Prevost (pictured at right) and Senior Associate Dean Marsha Howell Adams (pictured below) occupy the highest offices in two of nursing s most prestigious organizations. Dr. Prevost serves as President of Sigma Theta Tau International and Dr. Adams was recently installed as President of the National League for Nursing (NLN). Prior to her appointment as Dean of CCN in August 2013, Dr. Prevost held positions at the University of Kentucky College of Nursing, Middle Tennessee State University and the University of Texas Medical Branch. Dr. Adams is Senior Associate Dean of Academic Programs at CCN and has served the College in key roles for 30 years. Both Drs. Prevost and Adams are well-recognized for their achievements in nursing. Dr. Prevost is a Robert Wood Johnson Executive Nurse Fellow. She received a Lifetime Membership Award from the American Association of Critical Care Nurses and the Kentucky State Award for Excellence from the American Association of Nurse Practitioners. Dr. Prevost has also received national and international awards for research from Sigma Theta Tau, the American Association of Critical Care Nurses and the Hospice and Palliative Nurses Association. Dr. Adams is recognized as an Academy of Nursing Education Fellow and a Nursing Academic Fellow by the American Association of Colleges of Nursing. She has been the recipient of the CCN Board of Visitors Commitment to Teaching Award, the University of Alabama National Alumni Association Outstanding Commitment to Teaching Award and the Alabama League for Nursing Lamplighter Award. 3 December 2013, January, February 2014 The Alabama Nurse Page 3 The President s Message Recalling a Century of Service & Looking to ASNA s Second Century Arlene Morris, EdD, MSN, RN, CNE During Oct , nurses from across the state gathered at the Montgomery Renaissance Hotel for ASNA s celebration of a Century of Service! Attendees represented a variety of practice settings, levels of practice, ages, geographic areas, and participation in ASNA at district and state levels. The variety was thrilling as lively discussions were held during both the Mable Lamb Continuing Education Day and around the events of the 100th House of Delegates. Governor Bentley congratulated ASNA on 100 years of service to Alabama nurses and citizens, relating continuing important roles of nurses in the current healthcare delivery system. Dr. Debbie Hatmaker, recently appointed Chief Professional Practice Officer of ANA, presented the keynote address, discussing challenges nurses face across all levels of practice. The Alabama Association of Nursing Students concurrent convention allowed interaction with practicing nurses and many vendors. At the Awards Dinner sponsored by Arthur L. Davis Publishing Agency, scholarships and awards were presented from ASNA and individual districts. Charlotte Wynn was recognized as ASNA s initial Outstanding Legislative Advocate, reflective use of her leadership of ASNA s legislative committee. The Lillian B. Smith Award was presented to Carthenia Jefferson. Diane Buntyn was named Outstanding Nursing Administrator, and Mary Beth Bodin received the Cindajo Overton award for Outstanding Nurse Educator. The award for Outstanding Health Care Organization in Alabama was accepted by a table full of staff and the administrator of Forest Manor. Amy Southern was named Outstanding New ASNA Member, and the inaugural Helen Wilson Leadership Scholarship was awarded to Chuquita Martin- Ramey. Excellence in nursing was apparent across all areas of the state! The president s address to the ASNA House of Delegates recounted both challenges and successes of The analogy of a tree conveyed ASNA s strength and vibrancy, continuing growth begun by nurse leaders over the past century. During the president s address to the students, soon to be new nurses were challenged to consider ways each could leave a legacy for nurses in Alabama to look back on during the next 100 years. An ASNA 100th Convention canvas depicted a tree on which names of past presidents formed roots, signatures of delegates (representing current members) comprised the trunk, and student signatures formed the leaves of new growth for ASNA. View it and other historical letters, documents, and photos at ASNA s office! ASNA s Centennial Birthday Celebration dinner was enjoyed by members and guests who relished reminiscence with past ASNA presidents and executive directors. A brief overview of ASNA depicted the initial charter in 1913, closely followed by Alabama s first Nurse Practice Act in Photos of nurses in each decade revealed the tremendous impact of Alabama nurse leaders across wars, natural disasters, and times of change, in settings including health promotion, acute and rehabilitative care. Each of the five districts provided a moving historical presentation of nurses actions across the state during ASNA s first century. The evening revealed efforts of nurses in ASNA to address issues of across the past 100 years. Delegates were asked to envision the next 10, 20 and 100 years of ASNA in writing. One stated, In 20 years, my son will be ASNA president and my daughter will be governor. With vision like this, ASNA is poised