1 NORTH TECHNICAL HIGH SCHOOL SHOP REQUIREMENTS Program Expenses: Students will be issued appropriate tools at the beginning of the year. Students are responsible for exercising reasonable care in the use and security of tools. Students should obtain a personal lock for the assigned toolbox. The instructor must be informed of the lock combination. Students desiring to purchase the kits may do so any time during the program. Full payment must be made at that time. Several shops require students to purchase personal items for the shop the student is assigned to (Required safety glasses [ Z/87 clear], for many of the programs can be purchased in the main office at North Tech if needed for $4.00): CAE (Career Exploration-Sophomore Program): Construction, Manufacturing and Transportation Technology programs require: Z/87.1 safety glasses, close toed shoes. Student may not wear shorts in in these shops. 2 AUTO COLLISION REPAIR Pair of Z/87.1 safety glasses that need to be worn everyday, students must have leather shoes. Juniors and seniors need to purchase two tan Dickie shirts and two pair of tan work pants or two tan Dickie jump suits. Belts must be worn with pants and shirts must be tucked in at all times. NO SAGGY PANTS or SHORTS ALLOWED. Student should purchase a duel cartridge respirator from instructor to make sure it is the correct unit. (The cost will be between $14 to $20 dollars. This respirator is only good for 40 hours of paint time; therefore another unit may need to be purchased at a later date).

3 AUTO TECHNOLOGY Pair of Z/87.1 safety glasses; two pairs of blue Dickie-type dark blue work shirts and pants, black or brown leather belt, black or brown leather work shoes with oil-resistance soles. Work shirts can be short or longsleeved. Belts must be worn with pants at waist height with shirts tucked in. Saggy pants and shorts will not be allowed. Cell phones and i-pods will not be allowed in the shop or classroom (1) school rules, (2) and not allowed in professional shops which can pose a safety hazard.

4 BUSINESS AND FINANCIAL MANAGEMENT Professional business attire once a week and one week per quarter. Gentlemen will need suits or dress pants, shirts, ties, dress socks and dress shoes; ladies will need dresses, suits, or pantsuits. Dress and skirt length must follow school rules of fingertip length. Ladies will also be required to wear dress shoes (no open toe or heel) and nylons. Nails may be no longer than a quarter inch. Twenty-five hours of community service is required each semester, and active membership in Future Business Leaders of America (FBLA) or SkillsUSA is required. Membership dues are paid yearly by the student and range between $10 and $15. Program uniform is also being considered, and will be discussed in further detail at the beginning of school. Students should be prepared to invest approximately $50 in t-shirts/polo shirts and pants when the final decision is made.

5 CARPENTRY Pair of Z/87 clear safety glasses, steel toed leather boots, white carpenter/painter pants sized to fit (no baggy or droopy pants), belt sized to fit, worn at the waist, white tee-shirt that will be worn tucked in, and NO jewelry or shorts. Tool list: 16 oz. claw hammer, tool belt w/nail pouches, 25 ft. tape measure, 4 in 1 screwdriver, ¾ wood chisel, framing sq. w/rafter table, speed sq., magnetic torpedo level, 24 level, utility knife w/retractable blade, pack of (6) #2 phillips power screwdriver bits.

6 CONSTRUCTION TRADES Pocket folders with three eyelets, colored pencils, notebook paper, pens or pencils for daily use. Pair of Z/87 clear safety glasses (comfortable fit NO dark glasses), good quality key lock for tool locker with two keys, steel toed leather work boots, white carpenter/painter pants sized to fit (no sagging pants allowed), leather belt (to fit worn at waist), dark gray t-shirts (3-5 short sleeve) with pocket sized to fit and tucked in (no logo or writing on shirts). NO jewelry or shorts. TOOL LIST: 16 oz. claw hammer, carpenters tool belt with nail pouches, 25 ft. tape measure (16 th scale only tape measures with printed fractions will not be allowed), 4-1 screwdriver, ½ and ¾ wood chisels, combination speed square, magnetic torpedo level, lineman pliers, needle nose pliers, wire strippers/crimpers, utility knife with retractable blade, chalk box, drywall keyhole saw, 10 inch drywall taping knife, drywall mud pan. 7 COSMETOLOGY The following important things are needed for enrollment in the Cosmetology class. Due the First Day of School : High school transcript Birth Certificate $25.00 License Fee (Money order only- made out to State Board of Cosmetology) 4-2x2 current passport sixe picture $10.00 SkillsUSA membership dues (required) Complete Uniform(MANDATORY): Solid black or solid white, roll down collar, button front, polo style shirt (no colors or designs on shirt) Shoes-solid black or solid white work shoes (no color soles or designs on shoe-all one color). No open toe, sandals, flip flops or boots with uniform. Uniform pants will only be solid black Dickie or scrub pants with no colors or designs on pants. Parachute, black tights, leggings, sweats, Capri pants, etc. are unacceptable for the cosmetology shops. In cold weather, you may wear a solid white or solid black sweater with no designs or other colors on the sweater. A solid black or white zipper front hoodie may be worn. No outer wear jackets in class. No head gear is allowed in class or shop area. Materials need for class: 3-ring binder Pencils and pens Notebook paper Black permanent ink markers Highlighters

