2 a message from our president In 2012, the HCA North Florida Division played a vital role in providing healthcare services to the communities we serve in North Florida and South Georgia, with an economic impact exceeding $1,500,000,000 annually. Our more than 19,000 employees, physicians and volunteers provide vital healthcare services through our 14 hospitals, five surgery centers, five cancer centers and eight imaging centers. Our emergency departments served more than 600,000 patients, an increase of 50,000 over At the same time, our teams held steadfast to their dedication to expediting patient care and improving patient satisfaction. As our ER volumes grow, so does the number of patients without adequate health insurance or the financial resources to pay for this care. Last year, our hospitals treated more than 145,000 uninsured patients and over 250,000 patients covered through the Medicaid program. The cost of charity and uncompensated care provided by our hospitals totaled $161,833,669 in Despite these challenges, we continue to expand to meet the growing demand for quality healthcare services. Expansion projects were initiated at Gulf Coast Medical Center and Capital Regional Medical Center, adding much-needed capacity and services. In order to make trauma care services more accessible to the communities we serve in Central Florida, we opened a Provisional Level II Trauma Center at Ocala Regional Medical Center. Lastly, North Florida Regional broke ground for Phase IV of its major expansion, which will encompass a four-story patient tower with a new Level II Neonatal Intensive Care Unit. During its first full year of operation, the One Step Patient Transfer Center facilitated over 50,000 patient transfers in support of 21 HCA hospitals across the North Florida and South Atlantic Divisions. The Transfer Center provides critical support not only to our HCA hospitals, but also to over 190 hospital partners in five states by ensuring expeditious patient transfers to meet the patient s medical needs. Throughout the year, we continued our emphasis on providing the highest quality patient care possible. Each of our facilities earned accreditations and recognition of the hard work they do each day to ensure our patients receive the excellent care they deserve. Many of our hospitals offer Accredited Chest Pain Centers, Joint Commission Certified Primary Stroke Centers, and Cancer Centers accredited by the American College of Surgeons Committee on Cancer. These certifications represent the gold standard of care today, awarded by independent accrediting organizations. At 16 years old the last thing I wanted to do was have back surgery. I like to stay active and ride horses but the pain from my scoliosis was becoming so intense that my parents and I knew something had to be done. Community Support The affiliates of HCA North Florida and their employees are committed to improving the health and well-being of those in the communities they serve. This is accomplished not only through the quality of care they provide, but also through the financial and volunteer support they offer to countless community organizations, schools, colleges and non-profit and charitable organizations in their communities. Indigent Patient Education HCA North Florida Hospitals are committed to helping uninsured patients qualify for local and state health benefit programs. Many of our hospitals offer on-site benefit education and Medicaid enrollment assistance. Step Up for Students Since 2005, HCA has redirected corporate income tax to the Step Up for Students program. Through this gift, low income students receive a K-12 scholarship enabling them to receive a quality education tailored for their specific needs. It is well documented that a one size fits all education is not the best approach for every student. Families with the means may choose the perfect school for their child but that is simply not an option for many people. Through this program, children that qualify for the National School Lunch Program can receive scholarships. Qualifying parents can use these grants to send their child to the public or private school that best meets their child s needs. West Florida Healthcare was well represented at the 2012 American Heart Walk raising a total of $9, for the American Heart Association. 4 HCA North Florida

