All information requested is REQUIRED. Partial information or information omitted will delay the setup process. Step One: Set up an account with FC Background by faxing the following documents (included): Signed & Initialed Service Agreement (pages 2 and 3) Completed New Client Information Sheet (page 5) Credit Card Authorization Form REQUIRED (page 6) Account setup process requires one full business day. Details omitted will delay the setup process. You will receive an with instructions to log into our website, to download the employee consent form and view/print completed reports as well as maps to our DFW facilities. Step Two: Begin screening & badging process Make adequate copies of the consent form. Download additional consent forms online. Each consent form must have the employer s authorizing signature delays and additional fees will be incurred if the consent form is not signed by the employer and provided by the employee at the time of screening. No appointment is needed. Badges are normally mailed or ready for pickup within hours. Badging qualifications Valid SSN (name and SSN must match) See Page 4 for helpful information about SSN validation Negative drug test on file with FCB No felony convictions, no open or pending felony cases (no time limit) No misdemeanor convictions involving crimes against children or crimes of moral turpitude* No registered sex offenders No outstanding warrants for crimes that would disqualify an individual from receiving a badge *Crimes of Moral Turpitude include, but are not limited to: Dishonesty; fraud; deceit; theft; misrepresentation Deliberate violence Base, vile, or depraved acts that are intended to arouse or gratify the sexual desire of the actor Felony possession or conspiracy to possess, or any misdemeanor or felony transfer, sale, distribution, or conspiracy to transfer, sell, or distribute any controlled substance defined in Chapter 481 of the Health and Safety Code Felony driving while intoxicated (DWI) Acts constituting abuse or neglect under the Texas Family Code.

3 P a g e 2 FC Background Service Agreement FC BACKGROUND, LLC (FCB), Park Central and (Company Legal Name) hereinafter referred to as the CONTRACTOR, for the considerations hereinafter expressed, agree together as follows: The purpose of this agreement is to allow the CONTRACTOR to use the services provided by FCB as a risk management tool in screening contractors and consultants for District campus access. FCB will provide such services at such fee(s) as listed below: a. Social Security Number Validation/ID trace b. Criminal investigation i. Criminal history check in the subject s current county of residence ii. Criminal history check in Dallas County, Tarrant County, Denton County and Collin County iii. Search of the Texas DPS s Statewide Criminal Records Database and Sex Offender Registration Database iv. Criminal history check of the Multi-Jurisdictional Search (MJS) that includes the National Sex Offender Registry v. 11 monthly criminal re-screens of the Texas DPS s Criminal Records Database and Sex Offender Registration Database c. 5-Panel urine drug test i. Laboratory confirmation on initial non-conclusive results ii. MRO service on positive drug test results d. Photo ID badge Facial recognition cross matching scanning is a standard component of the badging process. This enhanced capability provides instant comparison of each new badge photo against FCB s proprietary database of all existing badge photos. FCB will review each background and drug test result to make a determination whether to issue an ID badge based on criteria established and provided by the District. Criteria are included in this document. There are NO badging exceptions to the criteria. Individuals meeting the District s criteria are issued a photo ID badge. FCB maintains a web site where the District and its authorized agents can validate all badges issued by FCB. Badged individuals are periodically rescreened for criminal records. If at any time an individual fails to meet the established criteria, the badge is subject to being revoked. It is understood and agreed that in order to evidence CONTRACTOR compliance with District policy, FCB is hereby granted permission to share background screening, drug test results, or contractor photos for ID purposes with District authorized personnel. Initial Price for the background investigation, drug test, and photo ID badge is $53.00* per individual, plus any additional fees for obtaining criminal records/case files. Federal law requires that any felony criminal records found on the national database search (MJS) be verified with the issuing county. These verifications result in additional search fees. Payment is due to FCB regardless of the result of the background investigation or drug test. Replacement badges are $10.00 each and require District notification and authorization. FCB reserves the right to charge the CONTRACTOR $5.00 if the proper consent form is not used. Initial FCB invoices weekly. Invoices are due and payable upon receipt. FCB will automatically debit the credit card provided by the CONTRACTOR on the date of invoice. FCB requires CONTRACTOR to keep a valid credit card on file. Invoices are available to view online. FCB reserves the right to refuse service to any CONTRACTOR with unpaid invoices. The District is notified of all past due invoices. Initial

4 P a g e 3 FCB will not provide services to any CONTRACTOR or individual that has not executed the proper paperwork with FCB. Initial It is the sole responsibility of the CONTRACTOR to secure and to furnish FCB proper and correct information for each subject sufficient for FCB to perform the services contracted. CONTRACTOR certifies to FCB that it will comply with all applicable state and federal laws; that information received from FCB will not be used in violation of any equal employment opportunity law or regulation. CONTRACTOR hereby releases FCB and its affiliated companies, officers, agents, employees and independent contractors of FCB and its affiliated companies from liability for CONTRACTOR S noncompliance with all applicable laws, rules and regulations. It is the CONTRACTOR S responsibility to ensure District contract specific screening compliance. Initial Photo ID badges are valid for a period of one year. CONTRACTOR is responsible for inactivating and then returning or destroying badges upon employment termination or completion of work. This agreement shall become effective on and shall remain in force and effect until modified or suspended by either party. Prices quoted above shall remain fixed for one year. 5 P a g e 4 Subject: SSN Validation for Contractors Absent E-Verify, FC Background utilizes credit bureau records to verify each subject s name matches to a valid SSN. The credit bureau runs each SSN through Social Security master index files to look for possible fraud such as the SSN matches to a deceased person or the SSN was issued before the subject s date of birth. Occasionally the credit bureau will not have sufficient information to match the subject s name with the SSN provided. The credit bureau is not necessarily reporting fraud associated with that number only that it cannot match that name and number. Standard practice when this happens is to request the employer or subject provides us with documentation from the SSA stating the SSN and name match. Employer/Contractor may print the online E-Verify confirmation number The subject may visit any local SSA office and leave with the documentation The employer may log into the Social Security Online/SSNVS website and print the documentation o To register online to use the SSNVS website, go to: o To login and begin using the SSNVS website, go to: SAMPLE SSN VERIFICATION LETTERS OBTAINED BY THE EMPLOYER THROUGH THE SSA We accept the documentation by fax, or in person and do not need an original. The documentation must match name with SSN, and the name listed on the documentation must be the same name originally provided to us by the subject or employer. We are also setup to access the E-Verify system as an authorized agent for our contractor clients. The contractor has to submit paperwork to setup their E- Verify account and then designate FCB as their authorized agent. Contact FCB for more information.
