3 The City of Halle Ministry of Health Department of Children s Health Frau Dr. med. Herrmann Why do we inquire about family environment? There is a clear connection between a child s family situation and its health. Your answers will help us provide individual support related to your child s particular situation. What will be done with the data collected? The collection, analysis and use of personal data are regulated by the law on the protection of citizens personal data (DSG-LSA), and subject to patient/physician confidentiality. The data collected serve the exclusive purpose of medical assessment, medical documentation and anonymous health-related reports of the state of Saxony Anhalt. Finally, one last request: To the school enrolment check-up, please bring the completed questionnaire, the child s vaccination card, the yellow check-up booklet, and if necessary your child s glasses. Submission of the vaccination card is strictly required, as prior to first entry in a public school, the Ministry of Health or responsible doctor must be informed of the child s vaccination status (SG SG 34 Abs. 11). Sincerely, Dr. med. Ch. Herrmann Paediatrician School paediatrician Consent to participation in survey according to 4 DSG-LSA (Law on the protection of citizens personal data) 12 March1992 The preceding personal data, collected in the context of a paediatric examination of my/our child, are at the disposal of the Medical Service for Children and Youth of the Ministry of Health. These data can be used by the Medical Service for Children and Youth exclusively for the purpose of medical documentation and anonymous health-related reports. I am aware of my right to revoke my consent at any time, to take effect from that point on. Furthermore, I am aware that it is also possible to submit social data to the examining doctor verbally, during the examination. Place, Date Signature of parent / guardian Fragebogen zur EINSCHULUNGSUNTERSUCHUNG (ENGLISCH) Parent Letter and Questionnaire: School Enrolment Check-up Dear parents / guardians, In the coming year, your child will be of school age. For this reason, the education law of Saxony Anhalt (1 August 2005 SchulG LSA 37 Abs. 2, GDG-LSA 9 Abs. 2) requires an obligatory medical check-up for your child. The following laws also serve as a legal basis for this check-up: the law on the protection of citizens personal data (12 March 1992 / DSG-LSA 9 Abs.1), the health services law of Saxony Anhalt (21 November 1997 (GDG-LSA 23 Abs. 1) and the Infection Protection Act (SG 34 Abs. 11). What is the purpose of the check-up? At the medical check-up, the school paediatrician from the Ministry of Health examines whether your child is developing in accordance with its age, from a medical point of view. In addition, school paediatricians advise parents and the school in questions of healthrelated development and, if necessary, special support for children. What happens during the check-up? Your child will be examined in your presence. The child s health status and physical, mental, psychological and social maturity are determined, and the senses are tested (eyesight test, hearing test). Should an impairment be discovered, it is recommended to do further tests, and measures can be taken to support the child. What do I need to bring? In preparation for the examination, you receive the attached questionnaire and are asked to bring the child s vaccination card and the yellow booklet documenting early medical check-ups (U-examinations). The responses to questions in section 5 family environment can also be discussed and submitted during the session with the doctor. What is the purpose of the questionnaire? All answers in this questionnaire are given voluntarily. The information helps in the analysis and classification of your child s diagnoses and development. You may already have experienced similar procedures in the context of your own medical examinations. If you choose not to answer questions, there is no disadvantage to your child.
