1 Ebla Cntact Mnitring Prtcl 3/2/2015 Ebla Virus Cntact and Traveler Mnitring Fllw-up Prtcl The fllwing guidance is t be used fr 21-day temperature and symptm mnitring fr persns wh have been identified as cntacts f an Ebla Virus Disease (EVD) case and fr persns wh have traveled t affected areas wh have either high, sme, r lw (but nt zer) expsure risks. Why is it necessary t mnitr temperature and symptms? Mnitring cntacts and travelers fr symptms f EVD will ensure that shuld a cntact r traveler becme ill, he r she will be quickly islated and transmissin mitigated. Cntacts f suspected r cnfirmed EVD cases shuld be mnitred twice daily fr symptms f EVD fr 21 days after the last date f knwn expsure. All travelers shuld be mnitred nce daily fr symptms f EVD fr 21 days after leaving the affected area. This is the lngest interval between expsure t EVD and the develpment f symptms. FORM INSTRUCTIONS Each day, recrd the cntact s/traveler s temperature and any f the symptms listed n the EVD Symptm Mnitring Lg: The date f the last knwn expsure t a ptential case is cnsidered day 0 and the date fllwing the last knwn expsure is day 1. Fr travelers, day 0 is the last day in the affected area and day 1 is first date utside f the affected area(s). Fill in the subsequent dates in the spaces under rest f the numbers. Fr example, if the date f last knwn expsure is Octber 3, 2014 mnitring wuld begin n Octber 4, 2014, and therefre yu shuld write 10/4/2014 under the number 1, and 10/5/2014 under the number 2, and s n. Tw symptm and temperature checks shuld be cnducted in a 24 hur perid at least 6 hurs apart. Fill in the time f symptm and temperature check fr the respective date f fllw-up. In the blank fr each symptm, type Y (reprted r bserved symptm) r N (did nt reprt r bserve symptm). D nt leave the space blank. Nte whether the cntact/traveler is taking aspirin, Tylenl (acetaminphen), r MOTRIN (ibuprfen r Advil ). Symptms (Y/N) and temperatures cllected during twice daily mnitring checks shuld be entered int Maven n a daily basis n weekdays using the cntact/travel mnitring wizard. Weekend mnitring results can be entered int Maven n Mndays. DPH staff will mnitr temperature/symptm lgs entered in Maven and wrk with LHDs t assure cmpleteness and timeliness f entries. LHDs will be cntacted if mre than 48 hurs pass withut an entry. MONITORING TYPE INSTRUCTIONS Fllw the instructins fr the specified type f mnitring (active r direct active). 1) Active mnitring: Telephne cntact nce per day An initial hme visit by LHD staff within 24-hurs f ntificatin by DPH f the traveler s arrival is strngly encuraged/recmmended. At this visit, LHD staff shuld instruct/bserve prper use f the thermmeter by the traveler and explain the applicable mnitring and mvement restrictins. Call the cntact/traveler nce daily at a pre-arranged time t assess fr signs/symptms f EVD 1 n that mrning and the previus evening Ask the cntact/traveler abut their health (feverish, verall general health). and/r texting can be used in lieu f a phne call if the traveler demnstrates cnsistent reprting at the pre-arranged time via ne f these methds. 1

If the cntact/traveler reprts a fever 2 call DPH Epi Prgram (days: , evenings: , Ebla direct line ) as sn as pssible. If the cntact/traveler des nt reprt a fever 2 cntinue with the full symptm assessment. If a cntact/traveler reprts ne r mre symptms (nt including fever), inquire abut pssible explanatins fr the symptm. Fr example, if the cntact reprts muscle aches but states aches are typical f thse they experience after exercising type Y and make any ntes in the space prvided n the frm. If n reasnable alternative is prvided fr the reprted symptm, call DPH Epi Prgram (days: , evenings: , Ebla direct line ) as sn as pssible t discuss need fr frmal medical evaluatin and EVD testing. If the cntact/traveler exhibits symptms indicative f EVD, the cntact is nw classified as a Persn Under Investigatin. Inability t reach active mnitring cntact/traveler: Fr the daily mnitring call, three attempts shall be made t reach the cntact/traveler at their primary and secndary telephne numbers. If the cntact des nt respnd via telephne t the daily call, cntact the DPH Epi Prgram (days: , evenings: , Ebla direct line ) t discuss the need fr an in-persn visit and/r law enfrcement welfare check. 