1 The Informant I n s i d e t h i s i s s u e : Important dates 2 A newsletter for students from the Educational Affairs Office School of Medicine V o l u m e 3, I s s u e 2 J u l y Question of the month Lecture evaluations 2 3 Staff changes 3 Year 4 clinical elective Student access to EMR New combined MD/MBA program The UTMB School of Medicine underwent a site visit from the Liaison Committee on Medical Education (LCME) in December, Preparing for this visit was almost a 2 year process, with 110 faculty/staff and 26 students participating. The LCME Secretariat met in June to consider the site team s report. I am delighted to inform you that the Secretariat voted to renew the school s accreditation for a full 8 years. This is fabulous news! Of the 100+ standards the LCME measures us by, there were 6 for which they had some concerns, and we will need to provide progress reports to show evidence that we are in compliance. What will this mean for you? I have tried to outline this briefly below: 1. Tracking clinical encounters - The LCME requires that we determine types of patients that all students must encounter during clinical training, track that students are encountering these patients, and provide alternatives if for some reason a student is unable to see that type of patient. This is why we have all clerkship students log their patient encounters in New Innovations. We must track this very closely, and follow up if a student s log is incomplete it may be that the student hasn t done the logging, or it may be that the student hasn t been able to see that LCME Accreditation Update patient type. Either way, we need to know if you have been unable to see that type of patient, your clerkship director can make arrangements for you to have a different clinical assignment, or work through an online case comparable to that requirement, etc. If you do use an alternate way of completing your patient encounter requirements, we also need to track that New Innovations has a spot to so indicate. 2. Direct observation of clinical skills all clerkships are constructing methods to ensure that you are directly observed with patients. Some clerkships already have graded observations in place. Others will work to ensure that you are observed doing at least part(s) of your history and physical. Again, systems will be put in place to document this, so this may result in more questions for you to answer on evaluations or New Innovations. 3. Mid-clerkship feedback you will be asked on New Innovations to document whether or not you received performance feedback. Clerkships may vary in the type of feedback provided, such as graded H&Ps, practice tests, observation of clinical skills, small group feedback, clinical preceptor feedback, etc. Continued on Page 5

2 P a g e 2 Question of the month We hope you are never on the receiving end of mistreatment, but if you are, we also hope you know where to look to figure out how to report. Last time we asked you how to get the word out about reporting mistreatment the message we got was that the more ways it is conveyed the better. Hence, you now have a hangtag for your badge and posters in your small group rooms. You ve had workshops, read newsletter articles. We hope you are never on the receiving end of mistreatment, but if you are, we also hope you know where to look to figure out how to report. (This was also true when asked about racist comments, they just weren t as frequent) How do you think we could better educate our students about this? How do we best convey what a sexist or racist comment is, and why it is disrespectful and harmful? 3 P a g e 3 Lousy Lectures One of the many surveys you get asked to complete is administered by the Student Government Association yearly. This generates reams of data, and we were pleased to see that SOM students are on the most part content. We read the comments, and one comment that was often repeated was a complaint about the quality of lectures. Although our curriculum de-emphasizes lectures in favor of case-based teaching through PBL, we do want those lectures that are offered to be of high quality. One thing we have already done to address this is that each course is asked to report to the Curriculum Committee which of their lectures are evaluated poorly, and to explain how they have addressed this. No Course Director wants to stand up and admit that their course includes many lousy lectures. This process is somewhat hampered by the low number of evaluations we receive for lectures. We will explore ways to make it easier to get to the evaluations and complete them; we need your help to ensure this happens. If a lecture receives poor evaluations, the Course Director has several options drop the lecture, replace the lecturer, or work with the lecturer to improve the quality. We will offer faculty development workshops targeted to improve lectures. We are looking into ways to reward the best lecturers. Most importantly, we are paying attention, and your concerns have been heard. Dr. Rampy is taking a position with Dell Medical School. We wish them all the best! Dr. Majka Woods is joining Educational Affairs as the new Director of OED and Assistant Dean of Educational Development effective July 1. Welcome, Dr. Woods! Dr. Jennifer Hughes is the new director of ORME in Austin. Former director Dr. John Luk is transitioning to a larger role at Dell Medical School. Welcome, Dr. Hughes! through PBL, we do want those lectures that are offered to be of high quality.