for strength! Both numbers and involvement are essential to produce new growth for ASNA to move into a new century. The profession of nursing in our state and the health of our citizens depend on it. Blue. Trying to juggle family (or relationships), work, finances and time can be exhausting! As I ve traveled around the state talking to nurses, most non-members have no strong objections to joining ASNA. They understand that many voices promoting and protecting the profession in Montgomery and D.C. are much louder than a few. And, when they see the value of benefits far exceed dues in real dollars the less than $25 monthly dues for ASNA/ANA is a non-issue. So why don t more nurses join? You guys can help me with this by ing me at edasna@alabamanurses.org and giving me your answer to that question. Until I get your input and absent a complete professional survey of RN s in the state, I m going to make an educated guess about why some do not / have not joined. (Although we are growing) Here are my assumptions: FOUR POSSIBLE OBJECTIONS AND ANSWERS TO THOSE OBJECTIONS: 1. It costs too much. Answer: Benefits far exceed dues in real $$$. 2. What can ASNA/ANA do for me? Answer: A bunch of things including professional development, making life-long friends, career networking, community service and legislative advocacy that can shape the future of your job! When you check out the benefits, you will see an abundance of costs savings on CE s, personal legal council (one hour FREE for any topic like your Will or issues such as job bullying or discrimination, family and personal stuff any topic). Heaven forbid but if you ever have to go before the ABN, your ASNA lawyer will research your case, represent you and go with you as you stand before the Board. That has happened. You would have ASNA FREE legal council for legal services that would otherwise cost you around $5,000! 3. I don t have time to participate. Answer: YOU CAN BE WHERE YOU CAN T BE AT THE SAME TIME! Here is what I mean by the somewhat weird title of this article. By becoming an ASNA/ANA member, you can support others who do have time to work in the trenches on your behalf. The best example I can think of is the traditional use of missionaries by churches and faith-based groups. Suppose a member of a church had a deep desire to feed the poor in a third world country, but they could not because of personal obligations. By virtue of being a member, The E.D. s Notes YOU CAN BE, WHERE YOU CAN T BE AT THE SAME TIME! that person can support missionaries that are out there feeding the poor. In a sense, the member can vicariously be where they can t physically be through their participation as a member. I know that being part of a professional organization is not equivalent to feeding starving people. It simply illustrates the principal of group power and representation. You can have an impact today even if your circumstances will not allow you TIME to be physically present at District meetings, parties, the state convention or legislative rallies. 4. I m just not interested. Answer:??? I have no answer for that one. Why someone would have no interest in promoting and protecting the profession is a mystery to me Maybe they ve just not thought about it. I ve met a number of nurses that didn t even know ASNA/ANA exists! MEMBERS CAN RELLY HELP tell them about ASNA/ANA and/or invite them to go with you to a meeting or a legislative rally When nurses know what s in it for them and what s at stake in the rapidly changing healthcare system for their profession, I believe they will be extremely interested. As an organization representing all specialties of nursing, ASNA has a proven track record (for over 100 years) of standing up for the profession. Since the passage of the first Alabama Nurse Practice Act in 1915, ASNA has worked to instigate/influence legislation and policy expanding the scope of nursing practice and promoting high standards (and higher rewards) for nursing. There are many examples over the decades of ASNA members sacrificing to help shape and preserve the sanctity of your profession, nursing. Remember, there was a time in our history when nurses were not even allowed to take blood pressure! A procedure that could only be preformed correctly by a doctor. Please consider joining. Many hands make light work The bundle of sticks, harder to break Chain is only as strong as it s weakest link How many clichés do you need! YOU matter more now than ever before. Conclusion: ASNA/ANA benefits exceed the cost of dues in real dollars not just coupons and discounts although we have those too. ASNA/ANA membership can do a lot for you, as pointed out above. TIME is not an excuse. Most members will be engaged and benefit professionally from networking, serving on committees and boards and developing life-long friendships. But, if you don t have time you can still BE WHERE YOU CAN T BE by simply becoming a member. Your fellow members, along with the ASNA staff, will represent your expressed interests and be a strong voice for nursing. Just do it! Joining is easy. Legal Corner Don Eddins, BS, MS, JD Admittedly I am biased, but in my opinion one of the most important advantages of