3 RECERTIFICATION Certification must be renewed annually. A sample recertification form follows on the next page. You must initiate the recertification by submitting the form. You must initiate the recertification prior to January of each year. Please note it is the responsibility of the certified business owner to reapply before the annual expiration. For the City of Cleveland you must apply for recertification prior to the anniversary month of your certification. (The Cleveland Public Schools, The Cleveland Regional Minority Purchasing Council, Cuyahoga Community College, The Northeast Ohio Regional Sewer District and The Cleveland Regional Transit Authority (RTA) may send reminders.) No recertification form will be accepted unless it is printed or typewritten, notarized and bears the signature of the owner(s). If there has not been any informational changes regarding your company within the last annual period including data regarding the ownership and control of the company, it is not necessary for you to provide additional information in addition to the sample recertification form on the following pages. In order to ensure your recertification and continued listing, please complete the attached form and return it or copies to the organizations where you wish to be recertified. If a copy is not received by January 31, your firm may not be recertified. If there has been a change in ownership or control of your company, or if you have added or deleted a product line or service, or if your business has relocated (change of address) and/or if there is other information which should be included in our certification file you must complete a new certification application package. Certification applications can be received from any of the following organizations: The Cleveland Public Schools; The City of Cleveland; Cuyahoga Community College; Cuyahoga County; The Cuyahoga Metropolitan Housing Authority (CMHA); The Greater Cleveland Growth Association and The Cleveland Regional Minority Purchasing Council; The Greater Cleveland Regional Transit Authority (RTA); and The Northeast Ohio Regional Sewer District. Remember you must make a copy of the recertification form and send a copy to any and all of the entities where you would like to be recertified. Each organization will then process your form and contact you individually. This recertification form does not waive the right of the individual organization to request additional information, or from making site visits or charging fees as part of their certification process. Individual organizations retain the right to deny certification to a company even when other entities have decided to grant that company certification. 2.
