2 EAST BAY INNOVATIONS Independent Living Services Living on your own with a focus on attaining maximum independence. ability to recognize and respond to emergencies independently. Typically, ILS offers up to a maximum of nine hours per week of teaching and support when a person first moves into their own home. Service hours decrease over time as the person grows towards independence. Teaching and support services focus on the areas that each person needs and wants to live well on their own. Service areas can include: meal planning, grocery shopping, cooking, household maintenance, scheduling and attending medical and dental appointments, finding affordable and safe housing, finding compatible roommates, utilizing public transportation. Independent Living Services also provide: Assistance with public benefit issues (SSI, Social Security, Independent Living Services (ILS) provide support to people with developmental disabilities who wish to live on their own. To be a match for ILS, people need to be able to demonstrate an MediCal, & MediCare). Assistance in finding medical, dental and (if needed) mental health providers. Ideas that Empower

Supported Living Services (SLS) assist people with developmental disabilities to live on their own, serving people who need more than nine hours per week of support and as much as 24 support hours per day. SLS provides access to a 24 hour emergency system for people who cannot respond to emergencies independently. SLS covers assistance in all the same areas as ILS and provides assistance so that people can pursue hobbies, interests and community activities. SLS provides personal attendant services for people who need help with physical and/or personal care. Supported Living Services also provide: Assistance with public benefit issues (SSI, Social Security, IHSS, MediCal, & MediCare). Assistance in finding medical, dental and (if needed) mental health providers. Assistance in finding and securing affordable, accessible and safe housing. Assistance in finding suitable housemates. 4 EAST BAY INNOVATIONS Employment Services Starting a career with a focus on learning to be a successful employee. Supported Employment Services assist people with developmental disabilities to become successfully employed in businesses in the community. EBI helps people identify the type of employment they desire and then networks with local employers to develop a match. Once hired, EBI provides one on one on-the-job training and support to assist the person to be successful in their work. Even after the individual is working independently, EBI continues to follow-up with the employee and employer to ensure continued success. One of EBI s newer program components, Project SEARCH, is a pathway to Supported Employment, providing intensive training and internships for individuals with developmental disabilities. Employment Services also provide: Resume writing. Interview preparation. Assistance in learning public transportation. Assistance in understanding Social Security work incentives. Ideas that Empower

5 East Bay Innovations Established in 1994, East Bay Innovations (EBI) is a private non-profit organization providing services to people throughout Alameda County. East Bay Innovations offers a variety of services to help persons with disabilities live and work independently in their communities. Our Mission EBI s mission is to arrange and provide personalized support that enables individuals with disabilities to live in their own homes, work in jobs of their choosing, and feel a sense of membership in their community. Philosophy We strive to understand each person s hopes and aspirations as well as needs for support. We work in partnership with the person and their close friends and family to provide services that reflect what the individual wants. The people we serve direct their EBI services as much as they can. We help people strive for independence when possible, and we provide assistance when independence is not possible. We believe in services that are flexible and change over time as people change. 6 How to Get Started with EBI People considered to have a developmental disability by the State of California may be eligible for EBI s Supported Living, Independent Living and/or Supported Employment Services. To establish eligibility, call the Regional Center of the East Bay for more information.