membership in the Alabama State Nurses Association is the legal services program. The program is two-pronged one pertaining to licensure/job performance matters, while the other relates to consultation on a broad spectrum of legal issues. On the licensure/job performance part of the program, I should state at the outset that anyone can be negligent. Another way of stating that is that anyone can make a mistake. As your ASNA attorney, I have dealt with issues ranging from a nurse practitioner s failure to check the advanced practice box on the license form to mishandling of narcotics. I have dealt with a nurse practitioner s failure to have a collaborating physician after she forgot to update her form when a doctor retired, to lapses in judgment pertaining to charting of medications. Sometimes there was no mistake. I ve dealt with cases involving nursing school professors experiencing difficulty with a student who didn t make a good grade. In many cases, the family of a patient who did not have a good outcome needs someone to blame and turns to the nurse on duty. And even when there were mistakes made in the majority of the cases, the CRNP or RN who made the mistake was a good, competent, caring professional who just experienced a lapse in judgment. However, the Board of Nursing staff often does not share my admiration for the RN who has committed an error. Moreover, when the dedicated RN receives that dreaded letter from the Board informing him/her that his/ her license is subject to an investigation he/she normally is just about frightened out of his/her mind. That is where the nurse s membership in the Alabama State Nurses Association pays off. If your license is subject to an investigation before the Board and you were a member of the ASNA when the event complained of occurred, I will represent you before the Board of Nursing at no charge to you. My services come with your ASNA membership. Legal representation that would normally cost you $2,500 or $5,000 is yours without charge because of your ASNA membership. And the peace of mind knowing that you will have on your side an Association and an attorney who has dealt with all kinds of licensure issues is equally important to the monetary savings. To be eligible for the benefit, you must be a member of ASNA when the Board raises the issue with your license. The second part of the program is broad-based. Your membership in ASNA entitles you to an hour s consultation each year on any legal issue on which I feel qualified to provide a consultation. Nurses call me regularly about matters pertaining to their jobs, as well as all types other issues from real estate matters to estate planning. I always am pleased to share my limited legal knowledge with any ASNA member. There are many good reasons to join ASNA from continuing education opportunities, to our lobbying efforts, to professional networking, to member discounts The list is lengthy and cannot be exhausted here. This training focuses on the psychological and the internal nature of the nurse. It challenges the nurse to do the right thing no matter what, and meeting the expectations of those who rely on the nurse for their care. The expectations are that the nurse will meet or exceed the Standards of Care, State Nurse Practice Acts and Codes of Ethics. If the nurse violates the Standards of Care, there is a disciplinary process in place. It is the expectation of the public that nurses will make wise, ethical decisions in caring for the sick. It is always acceptable to seek advisement when in doubt. Collaboration is key in using any decision models to reach a desired outcome or goal. And if by chance, a wrong decision is made, right the wrong. There are three steps to doing that: 1. Claim the wrong 2. Apologize to the appropriate people, and 3. Make amends if possible LPN Corner Professional Conduct: The Role of a Nurse Placing the blame on circumstances is a cop out. The bucks always stop with the Nurse or person in charge, Doctor, Executive Director, RN or LPN. It is ok to show regret or shame, but unprofessional to blame others or deny your role in the wrongdoing. There are decision trees and models to use that can help with decision making, such as: The Stop Model S is for Standards of care as set forth by the Nurse Practice Act. T is for Thorough and challenges us to consider our decisions including, but not limited to, past history, rules spoken and unspoken, personalities and sheer facts. O is for Outcomes and reminds us to check and recheck our decisions for the best results. P is for Personal value, which guides us to make consistent decisions which reflect one s own character and personal values. Find your tool, and make it your Bible for the care you give. This applies to nurses and student nurses. With modifications this applies to our everyday lives, whether you are a nurse or have some other profession. Buckner DSN, RN, CNE, Leigh Anne Minchew, DNP, RNC, WHNP-BC, Erica Elkins-Little, BSN, RN This is the second in a multi-part series on the 80 x 20 Call to Action. Based on the 2010 Institute of Medicine landmark report on The Future of Nursing: Leading Change, Advancing Health, there have been nation-wide