8 CULINARY ARTS Hepatitis A Vaccination: Students in Culinary Arts must have a Hepatitis A vaccination, which is a series of two shots. (The Hepatitis vaccination is due within the first two weeks of school.) If you have had the vaccination, bring your shot record indicating you have had one or both. If you are a returning student you must show proof of your second Hepatitis A shot. The second shot must not exceed one year beyond the first shot. The Hepatitis A vaccination is available at: 1. 10 EARLY CHILDHOOD CAREERS The Missouri Department of Health and the Child Care Licensing Bureau mandate the following Health Requirements. Therefore, students enrolled in this program will need these procedures completed prior to the start of school. Physical examination, current (within one year) PPD Test with statement of negative results (first year students only) Hepatitis A Vaccinations (series of two-six months apart) The Family Registry back ground check fee of $ o Family Care Registry/Criminal Background Check o The Missouri Department of Health and Senior Services require a Criminal Background Check on all students enrolled in Early Childhood Education Programs. Any student with prior criminal offenses deemed ineligible to work or volunteer in a Child Care setting will be excluded from participating in the Child Care Lab experience per State Law. Family Care registry will register the students and perform the background check for a onetime fee of $ The background o o check must be completed by the family care Registry to be valid for this Program. The background check must be completed on-line through the Missouri Department of Health and Senior Services at: Click on the register on-line following the prompts. A credit card is required to pay for the background check. This must be completed and turned in by Monday, August 18, Failure to complete the background check as instructed will result in not working in the Child Care Lab and may be sent to the Student Adjustment Center (in school suspension). Please print verification showing application has been completed and bring in by August 18, Please acknowledge you have received and understand the Family Care Registry/Criminal Background Check information. I acknowledge if I do not follow the information provided and/or I am ineligible by the Missouri Department of Health and Senior Services I will face removal from the Early Childhood Education Program. o Parent Signature Date Student Signature: Date The consequences for not meeting these requirements after the completion of the first week of school will be, in school-suspension until they have been met. A copy of these records must be provided to the Early Childhood Education Instructors on the first day of attendance. Supplies for the classroom: One large glue stick One pair of scissors Two black broad tip permanent markers and one box of colored markers One large hard backed three ringed binder (rings need to be at least two inches in diameter) Two sets tabbed index guides One paper punch (optional) One large file box Two sets of file cards Paper, pencils and pens Colored pencils Uniform must be worn on the first day of attendance. Required uniform: Polo style shirt, solid color Tan, black or blue pants, blue jeans with no tears or holes (no sweat pants or leggings) Tan, black or blue skirts and/or shorts must fit loosely and be fingertip length Students may wear a solid color sweatshirt or hoodie without pockets Only white shirts or turtlenecks may be worn underneath of polo style shirt. Must be the same length as polo shirt or be tucked in slacks. Tennis shoes only. And color. No boots of any kind or color.

11 Purses and backpacks are to remain in the student s locker while in lab.

12 ELECTRICAL TRADES Students will be required to provide their own tools and clothing. Shop clothing consists of a dark blue Dickie type work shirt. Footwear will be leather work boots. The student s pants must fit properly with no rips, tears or holes. The pants must comply with North Technical High School s Dress Code Policy. TOOL LIST: One pair of Z/87 rated safety glasses, wire-strippers, non-fractional tape measure (the tape measure should NOT have the fractions displayed on the tape), hammer, utility knife, 10 in 1 or 6 in 1 screwdriver, magnetic torpedo level, needle nose pliers, slip joint (channel lock/pump) pliers, lineman pliers (side cutters/kleins).