2) Direct active mnitring: In-persn cntact nce per day and telephne cntact nce per day. An initial hme visit by LHD staff shuld ccur within 24-hurs f ntificatin by DPH f the traveler s arrival. At this visit, LHD staff shuld instruct the traveler n prper use f the thermmeter and f the applicable mnitring and mvement restrictins. Schedule subsequent in-persn visits with the cntact/traveler at a pre-arranged times each day t btain visual cnfirmatin f temperature readings and t assess fr symptms f EVD 1. The acceptability f using secure vide cnferencing methds (see Appendix 1) in lieu f in-persn visits will be determined n a case-by-case basis in cnsultatin with DPH. Physical cntact with the persn under mnitring shuld be avided. Entering the hme is nt required. Temperature/symptm assessments can be cnducted at the drway. Temperatures can be taken by the individual being mnitred and the temperature reading can be dne frm a distance f 3 feet frm the cntact/traveler. A call t the cntact/traveler shrtly befre arrival at the hme is suggested t assure the individual is hme and is well. The secnd daily mnitring check can be cnducted either in-persn r via phne. The acceptability f vide cnferencing (see Appendix 1), texting, and/r will be determined n a case-by-case basis in cnsultatin with DPH. Fr example, if the cntact reprts muscle aches but states aches are typical f thse they experience after exercising type Y and make any ntes in the space prvided n the frm. If n reasnable alternative is prvided fr the reprted symptm, call DPH Epi Prgram (days: , evenings: , Ebla direct line ) as sn as pssible t discuss need fr frmal medical evaluatin and EVD testing. If the cntact/traveler exhibits symptms indicative f EVD, the cntact is nw classified as a Persn Under Investigatin. DPH NOTIFICATIONS In additin t situatins described abve, cntact the DPH Epi Prgram (days: , evenings: , Ebla line ) if any f the fllwing ccur: Any new r updated infrmatin n travelers being mnitred including changes t infrmatin cllected n initial interview including details f the traveler s dates and/r activities in the affected area. The traveler indicates plans t travel t anther state and remain vernight OR f plans t travel ut f the U.S. during the mnitring perid. Obtain date(s) f intended travel, mde f transprtatin, and address in intended destinatin. The traveler indicates a change in previusly reprted dates and/r lcatins f intended travel ut f state r cuntry. DPH will send bth US and Internatinal travel ntificatins t recipient lcatins that include: dates f travel, mde f travel, destinatins (including lcal address), and status f active mnitring. 1 Symptms f EVD include fever 100.4, severe headache, muscle pain, weakness/fatigue, diarrhea, vmiting, abdminal (stmach) pain, unexplained hemrrhage r bruising 2 The temperature threshld fr requiring medical evaluatin is F/38 C. Hwever, medical evaluatin may be recmmended fr lwer measured temperatures r subjective fever based upn expsure level and clinical presentatin. 3

4 Ebla Cntact Mnitring Prtcl 3/2/2015 APPENDIX 1 Prtcl fr Using Vide Telecnferencing fr Direct Active Mnitring f Cntacts and Travelers ~ Ebla Virus Disease Backgrund In Cnnecticut, active mnitring cnsists f telephne cntact nce daily at a pre-arranged time t assess fr signs/symptms f Ebla Virus Disease (EVD). Direct active mnitring requires in-persn cntact nce per day and telephne cntact nce per day t btain at least ne visual cnfirmatin f temperature readings and t assess fr symptms f EVD. An initial hme visit by lcal health department (LHD) staff typically ccurs within 24-hurs f ntificatin by DPH f the traveler s arrival. Fr cntacts r travelers requiring direct active mnitring, subsequent in-persn visits can be scheduled with the cntact/traveler at pre-arranged times each day. The acceptability f using secure vide cnferencing methds in lieu f in-persn visits can be determined n a case-by-case basis in cnsultatin with DPH. In-persn direct active mnitring can be time-cnsuming and cstly, especially in rural cmmunities where a LHD wrker may have t travel a significant distance fr hme visits. It may als be incnvenient fr cntacts/travelers that need t be accessible t the LHD wrker during daily visual temperature and symptm checks. Vide technlgy that is secure (HIPPA cmpliant) prvides an innvative and effective means t verify cntacts /travelers thermmeter readings.. Vide Telecnferencing tls fr use in Cnnecticut The fllwing prtcl describes a vide cnferencing tl, Vsee that is apprved fr use in Cnnecticut fr EVD direct active mnitring f cntacts and travelers. LHD staff can bserve cntacts/travelers taking their temperatures remtely via a smartphne, tablet, r cmputer. Als apprved fr use is FaceTime, an applicatin available n Apple devices (iphne, ipad, ipd Tuch r Mac cmputers). FaceTime cmes preladed n these devices and requires that bth the LHD and cntact/traveler being directly mnitred have cmpatible devices. Instructins fr using FaceTime can be fund at: VSee Vide Telecnference Instructins Belw yu will find help t yu get started with the Vide Cnference Sftware setup n yur hme PC r Laptp. If yu are using a desktp cmputer, a web Camera will need t be utilized in rder t cnduct a vide call. If yu are using a laptp, the internal web camera will be sufficient t cnduct the vide call.
