1 Wright State University- Miami Valley College of Nursing & Health Summer 2014 COURSE NUMBER: NUR 7611 COURSE TITLE: Family Psychiatric & Mental Health Nurse Practitioner Practicum I PRE- REQUISITES: NUR 7003, 7102, 7103, 7104, 7105, all with minimum grade of C. Restrictions: Acceptance to graduate program or permission of instructor CATALOG DESCRIPTION: The focus is on comprehensive symptom assessment, diagnostic reasoning and disease management of individuals and families with acute and/or chronic alterations in psychiatric/mental health status and functioning across the lifespan. Health promotion, health behaviors and disease prevention strategies are emphasized with respect to epidemiology, risks, and growth and development across the lifespan. Therapeutic communication principles, techniques, ethics and cultural competency are emphasized. Advanced practice role development is incorporated into the course through patient care management seminars, case study and clinical practice. CREDIT ALLOCATION: 8 graduate credit hours [5 didactic, 3 clinical (168 hours)] Faculty: Anita Dempsey PhD, APRN, PMHCNS- BC, PMHNP- BC 118 University Hall Office Hours: Tuesday 2pm 3 pm, and by appointment STUDENT LEARNING OUTCOMES: 1. Examine scientific findings from nursing, developmental and biopsychosocial fields, genetics, public health, quality improvement and organizational sciences in the role, assessment, diagnosis and planning for the provision of psych/mental health care to patients and families with psychiatric/mental health disorders 2. Explore leadership skills that emphasize ethical and critical decision making, effective working relationships, and a systems- perspective to promote high quality and safe primary care for patients and families with psychiatric/mental health disorders 3. Explore knowledge of health care resources, quality improvement, and organizations to assess diagnosis and plan for management of health promotion and maintenance for patients across the life span. 4. Explore theory and evidence based outcomes for application in mental health practice settings. 5. Use communication strategies and patient- care technologies to integrate, coordinate, deliver and enhance care in the clinical assessment and treat of patients, across the lifespan, and families with psychiatric/ mental health disorders.

Explore ethical, legal, economic, political, social, technological, and environmental issues and advocacy strategies necessary to the care of patients and families with psychiatric/mental health disorders. 7. Maintain collaborative relationships in a clinical setting to enhance role development for the psychiatric/ mental health nurse practitioner. 8. Examine health care variations related to gender, racial, ethnic, spiritual, developmental and cultural differences when initiating appropriate prevention, health promotion and maintenance strategies in mental health settings. 9. Examine knowledge from nursing and relevant sciences necessary for practice including both direct and indirect care components that influence health outcomes for individuals across the lifespan, families and systems served by the psychiatric/mental health nurse practitioner. TEACHING STRATEGIES:, seminar, case studies, student presentation, group discussion, guest speakers, computer- assisted instruction, interactive videos, integration of reading assignments, student journals and faculty supervised clinical preceptorships. EVALUATION presentation 10% You are responsible for a 50 minute presentation on a selected topic (see syllabus) First slide, presenter s name, second slide objectives, last slide references. Prior to the presentation, submit an outline of your presentation to Dr. Dempsey via . Audiovisuals and other presentation enhancement as well as creativity are encouraged. One week prior to our presentation post a handout and one required reading for all class members. The handout should contain your name, objectives of the presentation and one required reading a recent article. Submit

3 five multiple choice questions that you will review at the end of your presentation. the questions to Dr. Dempsey with answers. Midterm Examination 20% Final Examination 30% 20% There are two case studies for which you are responsible for responding. Your first question must be, What is the differential diagnosis? Explain. Develop a minimum of 4 questions and answer them with specifics. Be concise. Remember: Quality not quantity. Reference your work and use APA format. Comprehensive Clinical Case Study Presentation 20% Select a patient form this practicum, (diagnosis much have been covered this semester. Write a Comprehensive Mental Status Assessment, history and focused physical. Compile a list of differential diagnoses with rationale for your selection with degree of probability, diagnostics tests ordered and rationale, write a prioritized plan, including specific medications, treatments and other interventions including health promotion activities. Discuss your follow- up of the patient. Reference your work and use APA format. This Comprehensive case study must be computer generated or typed and double spaced. See syllabus for due date. Student Portfolio (to be finalized in 7613) (pass/fail) Information form a portfolio is valuable in seeking certification, licensure, and employment after graduation. Useful information includes course syllabi, evaluations from instructors and preceptors, clinical logs, major papers and case studies. Additional information to prospective employers includes documentation of clinical hours types of patients seen, types of clinical setting and interventions used. This portfolio will be completed in 7613 and should include: Resume ( to be completed in 7613) State Nurse Practice Act Rules and Regulations for APRN Practice Malpractice Insurance Information Salary Ranges for APRN positions Certification Materials Brochure/Articles describing the APRN role you wish to secure Standard Care agreement (to be completed in 7613 not required this semester) Information about 1 APRN professional organization related to your interest with contact info. All Assignments must be submitted to the course dropbox. No credit is given for papers not submitted. Failure to do so, results in a 0 for the assignment. There are no re- takes on any examinations or other assignments.