13 FIRE/EMS Each student will go through the following process: 1. Complete the North Tech application process 2. Must be going into junior class with a 70% or better grade point average with an eighth grade math and reading comprehension or better to start the FIRE/EMS Academy and maintain a 75% or better throughout the 2-year academy. 3. No student will be considered for Missouri State Firefighter I or II certification that has not completed the total 2-year program. The student must reach their seventeenth birthday by their senior year, and is on track to graduate. 4. The EMS Academy will be open to any senior student with a 75% or better grade point average with eighth grade or better math and reading comprehension, if the program has not reached program capacity. 5. Students that have enrolled in the Fire Academy and have maintained a 75% or better grade point average, in the Emergency Medical Responder program, will be guaranteed a priority in The EMS Academy. 6. Students will be required to be in Academy uniform during class time or any academy function/observation. Program instructors can give exception to the policy if needed. Uniforms and hair style will be worn to industry standards. Dangling jewelry, long fingernails, baggy or droopy clothes, and any facial or body piercing that may endanger the student s safety will not be allowed. 7. Students will be required to follow all school district, high school and program policies. Violation of policies can result in the student losing recommendation for certification. 8. Students will follow a strict academy absentee policy. No student will miss more than 10 days or 25 hours during the school year. Exception can be given for verified activity in students home school, required school testing or at instructor s discretion. Telephone and /or written verification will be required. 9. EMT-B students entering the EMS Academy (Senior) will go through a mandatory (preemployment) drug testing from an independent contractor in the first 60 days of the academy. The cost of the drug test will be paid by the student in the first 30 days. 10. Students entering the Fire and EMS Academy must have a Hepatitis B series of vaccination and a negative Tuberculosis Test (PPD) which must be given by students own health care provider prior to the start of school. The Firefighting student s will also be required to have a medical survey/physical examination within the first 90 days of the start of class. 11. Student will have an academy sanctioned uniform: Dark navy blue work pants $ Dark navy blue uniform shirt or Academy Polo shirt $ Black 1-3/4 uniform belt $ Black work boots or shoes $ Academy uniform shirts and other academy apparel ordered forms will be available at the start of class EMT students must have a stethoscope $ Students entering the Fire/EMS Academy will have a nonrefundable lab fee that will cover the National and State examination fees that may be needed. The lab fee must be paid in the first 30 days of the academy or arrangements must be made. Junior year (first year) $ Senior year(second year) $150.00

14 GRAPHIC DESIGN/GRAPHIC PRODUCTION AND PRINTING A blue or black dickie shirt is required and safety glasses. No open-toed shoes, high heeled shoes or high heeled boots will be tolerated. Shoes must have a non-skid sole and shoestrings must be tied. Loose hanging jewelry is not safe. Shorts and skirts must follow school rules of fingertip length. Fingernails should not be longer than ¼ of an inch.