initiatives to support progress toward the goal of having the registered nurse workforce 80% BSN prepared by the year Following this report from the Robert Wood Johnson Foundation (RWJF) and Institute for Healthcare Improvement (IHI), individual states undertook initiatives to advance the goals. Some of these activities in Alabama are described in this article. This part discusses the magnitude of the challenge in Alabama, groups and changes that have begun to address these challenges, and resolutions by the Alabama State Nurses Association (ASNA) supporting the process. The Magnitude of the Challenge in Alabama Currently in Alabama we have over 68,000 registered nurses (ABON, 2013). It is estimated that less than 40% hold a BSN. That leaves over 27,000 nurses in Alabama who would need to complete the BSN by 2020 to meet the national goals. In Alabama colleges and universities participating in the American Association of Colleges of Nursing (AACN) survey of schools of nursing reported slightly more than 1100 RN-BSN students were enrolled (AACN, Data Report-Alabama). Those Alabama schools providing RN to BSN and/or RN to MSN programs are: Auburn University at Montgomery, Jacksonville State University, Oakwood University, Samford University, Stillman College, The University of Alabama, The University of Alabama in Huntsville, Troy University, Tuskegee University, University of Alabama at Birmingham, University of Mobile, University of North Alabama, and the University of South Alabama, (AACN, 2013). In addition other live and online programs are available within or out of state. Even if every Alabama school was at capacity, the need could not be completely met. The rate limiting factor right now is enrollment. RN/BSN/MSN tracks are actively recruiting and space is available! This is not an environment for competition but for collaboration. To that end national and statewide groups have formed to address the needs for educational advancement and the processes of curricular development supporting educational progression. Bridge programs build on prior learning and credentials as licensed RN s and provide for career advancement. Nationwide there are growing numbers available with 173 programs of RN to Master s and 646 programs of RN-BSN nationwide (AACN, 2012a). In graduate level curricula, baccalaureate course work is embedded in order to prepare students for graduate level courses and to meet accreditation standards. RN to MSN programs can lead to preparation for faculty roles and future doctoral study, potentially reducing the faculty shortage. Educational progression has been supported through a Joint Statement on Academic Progression for Nursing Students and Graduates by the American Association of Community Colleges (AACC), the Association of Community Colleges Trustees (ACCT), the American Association of Colleges of Nursing (AACN), the National League for Nursing (NLN), and the National Organization for Associate Degree Nursing (N-OADN) (AACN, 2012b). In 2012 a task force was created by AACN to address ways to create a dynamic curriculum, responsive to the needs of registered nurses. This task force studied, discussed, and made recommendations to give exemplars for definitions of practice experiences in RN to Baccalaureate Curricula (AACN, 2012c). The task force addressed ways to meet the expectation for varied practice opportunities to meet The Essentials of Baccalaureate Education for Professional Nursing Practice (AACN, 2008). Practice experiences were described as experiential learning in any setting where healthcare is delivered or health is influenced that allows for and requires the student to integrate new practice knowledge and skills (AACN, 2012c). Dr. Buckner served as a member of this task force, and in preparation consulted with colleagues in several Alabama universities to determine best practices in RN-BSN/MSN education. Practice experiences are typically integrated across RN-BSN/MSN experiences in several different courses. These may include courses in health assessment, leadership, evidence-based practice, and capstone practice integrated projects. Practice experiences include activities requiring collaboration with health care settings or agencies, and address health needs of individuals, families, and communities. Practice experiences require clear objectives, outcomes, and evaluation by a faculty member. Practice experiences often incorporate learning of leadership, organizational systems, and interprofessional communication. Examples were drawn from numerous examples given by task force members (AACN, 2012c), such as: Participate in interprofessional performance improvement team currently working on implementation/ evaluation of national patient safety goals. Evaluate the effectiveness of continuity of care by assessing the needs of an individual patient and family and then reviewing the discharge, referral, and case management processes and plans for that patient. Collaborate with the quality improvement department to implement an intentional rounds initiative. The student completed the initial training, created a training video of exemplary rounds, then served as a coach to others who were implementing