1 Suffolk Sexual Health Service Job Description TITLE: GRADE: LOCATIONS: Senior Health Promotion Practitioner D Ipswich with frequent travel across Suffolk MANAGERIALLY RESPONSIBLE TO: Service Manager HOURS: 35 hours per week 1. Summary This job description relates to leading the delivery of a sexual health promotion and HIV prevention service targeted at vulnerable communities and high risk groups. The outreach sexual health service is delivered by THT under sub-contract to Cambridgeshire Community Services NHS Trust (CCS). 2. The Role The postholder will be responsible for the design and delivery of effective outreach health promotion (HP) and behaviour change communication (BCC) interventions appropriate to the needs of key target communities and groups. 3. Key Responsibilities 3.1 Leadership Lead the delivery of HP/BCC interventions as a senior practitioner, and manage the health promotion team working with vulnerable groups Encourage and facilitate a culture that involves/empowers staff, stimulating innovation, action learning, continuous service improvement and active participation in shaping developments and improving access to high quality service interventions Take an active role in self-development, objectively reviewing performance, and identifying and addressing personal development needs Provide training, information and guidance to staff across the service in order to ensure their knowledge, understanding and delivery of HP/BCC with different target groups is current and evidence-based Maintain a good working knowledge of sexual health promotion and HIV prevention needs amongst target communities Suffolk Sexual Health Service Terrence Higgins Trust 1

2 Ensure that interventions and activities are developed in line with current research and other THT frameworks and guidelines including Making it Count and The Knowledge, the Will and the Power. Ensure that interventions and activities are developed in line with current research and other THT frameworks and guidelines including Making it Count and The Knowledge, the Will and the Power. 3.2 Service Delivery Develop and deliver HP/BCC services in Suffolk targeting men who have sex with men (MSM) including men living with HIV through a range of interventions including gay venue, public sex environment and online outreach, one-to-one HP/BCC, skills development workshops and training in partnership with other sexual health services Align HP/BCC targeted at MSM with the delivery of the CHAPS programme in Suffolk and the East of England Design and deliver HP/BCC campaigns/ services in partnership with other organisations to target other sub-population groups vulnerable to HIV particularly sex workers and some migrant population groups Develop and deliver HP/BCC services targeting Black Minority ethnic communities particularly Black African populations through community engagement approaches that increase knowledge and awareness of HIV, increase HIV testing and highlight the benefits of early diagnosis and access to care and treatment Ensure that needs assessments are undertaken as necessary to increase understanding of the local needs of different target groups and inform service development and delivery Provide HP/BCC interventions integrated as appropriate with clinical service delivery in order to increase access to HIV/STI testing e.g. HIV Point of Care testing Deliver outreach to at risk communities (including condom distribution); in commercial and non-commercial settings and other community environments Provide face-to-face support to individuals with concerns about their sexual health, relationships, sex, and sexually transmitted infections including HIV Work with relevant colleagues to ensure development and implementation of HP/BCC services in accordance with THT s Clinical Governance and Quality Framework Liaise with THT national health improvement teams in order to ensure local interventions are informed by current learning and evidence-base from national HIV prevention programmes Initiate, design and deliver community development/engagement to improve relationships and services to at risk communities. take up of local integrated sexual health services including STI/HIV screening and contraception services Suffolk Sexual Health Service Terrence Higgins Trust 2

3 Provide training and capacity building to other professionals working within the NHS, local agencies and organisations in relation to developing effective interventions with vulnerable groups. Adhere to the information production policy and ensure all relevant health and social care information meets the requirements of the information standard. 3.3 Operational Support the Service Manager in the management of the sexual health service Work with the Service Manager to ensure activity performance is monitored, reviewed and action taken to address under- and over-performance Ensure the HP/BCC elements of the contract are achieved through ongoing monitoring of performance against agreed contract requirements and service outputs and outcomes Ensure a high quality of data entry to the sexual health/client service database and that service monitoring reports accurately reflect service performance Ensure that use of sessional staffing resources is closely monitored and regularly reviewed Work collaboratively with CCS to develop the commissioned services, adapting provision to maximise the delivery of client orientated, accessible and effective services Management and supervision of the HP/BCC staff including sessional staff and volunteers to deliver an efficient and effective high quality service Represent THT at health improvement meetings, networks and forums as appropriate Support the Service Manager in discussions with CCS and partners in the development of the sexual health service including through the production supporting reports and documentation GENERAL RESPONSIBILITIES All THT staff to: Promote and adhere to THT s Equality & Diversity, Data Protection, Information Governance and Confidentiality Policies and Code of Conduct Promote and maintain health and safety regulations, practices and conditions as per policies. Take responsibility for the security of buildings and their contents Attend meetings, conferences and undertake training, supervision and appraisals as appropriate Participate in promotional events e.g. Sexual Health, Condom Week, World AIDS Day, Fundraising and campaigning activities as appropriate.