We are very excited about your decision to be part of a program that will allow you to prepare for a career as a health care provider! On Monday, August 17, 2015 you will begin your interesting and worthwhile journey and we feel privileged that we are able to be your guides. The form included with this letter, Health Science Uniform Policy/Professionalism Code, is extremely important and should be read, signed and submitted to your instructor on the first day of class. Professional dress, appearance and conduct are essential in our field; expectations for each student are high! There are several important and necessary requirements that will be the responsibility of students and parents throughout the first year of the program; clinical participation in the senior year will depend on the satisfactory completion of these requirements. 16 HEALTH SCIENCES JUNIORS Health Sciences Course Description The Health Sciences program teaches the skills necessary for a foundation in healthcare. Through a combination of classroom activities and actual on-the-job clinical experiences in nursing homes, hospitals, and physician's offices, students learn human anatomy and physiology, medical terminology, disease processes, CPR, first aid, vital signs, human relation skills, and basic healthcare skills. It is imperative to be able to think and reason for this career field. Upon completion of the program, students may have the opportunity to take the Missouri State Test as a Certified Nurse s Assistant exam. If the student passes the exam with an 80 percent and has completed all clinical requirements, he or she may be certified as a nurse s assistant by the State of Missouri. Seniors may qualify to attend an off-campus program in a nearby acute care hospital or nursing home. Graduates may qualify for advanced standing at the college level. Off-Campus Clinical Program As a pre-requisite to participating in the off campus Health Sciences program, students must provide proof of: a negative tuberculin skin test or negative chest x-ray; a negative five panel urine drug screen; a clear criminal background check; and a medical exam indicating ability to participate in the Health Sciences Program. Students must also initiate a series of Hepatitis A vaccines. For those who qualify, the off-campus program includes a component in which students provide direct care to patients in a clinical setting. The clinical assignments are dependent on the job duties assigned to students by the acute care hospital or nursing home. Students will receive instruction in and must have the physical ability to perform a variety of tasks including but not limited to: bathing patients and attending to their personal hygiene needs; repositioning and ambulating patients; assisting patients with bedpans, urinals and commodes; turning and repositioning patients; lifting patients or items weighing at least 25 pounds and up to 75 pounds; pushing or pulling patients or items weighing at least 40 pounds and up to an average of 75 pounds; feeding patients and passing patients meal trays; and cleaning patients rooms. Students will receive instruction in and will be required to master correct body mechanics and body substance precautions. Students must be able to use correct safety techniques and initiate fall precautions within the policies and procedures of the clinical setting. Students in the off-campus program will likely be exposed to: needles or other sharp objects; toxic/caustic chemicals; dust/fumes/gases (including asbestos); bio-hazardous waste; potential electric shock; x-ray electromagnetic energy; blood or body fluids; and communicable diseases including but not limited to Cytomegalovirus ( CMV ), Parvovirus, Tuberculosis, HIV, Staphylococcus, and Hepatitis. Because providing direct patient care is physically demanding, students must be able to meet the required physical demands of their clinical assignment. These physical demands may include extended periods of: lifting; standing; walking; running to respond to patient emergencies; stooping; kneeling; crouching; climbing stairs; maintaining good balance; using a two hand carry; gripping and pinching; performing frequent repetitive motions; trunk flexion and rotation; and hand coordination, including reaching, grasping, fingering or feeling. Students must have sufficient visual and audio acuity necessary to perform patient care. A change in physical status that requires activity limitation may result in the need for a student to take a leave of absence from the Health Sciences program. Students in the off-campus program must be able to support the patient care team while learning under the direction of the SSD instructor and clinical site staff. Students must promote team-building with the SSD and clinical site staff by maintaining an attitude which promotes a healthy, supportive environment for residents and staff. Students must demonstrate behaviors which promote professionalism in the areas of: communication; organization; attire; and demeanor. Due to the patient focused objective of this learning experience, insubordination and disrespectful behavior is not allowed and students must exhibit good interpersonal skills. Students must be good stewards of the clinical site s resources. Students must strictly follow patient confidentiality requirements. Attendance The State of Missouri requires that students who are enrolled in the Certified Nurse Assistant program complete 75 hours of classroom instruction and 100 clinical hours. Students who are absent from school while enrolled in this program may be ineligible to take the Missouri State Certified Nurse Assistant Exam or attend the off-site clinical program. The Health Sciences program does not allow for an entire semester of independent study.

17 HEALTH SCIENCES JUNIORS Health Sciences Uniform Policy/Professionalism Code In preparation for a career in the medical field it is imperative to comply with a code of professionalism, which includes professional dress, appearance and conduct. When you go out into the clinical setting, you will represent yourself, our program and North Technical High School. The following are non-negotiable requirements: Uniform is a CEIL BLUE scrub top and pants. Scrubs should fit appropriately (not baggy/loose or too tight) covering the body suitably. Clothing should fit properly, be neat, and clean. Students/parents are responsible for purchase of their own scrubs. May only wear a CEIL BLUE lab coat if necessary. Sweaters, sweatshirts, coats, jackets, etc., are prohibited. Undershirts are permitted and must be WHITE ONLY. Colored undershirts are prohibited. Street clothes may not be worn under the uniform. White leather shoes must be worn. Sandals, opened toe shoes or canvas shoes are not to be worn. Shoes must be a solid material without holes in the shoe body. Shoes should be clean and cover the entire foot. ID badge must be worn at all times with photo facing outward and around the neck above the waist. Any item preventing the picture on the ID badge form being readily visible is prohibited. A watch with a second hand is required. Hair should be neat and clean. Long hair pulled back away from the face. No Scarves, curlers, distracting hair accessories, etc. may be worn. Hair color must be a natural tint; A COLOR NATURAL FROM THE SCALP. Participation in the clinical setting is not permitted if hair tint is an unacceptable color (orange/red, blue, purple, white, etc.). Fingernails will be short with only clear polish allowed. Artificial products including overlays and colored fingernail polish are prohibited. If the nail can be seen over the tip of the finger, the nail is too long.

