2 The medical equipment and healthcare technology sector one This sector is comprised of the following subsectors: Diagnostics and laboratory: reagents, tests, enzymes, serology, bacteriology, parasitology, refrigerators and incubators. Hospital furnishings and equipment: articulated beds, instrument carts and examination tables. Orthopedics, prosthetics and implants: bone replacement, osteosynthesis, silicon templates and attachment devices. Single-use products: stethoscopes, speculums, tuning forks, tongs, separators, scalpels, chisels, gouges, probes, catheters and examination lamps. Electromedical and diagnostic imaging devices: electrocardiographs, scanners, audiometers, sphygmomanometer, tomography devices, x-ray and radiotherapy devices, incubators and defibrillators. Dental: implants, surgical instruments, centrifuges, biomaterial, dental equipment and instruments. Spanish sector overview two The European healthcare technology industry is made up of around 11,000 companies with sales of 72.6 billion, which provide employment to some 529,000 people, according to data from EUCO- MED (European Medical Technology Association). Spain holds an important place with a market size of 8.3 billion, directly and indirectly employing around 32,000 people. Along with Germany, the United Kingdom, France and Italy, Spain is one of the five countries that account for 75% of the European market. Germany and the United Kingdom together account for 44%, (28% and 16% respectively), followed by France (14%) and Italy (8.7%). The Spanish medical equipment sector is made up of 720 companies, of which 520 are manufacturers. As regards size, over 90% of the businesses in the sector are SMEs, although a small group of large enterprises (8%) accounts for 57% of the production value. 3 Through the major investment in R&D made by the sector (around 3% of total sales or 222 million), in recent years a large group of SMEs has achieved a high level of technology and product quality. This has enabled them to position themselves in international markets, even against multinational companies that dominate these markets, in subsectors such as electromedicine and surgical instruments. Geographically, production is concentrated in the regions of Catalonia (42%), Madrid (30%), Valencia (11%), the Basque Country (4%) and Andalusia (4%). An increasing number of patients are demanding access to the best products and for the latest technological advancements to be incorporated, which requires a greater research effort. Spanish healthcare spending represents 8% of the GDP, and this figure is expected to increase in order to cover the increase in demand. The Spanish healthcare system is comprised of 804 hospitals with a total of 160,981 beds. There are 345 hospitals that are managed privately, as well as an additional 120 private charitable hospitals. The public system has 339 hospitals, 107,745 beds and 2,702 health clinics. Over the past several years, a new management concession system has been implemented at hospitals, with private companies providing services to the public sector. There are several methods for acquiring medical equipment: public tender, direct purchase and acquisitions through leasing (equipment maintenance and replacement provided by the supplier), depending on the management model, the amount invested and the technology acquired. Top products Spain is the ninth country in the world ranking of healthcare product consumption. The diagnosis, orthopedics and single-use products sectors make up the majority of the Spanish market, with a 70% share. Electromedical products and medical furnishings and dental complete the products on offer in the Spanish market. 3

Critical factors -Heavy competition from more technologically developed countries such as the United States and Germany. -Heavy competition from developing countries, with lower prices. -High cost of updating new technologies. -Growing specialization in the products offered. -Restrictions in certifications. -Tariff and fiscal barriers (protectionism). -Need to use intermediaries abroad. -Low levels of price competitiveness, based on market and product. -Difficulty in finding quality Customer Support Service (CSS). -Sector with a great need for investment in R&D. Foreign investment in the sector The Spanish sector s production covers just 20% of domestic healthcare technology consumption. The remaining 80% is provided by some 1,200 companies, which include importing and exporting subsidiaries of major multinational medical and pharmaceutical groups (primarily German and American), other small and medium-sized importers and distributors and small transformation workshops, which means there is a very significant amount of foreign capital in the market. These companies focus primarily on the medical instruments, electromedicine and dental subsectors. The major presence of foreign capital is a challenge and a stimulus for the Spanish companies. Sector associations FENIN The Spanish Federation of Healthcare Technology Companies, created in 1977, is the business organization responsible for representing, promoting and defending the healthcare technology sector. FENIN covers 80% of the companies in the sector. Its members include manufacturers, distributors and exporters, both national and international. 4
