2 This conference commemorates the first century of workers compensation in New York and prepares for the challenges of the next 100 years. There will be educational sessions and an opportunity for dialogue among participants. Breakout sessions will discuss topics in specific detail, allowing a more specialized view, while other topics will be presented from a more general perspective. Who Should Attend? All employers, insurance adjusters, self-insurers, third party administrators, safety and human resource managers, claimant and defense attorneys, health care providers, mediators and medical and vocational rehabilitation providers are encouraged to attend. In fact, anyone interested in the workers compensation system in New York will receive valuable information at the conference. Registration Fee The cost for attending this informative conference is: $300 Early bird registration before June 1st $350 Registration after June 1st Registration fee includes conference materials and special functions, including one continental breakfast and one luncheon. Casual attire is appropriate for all functions. Location Conference sessions will take place at the Empire State Plaza Convention Center, Albany, NY. Continuing Education Credits Participants may be eligible for continuing education credits for conference attendance. CEU applications are pending for approval in the following disciplines: attorneys (CLE) and rehabilitation providers (CEU for CRC, CDMS, CCM). A certificate of attendance will be available upon request for other professional disciplines.

3 ATTENTION EXHIBITORS Fee of $800 includes two (2) complimentary registrations for exhibitor representatives. Two (2) additional registrants will be accepted at the rate of $250 each. A separate registration form should be completed for each additional registrant. Space is offered on a first come, first serve basis and will only be reserved upon our receipt of this registration form, which may be faxed to (386) or ed to iwcf@bellsouth.net. Teardown will be on Tuesday, July 15, 2014 at 4:00 pm. Refreshment Breaks will be held in the Exhibition area. Registration fee includes one 6 draped table and two chairs, registration for two representatives and security. Full fee forfeiture if unable to attend.