18 No visible body piercings or jewelry will be allowed (nose, lip, tongue, eyebrow, etc.) with the exception of earrings and/or a wedding band. Only two stud/post earrings are allowed per ear. No dangling earrings or excessive jewelry permitted. Tattoos and/or other temporary unnatural skin markings may NOT be visible. A lab coat must be worn at all times to cover tattoos on the arms. Tattoos on the hands, neck and arms are prohibited. I acknowledge I have received and understand the Health Sciences Uniform Policy/Professionalism Code. I acknowledge that if I do not abide by this policy/code I may face disciplinary action as outline in the Student Handbook and I risk removal from the Health Sciences Program. 19 HEALTH SCIENCES JUNIORS Junior Year Clinical Policy Health Sciences is a two-year major at North Technical High School beginning the Junior year with the goal of certifying as a nurse assistant through the State of Missouri Senior year. Per State of Missouri regulations, students must complete 75 hours of classroom instruction and demonstrate proficiency in 64 skills before entering into any clinical facility serving long term care clients to complete 100 clinical hours required for nurse assistant certification. As part of the learning process, students at the Junior level will be having clinical shadowing experiences at a local long term care facility. Students will be providing limited patient care services to geriatric patients to be better prepared for the Senior level clinical experience. Clinical experiences at the Junior level is a privilege for students who exhibit continued competence in the 64 essential skills required for certification. To attend the clinical experience, students must pass a Vital Signs Proficiency Test with a score of 100%. If a student scores less than 100% on the Proficiency Test, students will remain in the classroom until a 100% is achieved. In addition, students exhibiting issues with attendance and discipline will also be prohibited from attending clinical experiences. Part of the 64 essential skills includes demonstration of proficiency in measuring and recording vital signs. Obtaining vital signs is an integral part of providing quality health care to all and includes accurate measurement and interpretation of blood pressure, temperature, pulse, and respirations. Monitoring vital signs is a fundamental component of health care essential to identifying clinical deterioration in a client s condition. Inaccurate recording of measurements in vital sign interfere with appropriate and timely interventions for clients. Related to the importance of mastering the skill of obtaining vital signs, Junior level students must demonstrate proficiency in measuring and recording vital signs for the instructors, both in the classroom and in the long term care clinical setting. Vital sign proficiency will be assessed by the end of second semester of Junior year through clinical experience after classroom instruction. *Inability to demonstrate proficiency in any of the 64 CNA skills taught Junior year including, but not limited to, measuring and recording vital signs, will result in students being ineligible to take the State of Missouri Nurse Assistant Certification Test Senior year.* I acknowledge I have received and understand the information provided in the Junior Year Clinical Policy and requirements for certification as a nurse assistant listed by the State of Missouri. 20 HEALTH SCIENCES JUNIORS Health Science Drug Screen Information The Healthcare industry mandates anyone providing care to patients must be free of illicit drugs. The Health Science/EMT Programs provide an opportunity for the students to learn in clinical areas proving patient care; therefore the students must be in compliance with business and industry standards. All students must submit a random five panel urine drug screen with negative results to continue in the Health Science/EMT Programs. If the drug screen is positive, the results will be shared with the parents if the student is under the age of 18 and the student will not be permitted to continue in the Health Science/EMT Program. The drug screen will be conducted unannounced during school business hours during the first semester. Refusal to participate in the drug screen will prohibit the student from continuing in the Health Science/EMT Program. Students not present at school on the date of the drug screen have five days to complete the drug screen at any one of the Mercy Corporation Health Clinics. If a student does not complete the drug screen within five days, the student may not continue in the Health Science/EMT Program. There are no exceptions regarding the completion of the drug screen! The drug screen can only be completed by Mercy Corporation Health Clinic as they are contracted with Special School District. The cost of the drug screen will be $ PAYMENT IS THE RESPONSIBILITY OF THE STUDENT. Money for the drug screen must be paid to the school secretary with CASH or CASHIER S CHECK payable to MERCY CORPORATION HEALTH CLINIC. Payment for the drug screen must be turned into the main office by Wednesday August 13 th, I acknowledge I have received and understand The Heath Science Drug Screen Information. 21 HEALTH SCIENCES JUNIORS HEALTH SCIENCE CLASSROOM RULES 1. Arrive to class promptly and prepared for class. Locker passes will not be issued. Class will start as soon as the bell rings and ends when the bell rings. 2. Be in complete uniform as outlined in the Health Science Uniform Policy and Professional Dress Code. 3. Electronic devices will not be allowed in the classroom as outlined in the Student Handbook. 4. Desks/tables will be free and clear of all belongings, including, but not limited to backpacks, food, purses, etc. The only items on the desks/tables should be the assigned textbook and workbook, pen/pencil, and paper for taking notes if necessary. 5. Bottled water is permitted on desks/tables in a clear plastic bottle. Any other food and/or drink are prohibited in the classroom. NO FOOD OR DRINK IS ALLOWED ON OR NEAR THE COMPUTERS! 6. Participation is required during class time; therefore students are prohibited from laying head down and/or sleeping during class time. 7. Only one person will speak at a time. Please raise your hand to be addressed by the instructor. 8. Be respectful towards fellow classmates, teachers, and the classroom environment. Profanity, obscene gestures, horseplay, or any other inappropriate behavior is prohibited. 9. Use of the restroom will be provided at assigned breaks or approval from the instructor. 10. Students will be accountable for all material covered in class as well as material in the text. Class activities, such as discussions, completed for a grade cannot be made up and will result in a zero. 25 HEALTH SCIENCES - SENIORS CONSENT FOR STUDENT PARTICIPATION IN HEALTH SCIENCES PROGRAM CLINICAL ROTATIONS (All Parents/Legal Guardians and Students Must Sign) In consideration for the opportunity of (Student) To participate in the Special School District of St. Louis County ( SSD ) Health Sciences Program, (Student) and (Parent/Legal guardian of student) agree to and acknowledge the following: 1. Health Sciences Program includes learning opportunities at clinical sites located in hospitals, nursing homes, and physician s offices ( Clinical Sites ). Parent authorizes Student to participate in learning opportunities at any and all Clinical Sites. 2. As a pre-requisite to participating in the Health Sciences Program, Student must provide proof of; a tuberculin skin test; a negative five panel urine drug screen; a clean criminal background check; and a medical exam indicating ability to participate in the Health Sciences Program. The Student s exam must indicate any activity restrictions. Students must also provide proof that Student has initiated a series of Hepatitis A vaccines. 3. Student s clinical duty at the Clinical Sites may include exposure to: needles or other sharp objects; toxic/caustic chemicals; dust/fumes/gases (including asbestos); bio-hazardous waste; potential electric shock; x-ray electromagnetic energy; blood or body fluids; and communicable diseases including but not limited to Cytomegalovirus ( CMV ), Parvovirus, Tuberculosis, HIV, Staphylococcus, and Hepatitis. 4. The Health Sciences Program will implement all reasonable safeguards to avoid Student s exposure to body substances and these safeguards will follow the Center of Disease Control s Recommendations. However, the exposure of students to the items set forth in paragraph 3 above could pose a health risk not only to Student but also to any fetus that Student is carrying. 5. Student must possess certain physical capabilities in order to participate in the Health Sciences program are set forth in the Health Science Career Education course description which is attached to and made a part of this Consent. 6. Prior to any direct patient care, the Health Sciences Program will provide Student with instruction in correct body mechanics and will require Student to have mastered correct body mechanics. However, the physical demands of the Health Sciences Program could pose a health risk not only to Student but also to any fetus that Student is carrying.

