1 Liberty Direct A complete health coverage plan For more information or to apply, visit LibertyDirect.org or Health.org What is Liberty HealthShare? Liberty HealthShare is a medical cost sharing organization. While Liberty Direct is not health insurance by definition, it exempts members from the Affordable Care Act tax penalty and offers less expensive and better health coverage compared to typical health insurance plans. What is Healthcare & Counseling Center? Healthcare & Counseling Center is a membership-based family practice clinic. at includes unlimited office visits, same day appointments, at-cost medications and labs, discounted referral and specialty care services, 24/7 on-call access to a physician and much more. How does it work? We have combined Liberty HealthShare and Healthcare & Counseling Center to give you Liberty Direct. With Liberty Direct you pay a monthly share amount. Liberty HealthShare places this money into your secure, online account called a ShareBox. For each month you do not have a shared medical expense, money from your ShareBox is directed to the ShareBox of another member that does have a medical need. When you have an eligible medical expense, funds are directed to your ShareBox from other members in our community. When those funds are received, Liberty HealthShare sends payments from your ShareBox directly to your providers. You then receive an Explanation of Sharing that verifies payments have been made to each of your providers. Typical health insurance plans have a deductible AND an annual maximum out of pocket amounts sometimes making it confusing how much more you will be spending above your monthly premium amount for medical care. Liberty Direct is different Liberty Direct has one simple amount you are responsible for paying before your benefits begin: an Annual Unshared Amount, which is $500 for individuals, $1,000 for couples, and $1,500 for families. Once you reach this amount, 100% of your eligible medical needs are shared up to $1,000,000 per incident with no lifetime limit (and if something major happens, that $1,000,000 limit resets after 45 days!). So the most you ll pay above the monthly amount is $500, $1,000 or $1,500, depending on the number of family members in your plan. over
2 What are the costs? By signing up with Liberty Direct (a plan offered through Liberty HealthShare), membership with Healthcare & Counseling Center is discounted. Liberty Direct Monthly Share Amount Healthcare Monthly (including discount) Total Monthly Cost For those with youngest spouse under 30 Individual $149 $35 (normally $95) $184 Couple $249 $100 (normally $180) $349 Family $399 $100 (normally $200) $499 For those with youngest spouse over 30 Individual $199 $35 (normally $95) $234 Couple $299 $100 (normally $180) $399 Family $449 $100 (normally $200) $549 Annual unshared amounts Individuals: $500 / Couples: $1,000 / Families: $1,500 Once you reach your annual unshared amount, 100% of your eligible medical needs are shared up to $1,000,000 per incident with no lifetime limit. ACA (ObamaCare) Exemption The Liberty Direct program is administered by Liberty HealthShare, a 501(c)3 non-profit organization that is recognized by the US government as a health care sharing ministry. As a member of Liberty HealthShare, you are exempt from the requirement to have health insurance, and therefore do not owe any tax penalty. Qualifying for Liberty Direct Built on the foundation of the time-honored, Christian tradition of medical cost sharing, Liberty Direct is comprised of a community of people who value and prioritize their health and who share a desire to help others do the same. The enrollment process provides one of the important ways in which costs are controlled in the program. Once you are a Liberty Direct member, you can never be expelled from the program, and your Monthly Share Amount cannot be changed because of your health status.
