5 The Caregiver Only course is designed for individuals wanting to work in the Assisted Living or Home Care industries. 111 Caregiver Training Course Caregiver fulfills all requirements for the NCIA Board to earn a Caregiver Certificate and sit for the state exam. Classes include required courses in: Caregiver settings, legal and ethical issues, communication and interpersonal skills, job management skills, understanding service plans, infection control, nutrition, fire safety and emergency measures, maintaining a comfortable environment, basic caregiver skills, mental health and special needs care, cognitively impaired care, restorative skills and activities, medication training, and skill lab. 116 hours that will be offered during the day and/or evening. Supplies include: Hartman Textbook, Foundation of Caregiving Supplemental, Dementia Care Foundations Textbook, Medication Management Textbook, People Skills Manual, Lab Skills and Cooking Principles. State exam included in price. Day days, Evening - 35 days. Some online work required COSTS Tuition: $950 Supplies: $160 PAYMENT SCHEDULES Financing is available if you qualify. No interest/no debt financing is also available. 6 COMPLETION REQUIREMENTS To complete the course and be scheduled for the NCIA State exam. o Student must have a 75% score or higher on the Caregiver final exam. o Student must have a 75% or higher on the Dementia final exam. o Student must have a 75% on the Medication exam. o Case studies and online classes must be completed satisfactorily. All fees owed to CGTI must be paid in full. CPR and First Aid (Required to gain employment) will be offered. (An Additional Fee of $45 will be charged.) No time can be missed or the minimum hours required cannot be met. You must then take a state caregiver exam. State certification is received after successfully passing the state caregiver exam with a 75% score. What Qualifications must be met to be certified as a Caregiver? 1. Current CPR card and basic first-aid card 2. Have a valid fingerprint clearance card 3. Complete a 104 hour caregiver class from an approved institution 4. 7 The Caregiver Bridge course is designed for individuals who have an Arizona CNA certificate and want to work in the Assisted Living or Home Care industries. 112 Caregiver Bridge Course This course provides people who have an Arizona CNA certificate in good standing to take a class to earn their Caregiver Certificate. The class includes: Assisted Living orientation, service plans and medication. Supplies include: Medication Textbook. 24 hours to be completed in 8 modules of 3 hours each. COSTS Tuition: $250 Supplies: $30 PAYMENT SCHEDULES All fees must be paid prior to registration CLASSES SCHEDULES Day Classes (24 hours) 8:30 AM to 3:00 PM 4 days 3:15 PM to 6:15 PM 8 days Our Tucson Center is located south of 22 nd and the Main Cherrybell Post Office, and East of Kino Parkway, across the street from the Goodwill Distribution Center, on Silverlake Road. 9 The Caregiver + CNA Course is designed for individuals wanting to work in the Assisted Living, Long Term Care, Hospital, Hospice or Home Care industries. 121 Nursing Assistant Plus Caregiver Training Course CNA Plus Caregiver is approved by the Arizona State Board of Nursing (AZBN) and the NCIA Board for graduates to sit for the state exams and earn Certified Nursing Assistant and Caregiver Certificates. Classes include: Skills, Body Systems, Basic Knowledge, Nutrition and Cooking, People Skills, Death and Dying and Clinical. This course is 212 hours in length and can be completed in 6 weeks of day classes (8:30 am to 3:00 pm) or 10 weeks of afternoon classes (3:15 pm to 6:15 pm) or evening classes (5:15 pm to 8:15 pm, or 6:30 pm to 9:30 pm) Supplies include: Stethoscope, BP cuff, Gait belt, one set of scrubs, CNA Textbook, Caregiver Supplemental Textbook, Medication Textbook, Directed Care Textbook, Nutrition Textbook, People Skills Workbook, and Skills Manual. Both state exams are included in price. 212 hours, Day 6 weeks, Afternoon & Evening 10 weeks Some online work required TUITION Tuition: $2,850 Supplies: $400 PAYMENT SCHEDULES If a student is eligible for government funding through Pima County One-stop or DES, we will recommend that they pursue this option. Financing programs are also available if a student qualifies. A no-interest, no-debt pay plan is also available; however, all fees must be paid by the middle of the course. CRITERIA FOR ADMISSION Students must be over 17 years of age High School diploma or GED is recommended High School diploma or minimum score of 7.0 on reading test Negative TB skin test or chest x-ray CPR training (Healthcare Provider level) DPS fingerprint clearance card Be able to lift or push or pull 50 pounds Reliable transportation Commitment to attend all classes Willing to work FT or PT after training Current government issued photo ID Proof of legal residency in U.S. to take state exam. COMPLETION REQUIREMENTS To complete the Nursing Assistant Program and be scheduled for the state CNA exam. o Student must have an 80% score or higher on the final exam. o Student must receive a B or A in the Skills final exam. o Student must receive a B or A in the Clinical experience. o Student must receive the endorsement of their RN faculty. To complete the Caregiver Program and be scheduled for the state caregiver exam. o Student must have a 75% score or higher on the Caregiver final exam. o Student must receive a C in the skills final exam. At least 40 hours of clinical must be completed. Missed time will have to be made up in order to take the CNA exam. No more than 15% of the total hours excluding clinical can be missed. 