3 Guidelines for Admission to the Rolf Institute Certification Program Foundations of Rolfing Structural Integration (FORSI) or Accelerated Foundations of Rolfing Structural Integration (AFORSI) We hope that you will find the application process simple and easy to follow. If you need help or have questions about the requirements, or any aspect of the admissions process, please feel free to contact the Admissions Department. Starting the Admissions Process To start the process of admission into Foundations (FORSI) or Accelerated Foundations (AFORSI), please submit your application, a signed copy of the enrollment agreement, the $50 application fee and the $300 deposit. Only the deposit is deducted from your tuition once you enroll. This begins the process, but you must submit all required components of your application to be fully accepted and to reserve your seat in the class. Dropping or Changing Classes Students may drop or change classes up to 90 days prior to a class without consequence or fees. The Rolf Institute bills any drops or changes made later than 90 days prior to the class start day by retaining $150 of the $300 deposit. First Come, First Served Enrollment All students applying for Foundations (FORSI) or Accelerated Foundations (AFORSI) are accepted on a first come, first served basis. Payment The Rolf Institute accepts checks, credit cards (Visa, Master Card, Discover and American Express), money orders, or cash (in person only). Make checks and money orders payable to The Rolf Institute. To pay by credit card, please contact the Admissions Department. Processing your application Once Admissions receives your complete application, including all required written documentation, fees and $300 deposit, your application is reviewed. An interview may be required to complete some applications. You will receive a formal letter of acceptance and a packet of information about the class. Applications are reviewed in the order they are received. Processing your application usually takes about two weeks. To confirm that the Rolf Institute has received your application and/or to check on your status, you may contact the Admissions Department. 4 Admissions Requirements The following is a description of the required components of your application. If you have any questions, please contact: Admissions Department Phone Fax admissions@rolf.org Foundations of Rolfing Structural Integration (FORSI) and Accelerated Foundations of Rolfing Structural Integration (AFORSI) Please read the following instructions carefully and use the attached checklist for either FORSI or AFORSI. (Please note: Students applying for Accelerated Phase I (AFORSI) have additional entry requirements.) APPLICATION FORMS AND PAYMENT 1. Application Form Please fill out the application completely and sign it. 2. Application Fee A $50 application fee is required from every applicant. This fee will be refunded if the applicant is either not admitted or has not signed an enrollment agreement. 3. Enrollment Agreements The Enrollment Agreement form for Foundations or Accelerated Foundations is included in this packet. Please read and provide the required signatures, and initial the bottom-right corner of every page of the enrollment agreement form, and send it with your application, application fee and deposit. The Enrollment Agreement is required by the State of Colorado Department of Higher Education and is to protect your rights as a consumer of education. The Enrollment Agreement form for the second phase, Embodiment of Rolfing SI and Rolf Movement Integration is also included in this packet to allow you to pre-register for your second class. Please read and provide the required signatures, and initial the bottom-right corner of every page of the enrollment agreement form and submit it, and the additional deposit, with the rest of your application materials. 3
5 4. Deposit A $300 deposit is required to confirm your space in each class. The $300 deposit is deducted from your tuition once you enroll. Please submit $300 for Phase I and $300 for Phase II. TRANSCRIPTS, LIFE EXPERIENCE ESSAY, and OTHER DOCUMENTATION 5. Documentation of Postsecondary Education or Equivalent Note: Students are not required to have an undergraduate degree to attend The Rolf Institute. All students must submit ONE of the following: a) An official academic transcript* showing at least 60 credit hours (about four semesters) at an institution of higher education. OR b) A High School Diploma, GED or state certificate received by a student after the student has passed a state-authorized examination that the state recognizes as the equivalent of a high school diploma. OR AND Official documentation verifying equivalent coursework and success in an adult education program. Please note: AFORSI students may use their massage school transcript to satisfy the requirement for equivalent adult education coursework and to document required massage training. c) A High School Diploma, GED or state certificate received by a student after the student has passed a state-authorized examination that the state recognizes as the equivalent of a high school diploma. AND Documentation of equivalent life experience and accomplishment. * An official transcript clearly displays a school seal, red stamp or other obviously official marking Secure, official e-transcripts sent from universities are acceptable Faxed transcripts are not acceptable A transcript without an official marking will need to arrive in an unopened school envelope This can be sent from the school to our office. This can be sent from the applicant as long as the applicant has not opened the envelope. 4