4 P ag e 4 Year 4 Clinical Elective Requirement [Y]ou will now have a much more realistic use of EPIC and should thus be better prepared to In the fourth year of the curriculum students were formerly required to take two clerkships: Senior Surgery and Neurology. The Curriculum Committee decided last year to remove the Senior Surgery requirement, leaving a gap that needed to be filled with another course. A subcommittee of the Curriculum Committee was convened to discuss what the best educational choice would be. The Integration Subcommittee analyzed several options for a Year 4 Senior Surgery replacement. The subcommittee deemed the following principles important: The replacement should be an appropriately rigorous, full time activity and not a place holder during residency interview months. The replacement must be clinical. The replacement should prepare students for internship and provide an opportunity for professional development. The replacement should be flexible to address the needs of individual students. Ultimately the recommendation of the subcommittee was to require students to choose from currently existing clinical electives that meet the following definition: electives where more than 50% of educational time is spent in direct patient care or electives in a diagnostic specialty that directly applies to patient care (pathology, radiology). Research electives (that require an R Form) are excluded. This new requirement takes effect this academic year. Our hope is to increase course offerings that fit this description. function in the EMR environment in the future. The EMR is coming! The EMR is coming! Currently, year 3 and 4 students may write notes on the inpatient services but not on the outpatient side in our electronic medical record, EPIC. Students currently also have no ability to write orders. I am delighted to announce that this will change very, very soon the anticipated go live date is this fall, so I feel confident it will definitely happen this academic year. What does this mean for you you will now have a much more realistic use of EPIC and should thus be better prepared to function in the EMR environment in the future. You will be able to write notes in the outpatient setting, complete telephone encounter documentation, and write orders at pended status. This also means some more training for current year 3 and 4 students, training sessions will be available frequently to give you access to the new functions. Once you have completed the training, you will be given the appropriate security clearance to use the new capabilities. Once the training schedule is determined, you will be invited to sign up for a session that fits your schedule. Stay tuned! For current year 1 and 2 students, your training will fall in the spring of year 2, and will include some online modules plus the in-person sessions to cover all of the functions you will have.

5 P a g e 5 Continued from Page 1 4. Outside electives there must be central oversight of external electives. If an elective is arranged through VSAS (the visiting student application system), there is no additional hurdle to jump. However, if you are taking a non-utmb, non-vsas elective, the proposed experience must be reviewed and approved in advance by the Electives Committee and there must be a legal affiliation agreement between UTMB and the institution you plan to visit. This process may take as long as 6 months. Some institutions already have signed affiliations with us for example, John Peter Smith Hospital in Fort Worth. Therefore, electives there take 1-2 months to arrange because they only need approval by the Electives Committee. Military electives, however, require an affiliation agreement for each student (their requirement, not ours) so it is a painstaking process. Moral of this story: plan ahead. 5. Student mistreatment we have a higher than average reporting of mistreatment on the Graduation Questionnaire, (a survey completed by 4th year students between February and June of their final year). This was especially noted for student report of sexist comments. Sadly, the reported source of sexist comments was nearly evenly split between clerkship faculty and other medical students. We are distributing hangtags to add to your badge that help define mistreatment and provide the avenues for reporting it. (If you haven t gotten one, we have them in Graves) We have put on workshops to educate faculty and students about this issue. Posters will be placed in small group rooms and team rooms with the same information. We stressed the importance of completing the Graduation Questionnaire, and will continue to do so. The workshop stressed the importance of reporting mistreatment we can t fix it unless we know about it. We all have a responsibility to treat each other with respect sexist (or racist) comments are hurtful, and do not demonstrate the kind of compassion we expect from future physicians. 6. Affiliation agreements despite valiant efforts, we have not been able to secure a legal affiliation agreement with St. Luke s Hospital in Houston since the hospital was sold to a new owner. We will continue to work on this and have hopes that it will be completed this academic year. Until we have that in place, we cannot assign students to St. Luke s this mostly impacts Internal Medicine and Neurology.

6 P ag e 6 New combined MD/MBA program Both sponsoring institutions are equally invested in your success. The MD degree is awarded by UTMB and the MBA degree is awarded by University of Houston-Clear Lake. We are pleased to announce that a new combined degree program, an MD/MBA, has been established with UTMB and the University of Houston Clear Lake. What are the key aspects of this dualdegree program Both sponsoring institutions are equally invested in your success. The MD degree is awarded by UTMB and the MBA degree is awarded by University of Houston- Clear Lake. The MBA portion of your education is the same full 54 semester-credithour (SCH) curriculum pursued by traditional single-degree MBA candidates it is not an abbreviated, less rigorous or executive MBA program. There are also healthcare oriented electives to allow the course of study to reflect business issues unique to healthcare. Similarly, your MD curriculum is the same medical curriculum taken by single-degree students at UTMB; electives allow substantial flexibility in tailoring the precise experience to your needs. How do my medical and business studies interface Your will enroll in separate and distinct courses for each degree. You are never taking School of Medicine (SOM) and Business School courses concurrently. However, flexibility is enhanced by the availability of all business degree courses being available both in-person on the UH- Clear Lake campus and online, the possibility for credit for business courses taken at the undergraduate level, and the opportunity to earn credit toward both the MD and MBA for specified courses. With careful planning, both degrees can be completed in as little as five years. Some students may require longer. What are the enrollment and cost implications of the dual-degree program Students accepted or enrolled in the School of Medicine make a separate application to the Business School upon approval of the Program Committee. The MCAT is accepted in lieu of the GRE as an application requirement. A customized schedule plan is developed for each student. The costs for each degree (tuition plus fees) are the same as for students who propose each degree separately. Financial aid, subject to normal qualifications, is available for both degrees. Please refer to the current degree cost information available at each school s website. What is the typical pathway to earn both degrees Students may apply to the Program upon acceptance to the SOM (Fall or Spring prior to matriculation) and if accepted, begin business school classes in the Summer term at UH-CL prior to beginning SOM studies. Alternatively, students already enrolled in medical school may apply as late as their 3rd year and if accepted, commence business school studies between their 3rd and 4th years of medical school by taking an approved leave from medical school. Students are never enrolled in UTMB and UH- CL courses simultaneously. Completing both degrees in five years is a rigorous course of study requiring discipline, careful planning, and judicious use of elective and vacation time.