27 HEALTH SCIENCES - SENIORS Health Sciences Career Education Course Description The Health Sciences program teaches the skills necessary for a foundation in healthcare. Through a combination for classroom activities and actual on-the-job clinical experiences I nursing homes, hospitals, and physician s offices, students learn human anatomy and physiology, medical terminology, disease processes, CPR, first aid, vital signs, human relation skills, and basic healthcare skills. It is imperative to be able to think and reason for this career field. Upon completion of the program, students may have the opportunity to take the Missouri State Test as a Certified Nurse s Assistant exam. If the student passes the exam with an 80 percent and has completed all clinical requirements, he or she may be certified as a nurse s assistant by the State of Missouri, Seniors may qualify to attend an off-campus program in a nearby acute care hospital or nursing home. Graduates may qualify for advanced standing at the college level. Off-Campus Clinical Program As a pre-requisite to participating in the off campus Health Sciences program, students must provide proof of: a negative tuberculin skin test or negative chest x-ray; a negative five panel urine drug screen; a clear criminal background check from the Missouri State Highway Patrol; and a medical exam indicating ability to participate in the Health Sciences Program,. Students must also initiate a series of Hepatitis A vaccines. For those who qualify, the off-campus program includes a component in which students provide direct care to patients in a clinical setting. The clinical assignments are dependent on the job duties assigned to students by the acute care hospital or nursing home. Students will receive instruction in and must have the physical ability to perform a variety of tasks including but not limited to; bathing patients and attending to their personal hygiene needs; repositioning and ambulating patients; assisting patients with bedpans, urinals and commodes; turning and repositioning patients; lifting patients or items weighting at least 25 pounds and up to 75 pounds; pushing or pulling patients or items weighing a least 40 pounds and up to an average of 75 pounds; feeding patients and passing patients meal trays; and cleaning patients rooms. Students will receive instruction in and will be required to master correct body mechanics and body substance precautions. Students must be able to use correct safety techniques and initiate fall precautions within the policies and procedures of the clinical setting. Students in the off-campus program will likely be exposed to: needles or other sharp objects; toxic/caustic chemicals; dust/fumes/gases (including asbestos); bio-hazardous waste; potential electric shock; x-ray electromagnetic energy; blood or body fluids; and communication diseases including but not limited to Cytomegalovirus ( CMV ), Parvovirus, Tuberculosis, HIV, Staphylococcus, and Hepatitis.