7 Accredited Chest Pain Center Accredited Chest Pain Center w/pci (Heart Attack) Center Primary Stroke Center CARF Certification Awards & Recognition Numerous awards and recognitions are bestowed on our facilities and management teams, acknowledging not only the outstanding medical care and services we provide, but also the leadership role we play in our communities. All of our facilities are Joint Commission (JC) Accredited, with nine JC Accredited Primary Stroke Centers, and JC Accreditation for Spinal Care at Gulf Coast Medical Center and Doctors Hospital of Augusta. This year, 10 of our facilities were recognized by The Joint Commission as one of the Top Performers on Key Quality Measures. This recognition is focused on accredited hospitals that attained and sustained excellence in accountability measure performance during calendar year As Top Performers on Key Quality Measures, our 10 facilities were among only 620 hospitals being recognized for 2011, which represents the top 18 percent of Joint Commission accredited hospitals (of those that report core measure performance data). Ten of our facilities offer Certified Chest Pain Centers, recognizing their consistent high quality care and expertise in dealing with patients suffering with chest pain. Six have been designated by Blue Cross Blue Shield as a Blue Distinction Center for Spine Surgery, and five for Hip and Knee Replacement Surgery. Fairview Park Hospital earned The Joint Commission s Gold Seal of Approval for certification as a Primary Stroke Center. This distinction was coupled with an award from the 2012 Georgia Coverdell Champion Hospital of the Year Award by the Georgia Coverdell Stroke Registry. One of only four hospitals to earn this recognition in the state, Fairview Park is at the top of the list for hospitals beds in size. Recognized for its outstanding care, Doctors Hospital of Augusta s Inpatient Rehab Facility was awarded a three-year Certified Acute Rehab Facility (CARF) accreditation for its adult physical rehabilitation program, the only CARF accredited facility in the greater Augusta area. Additionally, Doctors Hospital was one of 17 Georgia hospitals named to the Georgia Hospital Association s Partnership for Health and Accountability Core Measures Chairman s Honor Roll, the highest on the list. West Florida Hospital s Blood Bank was granted a two-year AABB accreditation for transfusion services following an intensive on-site assessment by specially-trained AABB assessors. This recognition establishes that the level of technical and administrative performance within the facility meets or exceeds the standards set by AABB. By successfully meeting those requirements, West Florida Hospital joins similar facilities located throughout the United States and around the world that also have earned AABB Accreditation. Ocala Health s commitment to accreditation and to evidence-based care is best reflected in its quality achievements for 2012, including its distinction among Thomson Reuters Top 100 Hospitals. HealthGrades Excellence Awards included those for Patient Safety, Pulmonary Care, Women s Health, Vascular Surgery, and Bariatric Surgery. Coliseum Medical Centers received full Heart Failure Accreditation status from the Society of Chest Pain Centers, and is the first hospital in the state of Georgia to achieve this accreditation. Coliseum Northside Hospital achieved Total Joint Certification from The Joint Commission. Additionally, Coliseum Medical Centers received the Get With The Guidelines -Stroke Gold Plus Quality Achievement Award for the third year in a row. 5

8 quality & patient safety Despite our achievements, HCA North Florida recognizes the importance of patient safety and continues to make it paramount. Our initiatives ensure the safety and quality of care given to our patients each day. SurgiTrak Family Notification of Surgical Status enables bedside patient and product verification for blood product administration for enhanced patient safety. In addition, it improves monitoring and reporting regarding blood administration and utilization, while decreasing errors. During 2012, SurgiTrak pilots were launched at Ocala Regional Medical Center, West Marion Community Hospital, and Capital Regional Medical Center. SurgiTrak provides notifications to designated family members of a patient s progression through the surgical episode. Notifications can be sent via text or to a mobile device such as a smart phone, a tablet, or any other computer or laptop with an Internet connection. This provides family and friends who are in a remote geographical location with immediate information about their loved one who is having surgery in an HCA facility. For family members at the hospital, it will provide them the freedom to leave the waiting area without missing vital information about their loved one s surgery. BCTA Barcode-enabled Transfusion Administration Barcode-enabled Transfusion Administration provides our hospital teams with barcoding capability for blood products, with automated reporting and trending of blood utilization in accordance with regulatory guidelines as well as automated reporting of blood transfusion reactions. BCTA implementation began in 2012 and will be completed by mid-year It is an important service to provide our teams because it Modified Early Warning System (MEWS) Monitoring Process In a busy, complex healthcare environment, recognition of deteriorating patient s condition can be delayed. Identification of negative changes in a patient s condition can decrease the number of failure to rescue events and resuscitation efforts through monitoring and timely intervention. The MEWS system has been proven to show a significant relationship between the MEWS score and transfers