11 You must then take a state caregiver exam. State certification is received after successfully passing the state caregiver exam with a 75% score. You must then take a state CNA exam (written and skills). State certification is received after successfully passing the state caregiver exam with a 75% score and passing the skills portion of the exam. What Qualifications must be met to be certified as a Nursing Assistant? R A. File an application with the Arizona State Board of Nursing 1. Show proof of satisfactory completion of a nursing assistant training program that meets the requirements in subsection (B); 2. One or more fingerprint cards, 3. Applicable fees B. An applicant for certification as a nursing assistant shall submit a passing score on a Boardapproved nursing assistant examination and provide one of the following criteria. 1. Proof that the applicant has completed an AZBN Board-approved nursing assistant training program. 2. Proof that the applicant has completed a nursing assistant training program approved in another state or territory of the United States consisting of at least 120 hours. 3. Proof that the applicant has completed a nursing assistant program approved in another state or territory of the United States of at least 75 hours of instruction and proof of working as a nursing assistant for an additional number of hours that together with the hours of instruction, equal at least 120 hours, or 4. Proof that the applicant either holds a valid nursing license in the U.S. or territories, has graduated from an approved nursing program, or otherwise meets educational requirements for a registered or practical nursing license in Arizona. C. An applicant who fails either a written or manual skills portion of the nursing assistant examination may take the failed portion of the examination until a passing score is achieved. An applicant shall pass both portions of the nursing assistant examination within two years from the date of completion of the nursing assistant training program or meet the requirements in subsection D D. An applicant who does not pass an examination within the time period specified in subsection (C) shall repeat and complete a training program before being permitted to retake and examination. E. An applicant who has never taken the examination and provides proof of at least 160 hours of employment as a nursing assistant for every two-year period since completing a state-approved nursing assistant training program meets federal requirements to take the written and manual skills nursing assistant examination. F. The AZBN Board shall certify an applicant who meets the applicable criteria in the Article 8 if certification is in the best interest of the public. G. An applicant who is denied nursing assistant certification may request a hearing by filing a written request with the AZBN Board writing 30 days of service of the Board s order denying the CGTI Course Catalog Page 11

12 application for certification. Hearings shall be conducted in accordance with A.R.S. Title 41, Chapter 6, Article 10 and 4 A.A.C. 19, Article 6. CNA RENEWAL Certified Nursing Assistant Certificate Renewal (R ) A. A certified nursing assistant may apply for renewal of a certificate by submitting an application to the Arizona State Board of Nursing on or before the expiration date of the certification. 1. The application packet shall include all required information 2. Documentation of proof of employment, such as a pay stub, W-2 form, or letter from an employer that verifies the applicant s employment as a nursing assistant or the applicant s performance of nursing assistant activities for a minimum of 160 hours within the past two (2) years, and 3. Applicable fees. B. The certificate of a nursing assistant who fails to renew expires on the last day of the month of a certificate holder s birthdate. 1. A nursing assistant s responsibility to renew is not relieved by the nursing assistant s failure to obtain an application. 2. A nursing assistant who fails to renew shall not work as a certified nursing assistant. 3. Based on consideration of a nursing assistant s record regarding timely renewal, the Board may impose a late fee on a nursing assistant who fails to renew certification in a timely manner. 13 182 ASSISTED LIVING MANAGER CEU Assisted Living Managers are required to earn CEUs over a 2 year period to keep their licenses current. CGTI will offer CEUs on different topics. We normally will schedule a class when we have enough interest and base it on a subject that will allow the ALMs to discuss and learn new best practices. The normal class is 8 hours and earns 8 CEUs. 15 CGTI also provides vocational courses designed for individuals working in the Health Care Fields to improve their skills. 090 Pre-requisite Classes 091 CPR/AED Adult & First Aid Red Cross approved class and provides the student with a 2 year certificate. It includes online study, review and test. Tuition: $ SERVSAFE Food Safety Management Training 2 to 4 hours, Designed for managers The ServSafe program prepares students for the ServSafe Food Protection Manager Certification exam. Course is taught by a ServSafe certified instructor. 093 The SERVSAFE Food Handler Program 2 hours The ServSafe Food Handler is a food safety training for employees. Course is taught by a ServSafe certified instructor. 