28 Because providing direct patient care is physically demanding, students must be able to meet the required physical demands of their clinical assignment. These physical demands may include extended periods of: lifting; standing; walking; running to respond to patient emergencies; stooping; kneeling; crouching; climbing stairs; maintaining good balance; using a two hand carry; gripping and pinching; performing frequent repetitive motions; trunk flexion and rotation; and hand coordination, including reaching, grasping, fingering or feeling. Students must have sufficient visual and audio acuity necessary to perform patient care. A change in physical status that requires activity limitation may result in the need for a student to take a leave of absence from the Health Sciences program. Students in the off-campus program must be able to support the patient care team while learning under the direction of the SSD instructor and clinical site staff. Students must promote team-building with the SSD and clinical site staff by maintaining an attitude which promotes a healthy, supportive environment for residents and staff. Students must demonstrate behaviors which promote professionalism in the arears of: communication; organization; attire; and demeanor. Due to the patient focused objective of this learning experience, insubordination and disrespectful behavior is not allowed and students must exhibit good interpersonal skills. Students must be good stewards of the clinical site s resources. Students must strictly follow patient confidentially requirements. Attendance The State of Missouri requires that students enrolled in the Certified Nurse Assistant program complete 75 hours of classroom instruction and 100 clinical hours. Students who are absent from school while enrolled in this program may be ineligible to take the Missouri State Certified Nurse Assistant Exam or attend the off-site clinical program. The Health Sciences program does not allow for an entire semester of independent study.

29 HEALTH SCIENCES SENIORS Health Science Check-off list The following must be completed or purchased before the start of the school year: Medical Needs: The following are mandatory to participate in the clinical experience 8/26/2014. I suggest taking this check list with you to show to your Health Care Provider. Negative PPD- small needle used on inner side of forearm to test for Tuberculosis. Physical Exam The exam must state that the student is able to participate in the Health Science Program without any restrictions. Students with restrictions of any kind will not be allowed to be involved in the clinical experience until released by their Health Care Provider. If the restriction is for a prolonged time the student may need to choose another program or return to their home school. Missing clinicals for any reason may render the student ineligible to sit for state C.N.A. exam Influenza Vaccination-This is mandatory per our clinical site and must be obtained by 11/02/15. There are no exceptions. Uniform-Please refer to attachment#2 Reminder-tattoos on the hand or on the neck (that cannot be covered with a white turtle neck) are not allowed in the program. 31 HEALTH SCIENCES - SENIORS Health Sciences Uniform Policy Professional Dress Code Ceil blue scrub top and pants. Solid ceil blue lab coat. The temperature is difficult to control. No sweaters, sweatshirts, coats, jackets or sunglasses can be worn. White leather shoes and white socks. Plain white top under scrub top. Colored shirts are not allowed. No Street clothes under uniform. Short fingernails. Only clear polish. No long or artificial fingernails; no extenders, acrylic wraps or nail art. No more than two pair post earrings and a wedding band may be worn. No other jewelry allowed. Hair color to be natural tint, only a color that grows naturally from the scalp, for example no orange, burgundy or blue hues. Long hair pulled back and secured off the face. No scarves or curlers may be worn in hair. NO facial or tongue piercing. NO tattoos or passion marks may be visible. Picture ID badge required. Watch with second hand. Black i nk pen and pocket notepad. Stethoscope-any color but yellow Gait belt-for long term care rotation. It can be purchased from clinical site at the beginning of that rotation. If purchased on own the buckle cannot be plastic. The buckle part must be metal. Occasionally, students will be allowed to dress in street clothes on non-clinical days for a donation of 50 cents. To dress out is VOLUNTARY. If the student does not want to participate in this voluntary community service project then the HES uniform is to be worn. The money will be donated at the end of the school year to charity. The 50 cents must be paid at the start of the class. If a student does not have the money for a donation and is out of uniform then said individual will lose the dress out privileges for two weeks. If this continues to be a problem then the student will be required to come in uniform every day for the remainder of the semester. This event is scheduled or revoked at the discretion of the instructor.

