3 IMPORTANT INFORMATION 1. Eligibility : Before filling the Application Form, the candidate should ascertain whether he/she fulfils all the requirements and is eligible for admission to M.Sc. Nursing Course as on the last date of submission of application. The conditions of eligibility are mentioned in the notification by Govt. of Chhattisgarh. Candidate s Responsibility: Candidates who appear in the test would do so at their own risk and responsibility so far as their eligibility is concerned. Hence, candidates are advised to ascertain their eligibility before applying. 2. Examination City : The test will be conducted at Raipur only. Center: Pt. J. N. M. Medical College, Raipur (C.G.) 3. Availability of Application Form : (a) The Application Form for the test is part of this posting. The two pages of the application form should be downloaded and printed on A-4 Paper by the candidates. The completed application form along with enclosures should be submitted at the office of C.G. Ayush and Health Sciences University Raipur. It should be accompanied by demand draft for Rs. 1400/- (For SC/ST/OBC) or 1800/- (For all others) as the case may be in the name of Registrar, C.G. Ayush & Health Sciences University, Raipur payable at Raipur OR paid in cash at the University Counter. 4. Last date for submission of Application Form : Duly filled application form must be sent by registered post/speed post or hand delivered so as to reach C.G. Ayush and Health Sciences University Raipur or submitted in person at C.G. Ayush and Health Sciences University Raipur on or before PM on C.G. Ayush and Health Sciences University Raipur shall not be responsible for any postal or other delay in receipt of application form. Application forms received after due date and time will be summarily rejected. 5. In-Service candidates too shall have to submit their application forms as mentioned at S. No. 4 after getting it forwarded through proper channel. 3

4 PRE P.G. (NURSING) ENTRANCE EXAMINATION IMPORTANT INSTRUCTIONS AND RULES FOR CONDUCT OF TEST The information given in the Application Form will be verified at the time of counseling/admission from the original documents by the counseling/admitting authority. If it is found later that a successful applicant has provided incorrect or false information or withheld some relevant information while filling the Application Form, the applicant shall be debarred from admission or her/his admission may be cancelled at any time during the course of his/her studies, by the head of the Institution/Admitting Authority/ C.G. Government, as the case may be without giving him any notice. 1 DOCUMENTS TO BE ENCLOSED ALONG WITH THE APPLICATION FORM As per check list provided on page no PROCEDURE FOR FILLING APPLICATION FORM- (i) Please fill the Application Form using blue/black ball point pen only. (ii) Please paste firmly 3.5cm x 4.5 cm colored photographs of good quality with white background, carrying the name & date of birth of candidate at the specified place in the Application form and Admit Card. Do not staple the photograph. Do not get it attested. Do not sign on the photograph. (iii) Make all entries in the boxes ( ) with blue/black ball point pen only. All entries must be made in English Capital letters by writing one alphabet in each box leaving one box blank between two words. 3 SCRUTINY/REJECTION OF APPLICATION FORMS Only the application forms received in C.G. Ayush and Health Sciences University, Raipur till the time mentioned on the last date of submission will be scrutinized. Application forms will be scrutinized on the basis of the information filled in. After selection, the same will be verified by the concerned authority at the time of counseling from the original documents. After completing the application from please go through the check list carefully. Any incomplete application in any form will be rejected. 4

5 4 INFORMATION REGARDING ACCEPTANCE: (i) Date and Time / PM (ii) The information of accepted application forms will be made available on the website of the C.G. Ayush and Health Sciences University, Raipur. 5 TEST ADMIT CARD (TAC): The Test Admit Card (TAC) will be sent to the candidates by Speed Post by the University to the address mentioned in the application form and Admit Card. It is in the interest of the candidate that the correct and full postal address should be filled in the Application form and Admit Card. In case the TAC is not received by post, any applicant whose application has not been rejected, can get a duplicate TAC issued by the center superintendent at the Centre of Examination between 9:00 and 9:30 AM on the day of examination by giving his or her name as written in the application form. No request will be entertained after 9:30 AM. Please note that a candidate will not be permitted to appear in the examination if he/she does not possess the TAC. IMPORTANT C.G. AYUSH AND HEALTH SCIENCES UNIVERSITY, RAIPUR RESERVES THE RIGHT TO CANCEL OR WITHDRAW ANY TEST ADMIT CARD ISSUED BY IT. 6 MATERIALS TO BE CARRIED IN THE EXAMINATION HALL (i) Test Admit Card (TAC) (ii) Blue/Black ball point pen No other material such as cellular/mobile phones, calculators, log tables, stray papers etc. should be found in the possession of the candidate in the test hall, otherwise the candidate is liable to be registered under Unfair means (UFM) and may be refused the chance to appear in the test. 7 UNFAIR MEANS (UFM) Candidates are advised not to bring mobile phones to the examination center. If any candidate is found using unfair means during the examination, which includes cellular/mobile phones, calculators, log tables, stray papers etc., referring to a book/note book/loose sheets, talking, giving assistance or seeking/receiving help from any source, indulging in any malpractice or misbehavior in any 5

