3 What s new for July 1, 2014? Changes to Our Plans Include: The Medical Plan HMO Office Visit copay will increase from $15 to $20 Routine vision exams once every 12 months Employees are responsible for the first $400 of the single deductible and for the first $600 of the family deductible of the medical plan. The Boston Conservatory will continue to fund the remaining $1,600 of the single deductible and $3,400 of the family deductible. Cigna will be the new Life, Accidental Death and Dismemberment, Voluntary Life, and Long-Term Disability insurer (replacing Lincoln Financial). Benefit Strategies will administer our HRA, Flexible Spending Accounts and Parking and Transit Accounts (replacing HRC). ING will change its name to VOYA. Eligibility for Our Plans Medical/Rx & Health Reimbursement Account: Full-Time benefit eligible faculty and any staff member working 1,820 hours per year who have fulfilled the 60-day waiting period. Dependent children are eligible up to age 26. Dental: Full-Time benefit eligible faculty and any staff member working 1,820 hours per year who have fulfilled the 60-day waiting period. Changing Legal Plan: Your Benefits Full-Time benefit eligible faculty and any staff member working 20+ hours per week for a full year. The only time you are allowed to make changes to your benefits other than annual open enrollment time is if you experience a Qualifying Life Event. For any Qualifying Life Event change, you must inform the Benefits Manager within 31 calendar days of the event. 4 Medical / Rx Plan HMO Blue New England Plan with Hospital Choice Cost Sharing You must choose a primary care physician (PCP) for you and each member of you your family from anywhere in New England. With this plan, the primary doctor is the one source of routine care and when a specialist is needed, your PCP will make any necessary referrals, anywhere in New England. Preferred Blue PPO This plan is only available to eligible employees whose primary residence is outside of the New England network (please see Benefits Manager for details). There are more than 5,000 dentists who participate with Blue Cross Blue Shield of Massachusetts. Dentists who participate with Blue Cross Blue Shield of Rhode Island and out-of-area dentists who participate in the DenteMax Network of Dentists are also available to Dental Blue members. Participating Dentists Dentists that participate with Blue Cross Blue Shield of Massachusetts, Blue Cross Blue Shield of Rhode Island, or out-of-area dentists that are in the DenteMax Network of Dentists, accept the lesser of either the dentist s actual charge or the allowed charge as payment in full for covered services. You pay only your deductible and co-insurance (if applicable), and charges beyond your calendar-year or lifetime benefit maximum. Non-participating Dentists Inside of Massachusetts Your group has also purchased supplemental coverage to provide benefits for covered services furnished in Massachusetts by non-participating dentists. You may be responsible for the deductible and co-insurance (if any), any difference between the maximum allowance and the dentist's actual charge, and all charges beyond your calendar-year benefit maximum. Non-participating Dentists Outside of Massachusetts Benefits for covered services by a non-participating dentist outside of Massachusetts are provided based on the dentist s actual charge or the allowed charge, whichever is less. The allowed charge is based on a schedule of charges. You may be responsible for any difference between the dentist s actual charge or allowed charge, whichever is less. You are also responsible for your deductible and coinsurance (if applicable), and charges beyond your calendar-year or lifetime benefit maximum. Accumulated Maximum Rollover Benefit This dental plan includes an Accumulated Maximum Rollover Benefit. This rollover benefit allows members to roll over a certain portion of their unused annual dental benefits so that they can use them in a future year. Pre-Treatment Estimate: Before beginning extensive dental work, it is STRONGLY recommended that you have your dentist obtain a pre-treatment estimate from the insurance company. 9 How to Locate a BCBS Provider Doctor, Hospital/Facility or Dentist Go to Enter as a Member and click on this box ) Start here if you are looking for individual doctors, dentists, eye doctors, behavioral health and substance abuse professionals, or other health care providers. 2) Begin here to search for hospitals, freestanding labs and imaging centers, x-rays/radiology, MRIs, MinuteClinic, ambulance services, durable medical equipment suppliers, physical therapy groups, or other specialty care service providers. Keep in mind: all fields on this screen must be completed in order to proceed to the next step. 3) Know your health care provider s name? Start here. Leave out Doctor and type only his or her name and location. A drop-down menu will appear and show you provider names that match the letters typed. If you are unsure of your doctor s full name or the correct spelling of his or her name, you can also search by specialty to help you identify the specific doctor you are searching for. 4) Identify your plan or plan s network in this drop-down menu. This will provide you with a list of professionals and facilities (depending on your search) that participate in our plan. If you are using the public Find a Doctor site (and not signed in to member central), be sure to verify the name of our Plan HMO Blue New England with Hospital Choice Cost Sharing. 