If not present at this meeting, you will have to meet with the Assistant Principal, Mr. Jones and the Instructor before the student will be able to go out on clinicals which are scheduled to begin on August 26 th. 35 NETWORKING ADMINISTRATION Students will be required to wear Cisco Networking Academy shirts daily. There will be opportunities to earn Free Dress Days, but plan on having enough shirts to wear every day. The same shirt stored in your locker and worn every day is not acceptable. The shirts must be clean and not wrinkled. An opportunity to order shirts will be in the FIRST WEEK OF CLASSES. You will be able to choose sizes, colors, and styles, but plan on purchasing 3-4 shirts so a clean shirt can be worn to class each day. Shirts are generally around $20.00 each. The money is due for your shirts by the end of the first week of class. Until ordered shirts arrive you should wear a collared shirt if possible. There is no requirement for pants to be worn, however if you have more than two violations for sagging pants, you will be required to wear khaki pants and a belt with shirt tucked in. Items needed for class: Four 3-ring binders-maximum 1 inch.(one for each semester). 37 VETERINARY ASSISTANT PROGRAM NOTE: Ms. Koehler s 1 st hour class for first year students will have a separate syllabus handed out on the first day of class by Ms. Koehler. Uniform: 1) For Juniors, students will be assigned either RED TOPS or HUNTER GREEN TOPS based on their last names. Students will be assigned a color on the first day of school and will have 1 week to buy the scrubs. It is very important that the scrubs are these exact colors since this determines which lab group they are in. Other shades of these will be asked to be returned. Students may wear any color bottoms they choose. Seniors are allowed to wear any top or bottom they choose. 2) Tennis shoes are the only type of shoe allowed when working with animals. They may be any color and must be tied. 3) NO hoop earrings are allowed at any time when working with animals. NO necklaces will be worn in shop either. These are very serious safety concerns when working with unpredictable animals and they are taken seriously in this program. 4) NO long fingernails. When looking at the palm of the hand, nails should not be able to be seen. Veterinary team members, whether male or female, just do not wear long nails. This is a very serious sanitary issue and it is taken seriously in this program. 4) Juniors are expected to be in uniform WHEN THEY ARRIVE TO ANATOMY AND PHYSIOLOGY at 7:30. This is the first class of every day. Juniors changing into scrubs upon arrival to anatomy will be marked as tardy. Seniors will be allowed 10 minutes to change into their uniforms between 12:00 and 12:10 each day. 5) IMPORTANT- Any failure to meet the above requirements will result in students being sent immediately to the in-school suspension room for the remainder of anatomy and veterinary class. Warnings will not be given, but students will be reminded of these policies on the first day of school. Other materials suggested for veterinary class three-ring binders, 1 for anatomy and 1 for veterinary 2. 1 large notebook for anatomy and 1 small notebook for veterinary 3. Approximately 300 3x5 index cards for vocabulary flash cards. Also, a plastic index card case is highly recommended. Students will have 20 new vocabulary cards each week, year-round. A final note: Students will receive a daily class participation grade for being dressed in proper uniform and having supplies and homework brought into the classroom when class begins. Students will lose daily class participation points when they are not prepared and on time. Remember, the technical programs are job-prep programs and students will have the same responsibility expectations as someone who is working in the veterinary field.