4 Late papers: Late papers are those not received on the due date and time. They are not accepted or expected. Only exception is an emergency, such as a family death or severe illness. In these situations the student must notify the faculty as soon as possible. All other late papers will receive a 20% deduction for each week (5 days) they are late. CLINICAL REQUIREMENTS Prior to starting clinical, a clinical guideline form must be signed and on file. You cannot proceed with clinicals if this is not done. 1. Clinical (Satisfactory/Unsatisfactory) You are responsible for abiding by the clinical guidelines in the packet. To pass the course you must pass both the didactic and clinical portions. To pass the clinical portion, in addition to successful preceptor evaluations, site evaluations and other clinical evaluations as needed. ALL assignments must be completed at a MINIMU OF A SATISFACTORY LEVEL. Students are expected to coordinate their clinical experience with their preceptors. Preceptors must be in practice for a minimum of one year prior to precepting. APRN preceptors must be recognized by the Ohio Board of Nursing as an APRN (if practicing in Ohio). Physician preceptors must be board certified. Students should plan to spend approximately 16 hours per week in the practice settings. This is a functional rotation- your time must be spent providing direct patient care and not simply observing. Any hours spent in observational role or attending lectures relevant to your rotation are encouraged but are not to be counted as hands on clinical hours. Submit a completed preceptor information form by week one along with a schedule of your hours for the first five weeks. E- mail me with this information. If changes are made in this schedule it is your responsibility to me within 24hours. If you will be absent from clinical on a previously scheduled day, you must notify your clinical instructor and preceptor by 8AM that morning. The remainder of your hours must be submitted each week. All students are subject to unscheduled, impromptu, site visits. Be certain to give your preceptor the evaluation forms. These are due the last day of class. Misrepresentation of clinical hours will result in an F for the clinical course and you will not be allowed to continue in the PMHNP program. Your preceptors MUST sign off on your hours on a weekly basis. These must be submitted on weekly basis to me. Hours not signed will not be counted toward your clinical rotation. Incomplete Clinical Policy Students who are unable to complete the required clinical hours within the semester may at faculty discretion be offered the opportunity to complete these hours by registering and paying for additional cred hours (In NUR7115; Independent Study) the following semester. Students register for NUR7115 using the following clinical hours per credit hour ratio: 1 56 hours to complete equals 1 credit hour hours to complete equals 2 credit hours hours to complete equals 3 credit hours

5 You cannot start NUR 7612 clinical hours until all hours of NUR 7611 are completed. Clinical Logs (40%) Students are expected to maintain documentation of their clinical experiences. You may choose to use the Weekly PMHNP Clinical Practicum Log for this documentation. Be certain to keep a copy of the log for your portfolio. Logs are due weekly and the completed log at the day of the final exam. Misrepresentation of facts in the clinical log will result in an F for the clinical course and you will not be allowed to continue in the PMHNP program. SOAP Notes/Comprehensive Mental Status evaluation/history and focused Physicals (40%) ALL NOTES MUST BE HAND WRITTEN. No COMPUTER GENERATED NOTES until specifically cleared to do so by your clinical instructor. Students are expected to maintain documentation of their clinical experiences.(complete and keep the Weekly PMHNP Practicum Log for your documentation.) Be certain to keep a copy of the log for your portfolio. Complete four SOAP notes/week on patients seen this semester for a total of 40 notes and two Comprehensive Mental Status Evaluation/History and focused Physicals. All notes must be co- signed by your preceptor. If the note is not co- signed it will not count toward your assignment. You may choose to photocopy your work; there is no need to rewrite the note if it is already handwritten. Be certain your handwriting is legible; your name is on the note and that you use standard abbreviations. You can NOT use your preceptors note. See syllabus for due dates. Consecutively number your SOAP notes. Black out patients names. To receive feedback it is essential that you turn in at least 4/week starting by week two. You are not allowed to submit 20 notes on week four or five! When you submit your weekly clinical log to your clinical instructor, also submit a hard copy of the hours completed that week along with a running total. You are required to also submit a paper copy with your preceptor s signature for verification each week. Failure to do so results in an F for the clinical. Reflective Journal (20%) Each week following your clinical experience you are to reflect on that experience in terms of your perceptions. What did you bring to the setting? What went well? What could have gone better and specifically what you would do differently? What have you learned about yourself through this experience? What are any questions that you are having about the experience. What do you need in order to continue to learn and grow. How can you facilitate getting this? This reflective journal is to be turned into drop box. Title each entry with the week that it is completed. Grading Scale: = A, = B, = C, = D, < 60 = F, no round off per CoNH policy. Students who receive a C in NUR 7611 cannot proceed in the PMHNP program
