays; they will have a special day at PCC.

8 Contract Highlights Parent Copy Down Payments A non-refundable, non-transferable down payment of $50.00 per child per week is due with registration. PAYMENTS Payment in full must be in the Swim & Gym office on the due dates. We accept checks, credit cards (Visa & MasterCard), money orders or cash. Please make payable to PCC. Payment is due the Monday before your child is enrolled to come to camp. For example, if your child is coming to Week 1 then payment is due June 8 th, If your child is coming week 7 then payment is due July 20 th, If your child attends camp, payments received after the dates are subject to a $50.00 per week late payment fee. If payment is not received by the due date, your child s space may be given to a child on the waiting list. Failure to make payments in a timely manner may result in your camp reservation being given to someone else. If you do not cancel a session before the payment due date, you are liable for the full cost of the week. Further, because we usually have a wait list, all cancellations must be reported to the office by , phone or in writing no later than 10 days before the camp week starts. Without cancelling prior to the week starting, you will be charged for the week your child does not attend. Please let us know when your plans change. OFF CAMPUS FIELD TRIP CONTRACT PCC Swim & Gym will have a weekly off campus field trip to a variety of locations in the Bay Area.