6 6. Life Experience Essay Because students at RISI have a variety of educational backgrounds, personal interests and experience, we require prospective students to write a 2-3 page personal life experience essay. The essay should be double-spaced in an11-12 point font. Please send a Microsoft Word file or a PDF. Please submit a 2-3 page personal essay addressing each of the following items: 1. Describe your reasons for pursuing a career in Rolfing Structural Integration. 2. Describe your greatest success in life and how it has influenced your career choice. 3. Describe one of your greatest challenges in life and what you learned from the experience. 7. Resume Briefly outline your educational and employment history. You may include massage training from other trainings, workshops, avocations. Please note: Foundations of Rolfing Structural Integration (FORSI) Students are not expected to have had any prior experience with bodywork, massage or Rolfing Structural Integration, but if you do, you may include it in your resume. Accelerated (AFORSI) students are required to have prior training in massage or bodywork (see item 11) and to list all clinical experience. An optional form is available in this packet. 8. Photograph Submit a photograph of yourself for our file. The photo must be a current solo headshot that clearly shows your face. Do not use group shots or busy backgrounds. Color photocopies may also be used. The photographs are for identification purposes and to prevent identity fraud. 9. Rolfing Structural Integration Sessions Prior to starting training at The Rolf Institute, students must have completed at least three sessions of the Rolfing Ten-Series with a Certified Rolfer. Please use the included form to document the sessions and the name of your Rolfer. The remaining sessions of the Ten-Series progression are required six months prior to the start of Phase II of the training. 10. Accelerated (AFORSI) applicants only To be eligible for Accelerated Foundations, students must: a. Submit an official transcript showing completion of a minimum of 250 Hours of professional training or course work in massage and bodywork. The transcript must show: 47 hours of Anatomy, 21 hours of Physiology 45 hours of massage or other approved touch training 15 hours of therapeutic relationship or ethics b. Document 50 Hours of Clinical Practice Experience Accelerated Phase I must verify 50 documented hours of postgraduate massage practice experience or comparable clinical practice hours. Please reference your postgraduate or clinical practice hours on your resume. An optional verification form to list and document your hours is also provided for you within this application packet. 5
7 11. International Students who need an I-20/M-1 Visa Students must submit their completed Application Packet no later than 60 days prior to the class start date. Visas will not be processed prior to the receipt of the completed application. If you are required to show proof that you have met the English language requirement as per the Language Policy, available on the website and in the catalog, submission of your test scores must accompany your completed application packet. Massage Licensing Laws Since the license to practice structural integration still falls under massage license in many states, we have provided a list of where to find more information about massage licensing in your state. 6

10 Important: Foreign Students (a/k/a alien ) desiring to enroll in a course of study at the Rolf Institute must obtain an M-1 visa from a consular office abroad before being eligible to enter the United States. Enrollment in a course of study is prohibited on a B-1 or B-2 (tourist) visa. It is the responsibility of the student to contact their consulate to determine how to go about obtaining an M-1 visa. The Rolf Institute is an approved school by SEVIS and able to issue I-20 forms once a student is enrolled and accepted into the training. (The following categories of visas are also permitted: L-2, V, K, A and Resident Alien) *You will have to provide proof of sufficient funds to cover tuition and cost of living to submit the I-20 form to your embassy. Payment a. To begin the enrollment process you must pay a deposit of $300; this deposit will be applied toward your tuition. b. Fifty percent of the remaining tuition (after subtracting the deposits) must be paid two week before the start date of the class. The second payment is due mid-way through the class. The Rolf Institute process payments in a timely fashion, but students should be aware that currency rates can change without notice so that a payment may be processed at a rate other than expected. The Rolf Institute is not responsible for any changes in the rate of currency that can occur between the time the student calls, s or faxes credit card and payment processing. 4. Forms of Payment The Rolf Institute accepts MasterCard, Visa, Discover, and American Express, personal and traveler s checks, money orders, and cash (in person). Payment is remitted to the Boulder, CO office for U.S. Students may use wire transfer to make payments but must contact the Admissions Department to get the current transfer information. 5. Late fee A late fee of $100 will be assessed for tuition received after the first two-week deadline prior to the class start date and the second payment deadline at the mid-point of the training. 13 6. Delinquent Tuition The Rolf Institute reserves the right to utilize collection services in the event of unpaid student balances. The Rolf Institute will contact students by phone and mail to inform them of unpaid balances. The Rolf Institute abides by the State of Colorado Department of Education, Division of Private Occupational Schools. There is no interest charged on delinquent tuition. 