10 Health Reimbursement Account As seen on pages 2-4, our Medical Plan has a deductible that applies to certain inpatient and outpatient services. The Boston Conservatory pays for the majority of this deductible for you through a Health Reimbursement Account (HRA). This year, employees are responsible for the first $400 of the individual deductible and the first $600 of the family deductible. The Boston Conservatory is responsible for the remaining $1,600 individual deductible and $3,400 family deductible. Once you or a family member obtains a service where the deductible applies, you will pay the provider directly up to $400 or $600. At time of service, please ask your provider to submit the claim to BCBS. They will then send you a bill to pay once the claim has been processed. Pay close attention to the Explanation of Benefits (EOBs) that you receive from BCBS, as well as any bills that you receive from the provider to ensure they match. You may call Benefit Strategies or look online at at any time to review your deductible paid to date. Transit/Parking Benefit The Boston Conservatory Commuter Benefit Program allows you to use pre-tax dollars for transit and parking expenses related to work. The IRS Limits for 2014 Monthly contributions are: Transit: $130/month Parking: $250/month These amounts can change annually, effective January 1 st. You may contribute for both parking and transit for a combined monthly contribution of $380 funds cannot be comingled or transferred between accounts. Parking expenses include: Parking a vehicle in a facility that is near the employee s workplace Parking at a location from where the employee commutes to work (i.e. the cost of parking in a lot at the train station so the employee can commute on the train) Transit expenses include: A transportation system A private mass transit enterprise conducted by a company or individual that is in the business of transporting people in a commuter highway vehicle: Such vehicle must have a seating capacity for six or more adults and at least 80 percent of the vehicles mileage must be from transporting employees to and from their place of work. Debit Card: Participants will receive a debit card that can be used to pay for qualified expenses. Please note that if you are also enrolled in a Flexible Spending Account you will be issued one card for all plans. The card will be programmed to recognize the difference between Health Care, Dependent Care and Commuter expenses. 11 Flexible Spending Account The Boston Conservatory provides you the opportunity to pay for out-of-pocket medical, dental, vision and dependent care expenses with pre-tax dollars through a Flexible Spending Account (FSA). Eligible employees have two options available to them: 1. Health Care FSA (maximum annual election: $2,500) 2. Dependent Care FSA (maximum annual election of $5,000 or $2,500 if filing separately) Option 1: Health Care Flexible Spending Account The Health Care FSA lets you pay for certain IRS-approved medical care expenses not covered by your insurance plan with pretax dollars. For example, cash that you now spend on deductibles, copayments, or other out-of-pocket medical expenses can instead be placed in a FSA on a pre-tax basis, to reimburse yourself after you incur the expenses. Eligible health care expenses that can be reimbursed from your Health Care FSA include more than just your deductible and copayments. Generally, any medically necessary health care expense that you can deduct on your tax return is considered an eligible expense. Some examples include: Hearing services, including hearing aids and batteries Vision services, including contact lenses, contact lens solution, eye examinations, and eyeglasses Dental services and orthodontia Chiropractic services Acupuncture Option 2: Dependent Care Flexible Spending Account The Dependent Care FSA lets you use pre-tax dollars towards qualified dependent care. If you elect to contribute to the Dependent Care FSA, you may be reimbursed for: The cost of child or adult dependent care The cost for an individual to provide care either in or out of your house Nursery school and preschools (excluding kindergarten) In order for dependent care services to be eligible, they must be for the care of a tax dependent child under age 13 who lives with you, or a tax dependent parent, spouse, or child who lives with you and is incapable of caring for him or herself. The care must be needed so that you and your spouse (if applicable) can go to work. Care must be given during normal week working hours Saturday night babysitting does not qualify and cannot be provided by another of your dependents. Generally, expenses for dependent daycare must be allocated on a daily basis. However, for administrative purposes, short or temporary absences from work may be disregarded. A minor illness or vacation is construed to be temporary if you must pay dependent care expenses on a weekly basis. The Boston Conservatory Employee Benefits Guide
12 Life Insurance Plans Basic Life and AD&D Insurance The Boston Conservatory provides eligible employees with group basic life and accidental death and dismemberment (AD&D) insurance. The benefit amount is one times your annualized salary up to a maximum of $100,000. Enrollment is automatic and your employer pays the full cost for basic coverage. Life insurance pays your beneficiary a benefit, should you die and AD&D insurance pays a benefit, should your death result from an accident OR if you are severely injured in an accident. Age may affect coverage levels. Also note, if you terminate your employment, you have the option to convert this group insurance policy into an individual policy. Designate a beneficiary Please contact the Benefits Manager to update your beneficiary. Voluntary Life Insurance Voluntary Life and AD&D coverage will allow you and your dependents the opportunity to purchase additional Life and AD&D coverage. During your initial offering to enroll in the Voluntary Life/AD&D coverage, you will be eligible for a benefit up to $130,000, regardless of any pre-existing health conditions you may have. Please note, this is the only time that you will be eligible for this benefit without providing evidence of insurability. Please review your specific enrollment forms for a summary of costs and return to the Benefits Manager if you would like to enroll. Short Term Disability Enrollment is automatic and your employer pays for the full cost of coverage. In the event that you become disabled for 15 or more consecutive dates due to a non-work related illness or injury, you