to the ICU or death. The score is comprised of six measures: Blood pressure Heart rate Temperature Level of Consciousness Respiratory rate MEWS transforms this physiological data, and using weighted criteria, determines an aggregate score that may be used to categorize clinical deterioration and direct care. The score is determined electronically and displayed on each patient s status board. It is important to note that MEWS is a monitoring aid to clinical judgment, a marker of critical illness, and an indicator of physiologic trends. It is intended to trigger critical thinking and assessment prior to making care decisions. Anticipated benefits derived from utilizing the MEWS scores include timely response to changing patients conditions, decreased resuscitation events and decreased patient mortalities. 6 HCA North Florida

9 Radiation Right Studies have shown that in addition to the benefits of diagnostic procedures that involve radiation, there might also be risks that should be considered by patients and those treating them. Some government agencies and medical organizations have stressed the adoption of two guiding principles of radiation protection for patients: appropriate justification for ordering each procedure and careful optimization of the radiation dose applied during each procedure. Radiation Right incorporates these baseline recommendations but adds additional steps, including adoption of education and competency standards for clinical professionals, meeting and achieving nationally recognized certification standards and adopting As Low As Reasonably Achievable or ALARA dose protocols. Radiation Right also follows The Alliance for Radiation Safety in Pediatric Imaging s Image Gently protocol for pediatric patients. Severe Sepsis Program Earlier this year, North Florida Regional Medical Center began a program aimed at the challenge of severe sepsis. NFRMC adopted early, goal-directed therapies, instituted a Sepsis Alert and invested $100,000 for 8 Vigileo monitors and interface equipment needed to support the therapies. Efforts included ongoing support for FloTrac arterial line sensors and PreSep Oximetric Central lines. Leaders formed a coordinated Sepsis Team. NFRMC s baseline mortality for severe sepsis and shock was 32 percent prior to the program. For the year, mortality is down to 18 percent, already surpassing the program s initial goal. Program managers believe even more reductions are possible and say they will be diligent in detecting and managing this difficult disease. Our radiation-reduction efforts focus on preventing accidental or isolated incidents of excessive exposure, reducing cumulative radiation dose, and focusing on nationally accepted guidelines to assure that we always give the lowest effective dose of radiation possible. Emergency Excellence The combination of our highly skilled emergency physicians and nurses, along with best practices and the latest technology, benefitted close to half a million North Florida and South Georgia residents who sought emergency care in our facilities last year. Protocol driven care that begins in the field through direct communications with local EMS teams, and meeting stringent standards to earn Chest Pain Center Accreditation and Primary Stroke Center Certification by the Joint Commission are all part of our commitment to improve quality in our busy emergency departments. Our strong focus on enhancing care and improving ER processes has also dramatically reduced wait times below national averages. The average ER wait times for each of our hospitals are posted on their websites as well as digital outdoor boards in many of the communities we serve, and are also available by text messaging ER to Smart phone users are able to access average wait times via itriage, an app that helps locate providers for emergency care, while also serving many other medical needs. During 2012, Coliseum Health Center and Fort Walton Beach Medical Center joined Capital Regional Medical Center in offering itriage s prenotification functionality to ER patients. Branded as inotify, this function allows smart phone users to notify the ER that they are on their way, along with symptoms and other information to help the ER team prepare for their arrival. During the course of the year, more than 2,000 ER patients used inotify. Lastly, Ocala Regional Medical Center opened a Provisional Level II Trauma Center, the first center in Trauma Service Area 6 which serves Marion, Hernando and Citrus Counties. This was no small commitment to make, as the project investment totaled more than $6.5 million and required specific training for all staff, as well as the recruitment of highly skilled trauma surgeons. Since its opening in early December, the Trauma Center garnered strong community support and started treating critically injured patients immediately. In 2012, West Florida Healthcare partnered with the Northwest Florida Heart Group to form West Florida Cardiology, providing a wide network of cardiovascular care to area patients. Heart & Vascular Network Our cardiovascular network ensures that lifesaving technology and the latest minimally invasive techniques are available in our communities. Several of our facilities offer accredited Chest Pain Centers: Capital Regional Medical Center, Coliseum Medical Centers, Doctors Hospital of Augusta, Fairview Park Hospital, Fort Walton Beach Medical Center, Gulf Coast Medical Center, North Florida Regional Medical Center, Ocala Health System, and West Florida Hospital. All of our facilities are beating 8 HCA North Florida