this on several floors. Alabama colleges and universities have led the way in incorporating these type experiences with curricular programs built on the abilities of registered nurses furthering their career development. Nursing Education and the Alabama 80 x 20 Task Force In Alabama several initiatives are underway with one of these focusing specifically on curricula. Nurse educators have come together in an 80 x 20 task force to examine Contracted Clinical Educator Diabetes Trainer Experienced teaching patients and healthcare professonials to use injectable medication in individual or group settings Flexibility Monday through Friday to conduct trainings Licensed RN, NP, APRN, RD, PA or PharmD Communication and interpersonal skills to foster partnership with multiple audiences Apply directly to Inquiries please call Educate. Empower. Repeat. pre-nursing requirements to facilitate seamless articulation in nursing education. This task force began through the Capstone College of Nursing at the University of Alabama and is currently chaired by Michelle Cheshire RN, MSN. It engaged RN/BSN/MSN educators across Alabama to discuss issues related to curriculum and the Statewide Transfer and Articulation Reporting System (STARS) curriculum requirements (STARS, 2013). Institutions were encouraged to make every effort to reduce mandatory core requirements by optimizing credits, including AP and CLEP credits, while still complying with the 120 hours required by STARS. Consensus was sought to work toward a common set of prerequisites and acceptable transfer credits. Community college representatives were supporting of changes in prerequisites that would facilitate progression. Further meetings are planned to address additional issues confronting educational progression. University initiatives In several cases academic institutions have reduced total hours and increased flexibility. Educators have built curricula that address the specific nursing experience and educational background of the ADN-RN with a) flexible scheduling for online courses, repeated offerings, part or full time, concurrent enrollment in outstanding prerequisites, b) leadership and collaboration emphasis, c) activities focused on clear and relevant outcomes for career development, and d) senior placement and/or seminars for courses to stimulate engagement and professional development. Special curricular development has also been done replacing clinical practicum hours with practice integrated experiences as described above (AACN, 2012c). Specialized advising is available in many colleges with preliminary review done by professional advisors from unofficial transcripts. Financial Issues Affecting Progression Often the most significant barrier for nurses pursuing educational progression is financial. In a recent meeting of the Alabama Nursing 80 x 20 task force, financial concerns were discussed including: a) Students must be seeking a degree or certificate at the school for which they wish to obtain financial aid, b) Financial aid will only pay for classes that count towards their specific degree, c) Pell Grants will not apply after the student is working, d) There is a lack of RN- BSN scholarship money, e) Most scholarships require fulltime enrollment, and f) Students can only receive financial aid at one institution. Possible solutions include: a) establishing advising tracks for RN-BSN students, b) encouraging ADN graduates to continue immediately to BSN in order to take advantage of final year of eligibility Pell Grant eligibility, and c) establishing RN-BSN specific scholarships. Many hospitals offer tuition reimbursement or other financial incentives. Financial support may be available and professional support for educational progression is growing with employers seeking better preparation in hiring and promotion decisions. Nurses should also consider the financial incentives for BSN completion as some hospitals give a per hour differential which would affect decisions about pace of part-time study (Alabama Nursing 80 x 20 Task Force, Minutes of April 23, 2013). Alabama Health Action Coalition (AL-HAC) This coalition is headed by Kathleen A. Ladner and Carol J. Ratcliffe, both of the Alabama Organization of Nurse Executives and Alabama Nurse Leaders in Education and Practice, in collaboration with Jane Yarbrough, Manager, Blue Cross and Blue Shield of Alabama, and Lacy Gibson, Director of Human Resources, Alabama Hospital Association. The coalition has taken the lead to respond to the IOM call to create the Alabama Health Action Coalition (AL-HAC). This coalition seeks to transform health care through nursing in our state (AL-HAC, 2012). They have represented Alabama to the national Robert Wood Johnson Foundation supported Campaign for Action. They have a strong agenda for supporting nurses educational progression with congruency with Alabama Statewide Transfer and Articulation Reporting System (STARS), congruency with Southern Association of Colleges and Schools (SACS) requirements, and flexibility in allowing transfer of course credits (AL-HAC, 2013). This summer AL-HAC submitted a State Implementation Plan (SIP) proposal with the primary focus to increase the percentage of Registered Nurses in Alabama with BSN degrees (Figure 1). The plan contributes to meeting the IOM recommendations toward increasing educational preparation Professional Issues continued on page 7