7. Buyer s Right to Cancel The student obligated for any part of the tuition for a class may cancel their enrollment at anytime, but will remain responsible for tuition and fees for the period in which they were enrolled. Refund will be based on the RISI refund policy. a. Cancellation occurs when a voice mail or the student informs the Enrollment Manager/ Financial Aid Officer of their intent to withdraw. Date of withdrawal will be date of voic , or date student informs the Enrollment Manager/Financial Aid Officer. b. Notice of withdrawal, if given by , is considered given when received by the Enrollment Manager/Financial Aid Officer of their intent to withdraw. Date of withdrawal will be sent date on the unless a later date is stated in the . c. Notice of withdrawal, if given by mail, is considered given when deposited in a mailbox properly addressed, date stamped, and postage prepaid. 8. Refund Policy The Rolf Institute of Structural Integration will retain 1/2 of a student's deposit or $150, whichever is less, for any class dropped within 90 days of its start date. A student shall be refunded tuition in the following manner in the event (and within 30 days of the date) that a student fails to enter a class, withdraws, or has been terminated at any time prior to completion: 1) Refund full tuition and fees paid, including the Application Fee, if the applicant is not accepted by the school or does not sign an enrollment agreement. Note: The Application fee is not refundable for items ) Refund full tuition and fees paid if the applicant withdraws within three days after signing the enrollment agreement or making an initial payment, provided the applicant has not commenced the course. 3) Refund full tuition and fees paid in the event that the school discontinues a class or program of education during the period of time within which a student could have reasonably completed same. 4) If the school ceases to operate, the student will be entitled to a full refund of any prepaid unearned tuition and fees at the time of closure unless another instructional option is available and accepted by the student. 5) Retention of a cancellation charge not to exceed $150 or 25% of the program price, whichever is less, if the applicant withdraws after three days of signing the enrollment agreement but before commencement of the program. 6) Except for retention of a cancellation charge not to exceed $150 or 25% of the contract price, whichever is less, the policy for cancellation, settlement, and refund of tuition and fees provides for at least the following: The Rolf Institute, 5055 Chaparral Ct. 14 a) A student terminating his/her enrollment within the first 10% (ten percent) of the class, the student shall be entitled to a refund of 90% (ninety percent) the tuition of the class, exclusive of books and fees that cover tools and supplies.. b) A student terminating his/her enrollment after 10% (ten percent), but within the first 25% (twenty-five percent) of his/her class, the student shall be entitled to a refund of 75% (seventy-five percent) of the tuition of the class, exclusive of books and fees that cover tools and supplies. c) A student terminating his/her enrollment after 25% (twenty-five percent), but within the first 50% (fifty percent) of his/her class, the student shall be entitled to a refund of 50% (fifty percent) of the tuition of the class, exclusive of books and fees that cover tools and supplies. d) A student terminating his/her enrollment after 50% (fifty percent), but within the first 75% (seventy-five percent) of his/her class, the student shall be entitled to a refund of 25% (twenty-five percent) of the tuition of the class, exclusive of books and fees that cover tools and supplies. e) A student who has completed 75% (seventy-five percent) of his/her class and has entered the final 25% (twenty-five percent) of his/her program shall not be entitled to any refund and shall be obligated for the full price of the class, which constitutes the maximum obligation. 7) Refunds will be calculated from the official date of dismissal, termination or withdrawal and calculated on the period of time designated on the current agreement executed with the student. 8) The official date of dismissal, termination or withdrawal of a student shall be determined in the following manner and be referred to as the Last Date of Attendance (LDA): a) The date on which the school is noticed to be the student s last date of actual attendance. b) The date on which the student violates published school policy which provides for termination. c) Should a student fail to return from any leave of absence, excused or unexcused, the effective date of termination is the earlier of either the date the school determines the student is not returning, or the day following the expected return date. 9) The granting of credit for previous training shall not impact the refund policy. 