11 the national STEMI benchmark of 90 minutes or less, dramatically improving the lives of our patients experiencing a heart attack. Only 40 percent of hospitals in the country have been able to consistently accomplish this, and similar to our average ER wait times, we post our average STEMI times on our hospital websites. The latest surgical techniques for minimally invasive mitral valve and atrial fibrillation repair provide a safer alternative for patients suffering from these conditions, improving their quality of life and reducing their risk for other serious complications. Our Cardiac Catheterization and EP Labs offer stateof-the-art technology and treatments to invasive and non-invasive cardiac procedures. Comprehensive Cancer Care Cancer is the second leading cause of death in this country and our facilities are committed to meet and exceed the many standards required for the American College of Surgeons Commission on Cancer (ACoS CoC). To become accredited, our hospitals commit to regular meetings of the cancer team, which include surgeons, medical oncologists, radiation oncologists, pathologists, radiologists and hospital clinical and support staff to proactively review patient cases. The North Florida Division facilities that have achieved this honor include Capital Regional Medical Center, Coliseum Medical Centers, Doctors Hospital of Augusta, Fort Walton Beach Medical Center, Gulf Coast Medical Center, North Florida Regional Medical Center, Ocala Health System, and West Florida Hospital. We are also committed to bringing the latest in technology to the communities we serve, and our Cancer Centers are no exception. The CyberKnife System, the world s first robotic radiosurgery system, is available at the Cancer Center at North Florida Regional Medical Center. We also offer advanced cancer treatment at our other four cancer centers in Tallahassee, FL, Lake City, FL, Pensacola, FL, and Augusta, GA. 12 specialized care Imaging Services The HCA North Florida network provides the most comprehensive array of outpatient imaging services in North Florida and South Georgia. Our facilities are equipped with a broad range of diagnostic technology and most offer extended hours. We are committed to offering imaging services needed across the full continuum of a patient s care. Specially-equipped burn patient ICU room Joseph M. Still Burn Center Ranked among the world s leading burn facilities, the self-contained Joseph M. Still Burn Center at Doctors Hospital of Augusta treats an estimated 7% of burn patients nationwide (3,000 patients each year) about one-third of them children. Renowned for its multi-disciplinary approach to acute burn care, it averages an impressive 97% survival rate annually. Its team of specialists consists of plastic surgeons, general surgeons, maxillofacial surgeons, critical care intensivists, pediatricians, psychiatrists, nurses, physical and occupational therapists, dietitians, and social workers. In addition, its Advanced Wound and Burn Clinic is an outpatient clinic specializing in burn treatment follow-up and other serious skin-related conditions. The Burn Center is the largest in the country with 70 beds, 4 surgical suites, a dedicated ambulance entrance, and 20 outpatient treatment rooms in addition to hyperbaric oxygen chambers and hydrotherapy. Diagnostic services available throughout the HCA North Florida network include: Bone Density/Dexa CT 64-slice CT Digital Mammography Echocardiogram EKG Fluoroscopy Mammography MRA Open & High Field MRI Nuclear Medicine PET/CT Scan Ultrasound X-Ray West Florida Hospital s mobile mammography unit. 10 HCA North Florida

13 O-arm System creates detailed, two- and three dimensional images, which helps streamline the process during surgery. This can mean less time in the operating room, and make all the difference in the patient s surgical experience. In 2012, Fort Walton Beach Medical Center began offering surgical robot technology. Orthopedic and Spine Surgery Patients needing advanced treatments for orthopedic and spine issues can find high quality care at our North Florida Division hospitals. Advanced surgical procedures including Minimally Invasive Hip Replacement, Anterior Approach Hip Replacement, Custom Fit Knee Replacement, Gender Specific Knee Replacement, Knee and Hip Resurfacing, Partial Hip Resurfacing, and Partial Knee Replacement are available to relieve pain and help patients return to normal activities. Our surgeons are committed to performing the latest innovations in their