may be eligible to receive 60% of your weekly earnings up to $1,000 per week. Maximum benefit duration: 24 weeks If you become disabled and file a claim, if approved, disability payments will begin after your 15 th day of disability and continue until you are eligible for Long Term Disability, if you are unable to return to work. 13 Long Term Disability If you become disabled and cannot work, no benefit becomes more important to your financial security than Disability Income protection. Enrollment is automatic and your employer pays for the full cost of coverage. In order for the benefit to be received on an after-tax basis should you ever go out on disability, the cost of the benefit will be included on employees year-end W-2 as imputed income. Benefit Amount In the event that you become disabled for 180 or more consecutive days due to a non-work related illness or injury, you may be eligible to receive 60% of your monthly earnings up to $10,000 per month. If you become disabled and file a claim, if approved, disability payments will begin after your 180th day of disability and continue until age 65, or Social Security Normal Retirement Age, if you are unable to return to work. Important contract limitations may apply. Note: The age at which you become disabled may affect the duration of your benefits. Retirement Plan The Boston Conservatory offers a defined contribution 403(b) plan with three contribution options managed by ING. Participation is elective and available to full-time faculty and any staff who are scheduled to work 1,000 hours or more in a plan year. Employees are eligible to participate immediately. 1. Unmatched Employee determines percentage or flat dollar amount to contribute bi-weekly Employee allocates among approved funding vehicles with ING Total annual employee contribution is limited to $17,500* 2. Matched 2% of pretax base will receive 4% match-or-3% of pretax base will receive 6% match from the Conservatory Employees allocate contributions into any approved ING investment option Total annual employee contribution is limited to $17,500* Eligible upon completion of one (1) year of employment. 14 Legal Plan Legal Club of America Family Protection Plan Every year, one in two families require the services of an attorney. In many instances, legal issues are precipitated by other personal and professional challenges such as mounting debt, tax season, or personal crisis. To help address these needs, Legal Club created the Family Protection Plan. This professional network has contracted with Legal Club to provide its members with free and discounted services in all of the following areas: Free & Discounted Legal services Tax preparation & Advice Financial Education & Credit Counseling Identity Theft Solutions Membership includes the plan member s spouse or domestic partner, dependent children who are under the age of 25, and any dependent individuals living in the plan member s home such as a parent or grandparent. Pet Insurance VPI Pet Insurance VPI plans reimburse eligible veterinary expenses relating to accidents, illnesses and injuries for dogs, cats, birds and exotic pets. 15 TUITION BENEFITS Full-time employees may enroll in courses at the Conservatory beginning the first academic registration period following their date of hire. All fees are waived for courses at the Conservatory, but you are responsible for the costs of books or other coursework materials. Full-time employees are also eligible upon completion of one year of employment to receive a 50% discount for applied study at the Conservatory. Please note that only undergraduate courses are available for registration as there is no discount for graduate level course work. As a full-time employee, your spouse, and dependent children are entitled to take courses at a 50% discount. This applies only to undergraduate courses offered at The Boston Conservatory, but not to courses offered at other ProArts Consortium schools. Enrollment may be limited due to class space and the fulfilling of course prerequisites. A Tuition Remission Benefit form can be obtained from the Office of Human Resources. If you wish to take a course that is offered during the normally scheduled work day, your supervisor s approval is required on the form as well. Approval from the Office of Human Resources is required before registering for any courses. Full-time employees may also enroll in undergraduate courses at ProArts Consortium schools without charge. Employees are encouraged to check with other ProArts Consortium schools directly about their policies regarding fees and books. Enrollment may be limited due to class space and the fulfilling of course prerequisites. Please contact the Registrar s Office or refer to the ProArts Consortium website at for more information about the cross registration process. COMPLIMENTARY TICKET POLICY Complimentary tickets are available for most performances to students, faculty, and staff with a valid Conservatory ID. Please note that special events and fundraisers are handled on a case-by-case basis, and complimentary tickets may not be available. PLEASE NOTE: Each year the Ticket Policy changes. Information will be disseminated by the Box Office over the summer months. WHO GETS WHAT: (Please note this is only your eligibility tickets are not guaranteed) Production Cast and Crew: Two comps per production (not per performance) must be reserved in advance by contacting the Audience Services Manager. TBC Faculty/Staff: Two comps per production (not per performance) must be reserved in advance at the Box Office (in person or by phone). Director (of the production) and Division Director (of corresponding division): One comp per performance, plus four additional comps per production one comp per performance is automatically reserved, but the four additional comps must be reserved in advance by contacting the Audience Services Manager. Any additional comps for press, industry professionals, etc. must be requested through and approved by the Audience Services Manager and are not guaranteed.