9. Postponement of Starting Date Postponement of a starting date, whether at the request of the school or the student, requires a written agreement signed by the student and the school. The agreement must set forth: a) Whether the postponement is for the convenience of the school or the student, and: b) A deadline for the new start date, beyond which the start date will not be postponed. If the course is not commenced, or the student fails to attend by the new start date set forth in the agreement, the student will be entitled to an appropriate refund of the prepaid tuition and fees within 30 days of the deadline of the new start date set forth in the agreement, determined in accordance with the school s refund policy and all applicable laws and rules concerning the Private Occupational Act of The Rolf Institute, 5055 Chaparral Ct. 15 Placement Guarantee Disclaimer I understand that Certified Rolfers are typically self-employed in private practice and typically not in need of placement assistance. I understand that participation and completion of this class does not guarantee or result in employment in a job. I understand that signing this Agreement does not in any way guarantee final certification of completion of the program. I understand that completing Foundations of Rolfing Structural Integration in no way guarantees entrance into Phase II of the Rolfing Training, Embodiment of Rolfing Structural Integration & Rolf Movement Integration. I understand that the Foundations, Embodiment, and Clinical Application classes are the three required courses leading to Certification as a Rolfer. 11. Final Acknowledgements I understand class format, dates, and all tuition fees are subject to change. Students are given as much advance notice of these changes as possible. I verify that I have read all the pages of the Enrollment Agreement, and that I understand all terms of the Enrollment Agreement. I understand that the signed Enrollment Agreement is a legal and binding document and that I am entitled to an exact copy of the executed agreement form. I understand that the Rolf Institute is approved and regulated by the Colorado Division of Private Occupational School Board, Department of Higher Education. I understand that I can file complaints with the Division, and that there is a two year limitation on the Division taking action on student complaints. The Division of Private Occupational School Board shall not consider any claim that is filed more than two years after the date the student discontinues his or her training at the school. Payment a. To begin the enrollment process you must pay a deposit of $300; this deposit will be applied toward your tuition. b. Tuition for Embodiment of Rolfing SI and Rolf Movement Integration is due in two installments. Fifty percent of the remaining tuition (after subtracting the deposits) must be paid two week before the start date of the class. The second payment is due mid-way through the class. A specific payment date will be provided to you during the first week of class. c. Payment Processing Students are responsible for all currency rates and banking fees. The Rolf Institute process payments in a timely fashion, but students should be aware that currency rates can change without notice so that a payment may be processed at a rate other than expected. The Rolf Institute is not responsible for any changes in the rate of currency that can occur between the time the student calls, s or faxes credit card and payment processing. 4. Forms of Payment The Rolf Institute accepts MasterCard, Visa, Discover, and American Express, personal and traveler s checks, money orders, and cash (in person). Payment is remitted to the Boulder, CO office for U.S. Students may use wire transfer to make payments but must contact the Admissions Department to get the current transfer information. 5. Late fee A late fee of $100 will be assessed for tuition received after the first two-week deadline prior to the class start date and the second payment deadline at the mid-point of the training. 6. Delinquent Tuition The Rolf Institute reserves the right to utilize collection services in the event of unpaid student balances. The Rolf Institute will contact students by phone and mail to inform them of unpaid balances. The Rolf Institute abides by the State of Colorado Department of Education, Division of Private Occupational Schools. There is no interest charged on delinquent tuition. Buyer s Right to Cancel The student obligated for any part of the tuition for a class may cancel their enrollment at anytime, but will remain responsible for tuition and fees for the period in which they were enrolled. Refund will be based on the RISI refund policy. a. Cancellation occurs when a voice mail or the student informs the Enrollment Manager/ Financial Aid Officer of their intent to withdraw. Date of withdrawal will be date of voic , or date student informs the Enrollment Manager/Financial Aid Officer. b. Notice of withdrawal, if given by , is considered given when received by the Enrollment Manager/Financial Aid Officer of their intent to withdraw. Notice of withdrawal, if given by mail, is considered given when deposited in a mailbox properly addressed, date stamped, and postage prepaid. 8. Refund Policy The Rolf Institute of Structural Integration will retain 1/2 of a student's deposit or $150, whichever is less, for any class dropped within 90 days of its start date. A student shall be refunded tuition in the following manner in the event (and within 30 days of the date) that a student fails to enter a class, withdraws, or has been terminated at any time prior to completion: 1) Refund full tuition and fees paid, including the Application Fee, if the applicant is not accepted by the school or does not sign an enrollment agreement. Note: The Application fee is not refundable for items ) Refund full tuition and fees paid if the applicant withdraws within three days after signing the enrollment agreement or making an initial payment, provided the applicant has not commenced the course. 