field, and patients benefit from the experience of a panel of orthopedic experts, individualized treatment plans and a high caliber of care. Robotic and Minimally Invasive Surgical Services Our network offers some of the most efficient and well equipped operating rooms in the area. Advanced systems from scheduling to pre-op and discharge planning ensure that patients receive the care they need. One of the most exciting technologies today is the da Vinci Robotic Surgical System. Through the use of robotics, surgeons are able to perform complex and delicate procedures through tiny incisions, with unmatched precision. For patients, this means less blood loss and scarring, and a quicker recovery and return to normal daily activities. Eight of our facilities offer this option for appropriate surgical procedures: Capital Regional Medical Center, Coliseum Medical Centers, Doctors Hospital of Augusta, Fort Walton Beach Medical Center, Gulf Coast Medical Center, North Florida Regional Medical Center, Ocala Health System, and West Florida Hospital. The Orthopedic and Spine Centers of North Florida and South Georgia provide a panel of experts who collaborate on treatment plans for patients. The teams at each Center include pain management professionals, diagnostic-imaging specialists, orthopedic surgeons, physical medicine and rehabilitation specialists, neurosurgeons and physical therapists. Coliseum Medical Centers and North Florida Regional Medical Center offer the O-arm Multidimensional Surgical Imaging System technology. Neurosurgeons are utilizing this revolutionary technology for spine surgery, including lumbar, cervical and thoracic procedures. The Doctors Hospital was recently named a Center of Excellence in Women s Health Surgery by The American Institute of Minimally Invasive Surgery (AIMIS). The designation represents Doctors Hospital s commitment to offering expert surgical care and new and proven medical technologies. Our Commitment to Excellence 11

14 surgical treatment options for various neurological disorders. The focus has expanded to include patients living with Parkinson s disease and other movement disorders that could benefit from Deep Brain Stimulation surgery. Our facilities utilize the team concept, incorporating therapists, dietitians, neurologists and case managers to diagnose and treat the needs of patients. Women s Services Obstetrics services are offered at eight of our facilities, with a range of amenities that include private family waiting areas and beautiful home-like surroundings, along with state of the art equipment to help make the birthing process go as smoothly as possible. Our commitment to providing the best in women s services, from digital mammography, bone density ultrasound, to stereotactic breast biopsy, is unsurpassed. Capital Regional Medical Center and Coliseum Medical Centers offer comprehensive breast centers, providing women access to advanced breast imaging technology in a warm, spa-like environment. Patient navigators at several of our facilities work with women who have potentially concerning findings to help them understand the process from detection to diagnosis to treatment. Our facilities ensure that patients receive the most advanced diagnostic technology and the highest quality of healthcare available in North Florida and South Georgia. NeuroSciences and Stroke Care Nine of our hospitals have earned Joint Commission Primary Stroke Center designation, the gold standard for ensuring the highest level of care is immediately available: Capital Regional Medical Center, Coliseum Medical Centers, Doctors Hospital of Augusta, Fairview Park Hospital, Fort Walton Beach Medical Center, Gulf Coast Medical Center, North Florida Regional Medical Center, Ocala Health System, and West Florida Hospital. HCA hospitals also offer surgical and non- 12 HCA North Florida Bariatric Surgery The goal at our Bariatric Centers is to improve the health and lives of those in our communities who are struggling with obesity and its associated health problems, which can be fatal. With more than 60 million adults considered obese, the Centers offer specialized weight-loss treatment options to those who need help with their obesity. The services offered include behavioral, nutritional, and exercise intervention, as well as surgical options that consist of gastric banding, gastric bypass and gastric sleeve. After a complete medical evaluation, a customized program is developed for each patient. Six of our hospitals offer bariatric surgery programs: Capital Regional Medical Center, Coliseum Medical Centers, Doctors Hospital of Augusta, Gulf Coast Medical Center, North Florida Regional Medical Center, Ocala Regional Medical Center. In addition, North Florida Regional Medical Center and Ocala Regional Medical Center are designated bariatric Surgery Centers of Excellence by the American Society of Metabolic and Bariatric Surgery. I talk to people every day about the dangers of diabetes and really didn t feel healthy myself. That has all changed now, thanks to Coliseum!
