The website also provides downloadable forms for: - Congress registration - Lecture/poster submission - Hotel booking CONGRESS FEES Fees The fee depends on the date it will be credited to the bank account. Participant congress fee: - before (early) 120 Euro including 23% VAT - after (regular) 145 Euro including 23% VAT Banquet (Friday) - 75 Euro including 23% VAT The participant congress fee includes: congress materials (abstracts, programme and other materials), identity badge, lunches on and , coffee/tea during breaks, admission to the lecture rooms and commercial exhibition, transport to the congress venue on and , WiFi hotspot on the premises of the Congress venue. 5

6 Payment methods Payments can be made in Polish Zloty (PLN) or in EURO (foreign participants) in the following ways: 1. On-site payments Cash or credit card (Visa, Mastercard, Eurocard) payments will be possible during the Congress. CANCELLATION & REFUND Withdrawal from participation should be submitted to the Organizing Committee in writing. INSURANCE The Organizing Committee is not liable for any injury, damage, or inconvenience which may occur during the Congress or while travelling to and from the Congress. It is recommended that all Participants hold their own insurance. SCIENTIFIC PRESENTATIONS Lectures Invited speakers from Poland and abroad will deliver keynote lectures. Participants reports are welcome as oral or poster presentations. Scientific Committee will review submitted papers on the basis of anonymous evaluation. English and Polish are the languages of the Congress. Posters There are 2 poster sessions planned in the form of discussions on (Friday) and (Saturday). Scientific Committee will select 3 best posters which will be awarded diplomas. Publications Abstracts of all lectures and posters will be published in Polish and English version in the supplementary issue of the medical journal Ortopedia Traumatologia Rehabilitacja (Orthopaedic Traumatology Rehabilitation). All participants will receive the supplement in their congress materials. 6

7 Lecture/poster submission Lecture or poster submission is possible only online on Certificate of attendance and educational (CME) points Participants will receive Certificate of Participation and educational points in accordance with the ordinance of the Polish Minister of Health of regarding means of fulfilling obligation of continuous medical education. Depending on the type of participation the following amount of points is to be accredited: 14 points for participation in congress sessions, 24 points for participation and poster presentation, 34 points for participation and lecture delivery. Online registration Detailed information concerning registration is available on SOCIAL PROGRAMME Opening lecture During the opening ceremony on (Friday) Prof. S.E.Papapoulos will present the lecture Novel therapy in osteoporosis. Lunch The organizers will arrange lunch for participants on (Friday) and (Saturday). Lunch vouchers will be attached to the participant materials (note that duplicated will not be issued). Banquet After the scientific sessions on Friday ( ) all participants are invited to the banquet organized in Hotel Park Inn by Radisson Krakow Hotel, ul. Monte Cassino 2. Banquet entry 75 EURO per person. Number of banquet invitations is limited, therefore, it is necessary to book in advance.
