Designing healthcare 30th april, Belfast Marianne Olsson, Project Director Presentation Project Director Angereds närsjukhus, Region Västra Götaland Background- Former Director of Quality Improvement at Sahlgrenska University Hospital and at the Swedish Association of Local Authorities Former president of the Swedish Society for Quality in Healthcare Member of the board of EHMA, European Health Management Association 1
EHMA The European Health Management Association is a network of health organisations throughout Europe. Our objective is to improve health through better management and we do that by: Building Bridges between countries, academia, managers, policy makers and health professionals. Sharing Knowledge between experts and health organisations so that we can influence policy in Europe Angereds närsjukhus - a community hospital in a multicultural urban setting 2
The Northeastern part of Gothenburg Four city districts of Gothenburg inhabitants 50 percent born outside Sweden A population in need of better care Mortality rate higher than the country on a number of diseases Cardiovascular disease COPD/Lung cancer Alcoholrelated deaths Also higher numbers on following Obesity Daily smoking Physicial inactivity Stress 3
Vision We are going to improve the health of the population in the northeastern part of Göteborg by creating a innovative hospital that helps mobilise the local community and thereby increase the sense of safety and security amongst the people living in the area Mission We will take the lead in creating a wellfunctioning local healthcare-system We will create an ongoing process to asses the health-care needs of the population We will systematically involve patients and citizens in our work We will design processes to meet these needs We will provide an arena for testing innovations in all areas of health-care 4
Example of current aims We will improve the health of the population so that it equals the rest of the city The numbers of daily smokers will be diminished by 50% in two years time The number of days on sick-leave because of chronic pain will decrease The new hospital will be ready by the end of 2011 It will become a central part of a local commercial centre A topmodern building Hosting all types of local care integrated 5
but we are already up and running in temporary facilities In place Practice for speech pathologists Psychiatric care GP duty evenings and weekends Due 2008 Multidiciplinary pain centre Internal medicine teams (cardiovascular, lung and diabethes) Specialist centre for children and young adults Gynecology The planning has reached the sketchboard 6
Organising the building project Project manager from the real estate department within the region Architect from leading agency Multiple working groups Contractor not settled yet Private company will be the legal owner of the building Designing for the customer Healthcare trying to customise care Building industry trying to customise the building of health-care property 7
Difficulties? Understanding to what extent you are not customer-oriented - Perspective Language The struggle for power Timeline Lack of coordination Well defined timeplans for building, not as well defined plans for deciding on content Those who commission building not taking into account the time needed to redesign process, a new building won t change outcome if you don t change how things are done 8
What we want to create A beautiful and welcoming hospital A health-care system that makes sense Integrating care around the individual patient Accessability and good outcome results A dynamic and joyful working environment Mortality COPD, (Chronic Obstructive Pulmonary Disease) Bergsjön Gunnared Kortedala Lärjedalen All 4 s Region Country Women Men Number of dead/ (averag pop over 15 + yrs). 11 Communication a key success factor Different professions and different languages A lot of energy has been used on just understanding each other This takes energy from the actual task and limits creativity We have tried hard to act like clients and teach the contractors to talk to us in our language, but have we succeeded? Time is a obvious limit Our way of working is time consuming The ambition is to be innovative requires research and cooperation with scientists from different backgrounds The ambition to meet the needs of the population requires time consuming dialogue The building takes more than three years to build Cath 22! Our solution has been to try and choose as flexible soulutions as possible so that future changes will not be impossible 11
A local hospital with integrated care A unique healthcare project caracterized by innovation Political consensus behind the project Based on the idea of integrated care, which involves all local healthcare suppliers A good cooperation within the healthcare sector is crucial for this project Angereds Närsjukhus as a project Assignement Analysis of need Stakeholders agreementl Cooperation Starting practices Planning the building Dialogue with the population 12