7 December 2013, January, February 2014 The Alabama Nurse Page 7 Professional Issues continued from page 6 and removing the barriers to so that RN s and APRN s can function to the full extent of their education, experience and competence. The overall goal of the AL-HAC is to improve the health status of all Alabamians by increasing access to cost-effective, high quality through inter-professional and community collaboration. The AL-HAC project will be accomplished by four activities: 1) collecting, analyzing and tracking RN workforce data, 2) working with health care agencies to provide tuition support so AD graduates can enter a BSN program post-graduation, 3) work with state AD and BSN programs, schools and colleges to design seamless education progression pathways, and 4), design outreach programs to promote nursing as a career choice to churches, civic organizations, elementary, junior and high schools students. A central strategy to these four activities includes addressing geographic and diversity workforce needs (K. Ladner, personal communication, August 1, 2013). The ASNA 80 x 20 Resolution: Alabama s Future of Nursing 2012 and 2013 ASNA Resolutions At the ASNA 2012 Convention a resolution was offered by a member of the Inaugural Class of the ASNA Leadership Academy, Dr. Leigh Minchew. This resolution was for the creation of a Task Force from ASNA to move the 80 x 20 planning forward. The resolution lists some of the background supporting educational progression and identifies actions for ASNA to further awareness and facilitate achieving the 80% x 2020 goal (Figure 2). A similar resolution was approved in 2013 demonstrating ASNA s commitment to this ongoing process with one additional action item: That the ASNA will promote educational advancement throughout the districts to promote the highest quality of health care for the citizens of Alabama (ASNA, Convention Resolutions, 2013). Conclusion Through these and other initiatives, nurses and stakeholders in Alabama have joined forces with others committed to educational advancement for nurses. Already numerous hospitals and healthcare agencies are offering tuition reimbursement for RN-BSN courses. Graduates of RN-BSN/MSN tracts are encouraging their colleagues and friends to start the journey. From rural hospitals, to statewide professional organizations, the call to action is clear! Nurses must advance their educational level for the dynamic, collaborative, and engaged practice of tomorrow. Dedication The 2013 Alabama State Nurses Association (ASNA) Annual Convention was dedicated to two outstanding members, both of whom died in early They both became actively involved members from the day they transferred membership to the association. Janet Donoghue, BS, RN, SANE-A Janet s first and lifelong involvement in ASNA was with continuing nursing education committee. She was one of the most prolific activity reviewers on the Approval Committee. Along the way she became an ASNA President. Members of District 1 describe her as self-starter and passionate about everything she touched. She was a pioneer in the field of forensic nursing and became one of the first SART/SANE Nurses. She assumed leadership of the Alabama Coalition Against Rape and led the legislative campaign to make strangulation a felony in Alabama. In addition she was a leader in the ASNA Ethics and Human Rights Committee and became a champion for prevention of hazing and bullying. Janet was an active member of local, state and national Republican women organizations. She led the fight to have Alabama recognized as a strong political force at ANA with her involvement with the ANA Political Action Committee. In addition to all of this Janet remained in the PRN Pool at Huntsville Hospital in the Critical Care Unit. Despite all of her non clinical nursing activities, she never stopped working in the ICU. Janet was one of those nurses who talked the talk but also walked the walk. She will be missed. Helen Wilson, MSN, RN Helen moved to Alabama in 1989 and immediately transferred her membership and became actively involved in all aspects ASNA. She started as a committee member in the continuing nursing education Approval Unit, approving activities and later transferred to the Provider Unit and became the Lead Nurse Planner. For the past 15 years Helen was involved with the planning of every ASNA educational activity. She was actively involved at the Montgomery, Autauga, & Elmore County Nurses Society as well as District 5. She is credited with mentoring a number of aspiring members into leadership positions. She assumed leadership of the ASNA Environmental Task Force. Helen became known both nationally and internationally for her work with the ASNA Environmental Task Force. She also served as the ASNA parliamentarian. And along the way Helen became an Interim Executive Director at ASNA. Helen represented ASNA on many state-wide committees almost all dealing with wellness. She will be missed.