3) Refund full tuition and fees paid in the event that the school discontinues a class or program of education during the period of time within which a student could have reasonably completed same. 4) If the school ceases to operate, the student will be entitled to a full refund of any prepaid unearned tuition and fees at the time of closure unless another instructional option is available and accepted by the student. 5) Retention of a cancellation charge not to exceed $150 or 25% of the program price, whichever is less, if the applicant withdraws after three days of signing the enrollment agreement but before commencement of the program. 6) Except for retention of a cancellation charge not to exceed $150 or 25% of the contract price, whichever is less, the policy for cancellation, settlement, and refund of tuition and fees provides for at least the following: a) A student terminating his/her enrollment within the first 10% (ten percent) of the class, the student shall be entitled to a refund of 90% (ninety percent) the tuition of the class, exclusive of books and fees that cover tools and supplies.. b) A student terminating his/her enrollment after 10% (ten percent), but within the first 25% (twenty-five percent) of his/her class, the student shall be entitled to a refund of 75% (seventy-five percent) of the tuition of the class, exclusive of books and fees that cover tools and supplies. 19 c) A student terminating his/her enrollment after 25% (twenty-five percent), but within the first 50% (fifty percent) of his/her class, the student shall be entitled to a refund of 50% (fifty percent) of the tuition of the class, exclusive of books and fees that cover tools and supplies. d) A student terminating his/her enrollment after 50% (fifty percent), but within the first 75% (seventy-five percent) of his/her class, the student shall be entitled to a refund of 25% (twenty-five percent) of the tuition of the class, exclusive of books and fees that cover tools and supplies. e) A student who has completed 75% (seventy-five percent) of his/her class and has entered the final 25% (twenty-five percent) of his/her program shall not be entitled to any refund and shall be obligated for the full price of the class, which constitutes the maximum obligation. 7) Refunds will be calculated from the official date of dismissal, termination or withdrawal and calculated on the period of time designated on the current agreement executed with the student. 8) The official date of dismissal, termination or withdrawal of a student shall be determined in the following manner and be referred to as the Last Date of Attendance (LDA): a) The date on which the school is noticed to be the student s last date of actual attendance. b) The date on which the student violates published school policy which provides for termination. c) Should a student fail to return from any leave of absence, excused or unexcused, the effective date of termination is the earlier of either the date the school determines the student is not returning, or the day following the expected return date. 9) The granting of credit for previous training shall not impact the refund policy. 9. Postponement of Starting Date Postponement of a starting date, whether at the request of the school or the student, requires a written agreement signed by the student and the school. The agreement must set forth: a) Whether the postponement is for the convenience of the school or the student, and: b) A deadline for the new start date, beyond which the start date will not be postponed. If the course is not commenced, or the student fails to attend by the new start date set forth in the agreement, the student will be entitled to an appropriate refund of the prepaid tuition and fees within 30 days of the deadline of the new start date set forth in the agreement, determined in accordance with the school s refund policy and all applicable laws and rules concerning the Private Occupational Act of Placement Guarantee Disclaimer I understand that Certified Rolfers are typically self-employed in private practice and typically not in need of placement assistance. I understand that the Foundations, Embodiment, and Clinical Application classes are the three required courses leading to Certification as a Rolfer. 11. Final Acknowledgements I understand class format, dates, and all tuition fees are subject to change. Students are given as much advance notice of these changes as possible. I verify that I have read all the pages of the Enrollment Agreement, and that I understand all terms of the Enrollment Agreement. I understand that the signed Enrollment Agreement is a legal and binding document and that I am entitled to an exact copy of the executed agreement form. I understand that the Rolf Institute is approved and regulated by the Colorado Division of Private Occupational School Board, Department of Higher Education. I understand that I can file complaints with the Division, and that there is a two year limitation on the Division taking action on student complaints. 21 Intent to Apply for Financial Aid Within the world of financial assistance, there are many different forms available to students. Scholarships are financial awards given to eligible students and do not need be repaid. Grants can be federal, state or privately issued and also do not require repayment. Loans are a form of low-interest debt that must eventually be repaid and are a good option for those who do not qualify for a grant. The Rolf Institute administers two financial aid programs to help cover tuition (fees are not included) for students who qualify Federal Pell Grants and Federal Direct Student Loans. Students receiving Title IV Financial Aid must maintain attendance and academic standards in order to remain eligible for continued participation.
