First focus Priority Primary Care Providers MD*, DO, OBGYN, PA, NP, CNMW *(Internal Medicine, Family Practitioner, Pediatrician, General Practitioner, Geriatrics, Gynecology, Adolescent Medicine) Dual-Specialist Eligibility: 50% of patient encounters* are Primary Care Services *(annual patient encounters) Care settings: Small private practices of 10 providers or less Community Health Centers Other settings that predominately serve uninsured, underinsured, and medically underserved populations 3