6 manner in the test hall, harassing or doing harm to other candidates or invigilator or supervisory staff or if any action of the candidate is interpreted by the Observer/Centre Superintendent/ invigilator as amounting to adopting unfair means, a case will be registered under unfair means and shall be legally dealt with accordingly. The answer sheet of the candidate booked under UFM shall not be valued and his/her candidature shall be cancelled. Additionally, any case of use of unfair means on the part of the candidate may be handed over to the police. Criminal proceedings shall be initiated against such candidates. 8 NATURE OF TEST The entrance test shall be of objective type questions only, where four answers/responses will be provided for each question, out of which only one will be correct. The candidate is required to select correct answer and mark the corresponding option (A or B or C or D) on the OMR answer sheet using Blue/Black ball point pen only. 9 MARKS AND TIME ALLOTTED FOR QUESTION PAPER The Question paper will contain 100 questions. Total duration of the test will be of 2 hours 15 minutes, out of which the first fifteen minutes are allotted for making the entries in the OMR answer sheet and inspecting the question booklet for any printing or other errors etc. The candidates must ensure that the question paper is complete with 100 questions. 10 SYSTEM OF VALUATION (i) In objective type questions, two marks will be awarded for each correct answer. No mark (zero mark) will be awarded for an incorrect response or multiple marking or for an unanswered question. There shall be no negative marking. (ii) Marking will be done through electronic means. There is no provision of retotalling/revaluation or scrutiny. 11 DEFECTIVE QUESTION, THEIR CANCELLATION AND AWARD OF MARKS IN LIEU OF CANCELLED QUESTIONS University gets each question of the paper examined by a group of subject-experts along with the answer suggested by the paper-setter and moderator. If any question is found defective by these experts, the question may be cancelled due to:- (i) Wrong structuring; 6

7 (ii) (iii) (iv) More than one option being correct; None of the options being correct; Any printing error leading to a situation where more than one answer may be correct or none of the answers may be correct. The decision of the expert panel regarding the question and the correct answer will be final. In lieu of the cancelled questions, every candidate may be awarded marks in proportion to the marks obtained by him/her, irrespective of the fact whether the candidate has attempted such cancelled questions or not. For example if 2 questions out of total 100 questions are cancelled in a particular question paper and if each correct answer carries 02 marks, then after evaluation it is found that the candidate has secured 90 marks from the remaining 98 questions considered for evaluation purpose, then his/her marks for 100 question are computed as under. 90 x 100 / (100-4) = Note: All calculations shall be up to two decimal places and will not be rounded off. 12 DECLARATION OF RESULTS University will prepare the merit list as per the admission rules mentioned in notification of Government of Chhattisgarh. The results shall be available on the website of the University 13 MARK SHEET University will not send individual mark sheets to the candidates. A candidate is required to download it from the website of the University using his/her Roll Number. 14 DUPLICATE MARK SHEET There is no provision for issue of mark sheet. A candidate can obtain any number of copies as described in #13. 7

8 15 CANCELLATION OF ADMISSION If it is found that a candidate has succeeded in obtaining admission on the basis of false/incorrect information of withholding relevant facts, during counseling or at any time after admission, the admission so granted to the candidate shall be cancelled without any notice, at any time even during the course of his/her studies, by the Head of the Institute/Admitting Authority concerned or Govt. of Chhattisgarh. 16 The responsibility of C.G. Ayush and Health Sciences University, Raipur is confined to the extent of conducting the Entrance Test, declaring the merit list and forwarding it to the Director, Medical Education. The C.G. Ayush and Health Sciences University, Raipur shall be final authority for deciding all policies regarding conduct of the Test. If any question relating to the interpretation of Rules & Instructions for the conduct of Pre P.G. (Nursing) arises, it shall be referred to the Government of Chhattisgarh whose decision shall be final and binding. The University reserves the right to amend any rule/procedure for the conduct of the Entrance Test and any modification so made shall be binding. 17 Any kind of correction in information submitted in the Application form for any reason is not allowed/ accepted after the examination results have been declared. 18 JURISDICTION In case of any legal disputes, the jurisdiction shall be limited to the city of Raipur only. 8

9 CHECK LIST Before submitting the Application form at Chhattisgarh Ayush and Health Sciences University, Raipur cross check with the check list mentioned below: Check the following entries and tick ( ) in the box in case it is found correct. 1. Have you filled the required entries in the boxes ( ) with blue/black ball point pen. 2. Have your pasted your recent colour photograph at the two specified places in the application form? 3. Have your put your signatures at the specified place in the application form & admit card? 4. In case of In-Service candidate, have you sent your Application Form duly forwarded. 5. Proof of date of birth- Marksheet of class 10 th Exam. 6. Mark sheets of B.Sc. Nursing OR B.Sc. Hons. Nursing OR Post Basic B.Sc. Nursing (All years) 7. Certificate of Work Experience 8. Caste Certificate (If applicable) 9. Disability Certificate from State Medical Board (If applicable) 10. Domicile Certificate (of C.G. or other State) 11. A candidate entering the examination hall after 15 minutes from the commencement of the test i.e. after 10:15 AM shall not be permitted to take the Test. Candidates must hand over the OMR answer sheet and the question paper to the invigilator at the end of examination, before leaving the examination hall failing which such cases shall be registered under UFM clause. The candidates are allowed only to carry away the carbon copy of the OMR answer sheet with them. Use of any kind of unfair means (UFM) (see IMPORTANT INFORMATION # 7) is strictly prohibited. 10