600 to 699 Career Improvement Courses Caregiver, CNA, and Family Caregiver Improvement These classes can range from 2 to 16 hours of education and are designed to assist employed caregivers, CNAs or family caregivers, improve their knowledge and skills. 16 PAYMENT SCHEDULES Tuition paid prior to beginning date. CLASSES SCHEDULES As scheduled for each course 610 Skills a-la-carte 3 to 8 hours depending on the requirements of the Company Organizations that wish to improve the skill level of their staff Companies can identify skills they would like taught and CGTI will create a custom class. We will demonstrate the skill, allow students to practice and then sign them off on the skills. There is normally a class size restriction depending on the number of skills and which skills are taught. INSERVICE CONTRACTED SERVICES Cost: Depends on length of class and number of faculty required These classes are offered based on requests from an organization or several organizations that would like to offer a specific class. More participants in class lower the per student cost. Why do organizations have CGTI provide these classes? Expertise, lower cost, less disruption to work schedules, and certification. 18 Refund Policies (Courses 150 hours and over) The following policy will be followed by CareGiver Training Institute (CGTI) in the event a student cancels or terminates their enrollment in a CGTI Program. Rejection: An applicant rejected by the school is entitled to a refund of all monies paid by the student directly to CGTI. Three-Day Cancellation: An applicant who provides written notice of cancellation within three days (excluding Saturday, Sunday and federal or state holidays) of signing an enrollment agreement is entitled to a refund of all monies paid. No later than 30 days of receiving the notice of cancellation, the school shall provide a 100% refund of all fees and tuitions paid by the student directly to CGTI. If a textbook has been proived the cost of the textbook will be retained by CGTI. Other Cancellations: An applicant requesting cancellation more than three days after signing an enrollment agreement and making an initial payment, but prior to entering the school, is entitled to a refund of all monies paid by the student directly to CGTI minus a registration fee of $ Refund after the Commencement of Classes: 1. Procedures for withdrawal/withdrawal date: a. A student choosing to withdraw from the school after the commencement of classes is to provide written notice to the Director of Education. The notice is to indicate the expected last date of attendance and be signed and dated by the student. Nonattendance does not indicate withdrawal. b. If a student is on an authorized Leave of Absence and fails to return from that leave, the withdrawal date is the scheduled date of return. c. The withdrawal process will be initiated if the student has not attended any class for 5 consecutive days. 2. Tuition Charges a. Before the beginning of classes, the student is entitled to a refund of 100% of the tuition paid directly to CGTI, less the registration fee. b. After commencement of classes, the tuition refund amount shall be determined as follows. Tuition refunds will be issued within 30 days of the date of student notification, or date of school determination (withdrawn due to absences listed in the student handbook), or in the case of a student not returning from an authorized Leave of Absence (LOA), within 30 days of the date the student was scheduled to return from the LOA and did not return. 3. Tuition Refund a. Before the beginning of classes: The student is entitled to a refund of 100% of the tuition paid directly to CGTI, less the registration fee ($100). b. After commencement of classes: The tuition refund amount shall be determined as follows. i. Tuition charges will be determined based upon the percentage of the clock hour s student is registered in class. The percentage of the clock hours attended is determined by dividing the hours registered by the total number of clock hours. The tuition is then multiplied by percentage of the clock hours registered. c. Books, Supplies and fees: Students will not receive a refund for supply or textbook costs once they receive the supplies as the equipment cannot be reused. d. Refunds: Will be issued within 30 days of the date of student notification, or the date of school determination (withdrawn due to absences or other criteria as specified in the school catalog), or in the case of a student not returning from an authorized Leave of Absence (LOA), within 30 days of the date the student was scheduled to return from LOA. e. Special Cases: In case of prolonged illness or accident, death in the family, or other circumstances that make it impractical for the student to complete the program, a reasonable settlement will be reached with the student. Special Cases: In case of prolonged illness or accident, death in the family, or other circumstances that make it impractical for a student to complete the program, the school may make a settlement, which is reasonable and fair. Other Policies Holidays The school suspends classes on the following holidays: New Years Day Memorial Day July 4 th Labor Day Thanksgiving Day The Day After Thanksgiving Christmas Eve Day Christmas Day Student Services CareGiver Training Institute has an academic counselor on staff to work with student. Based on discussions and classroom results, the counselor will may work with students to identify needs and provide strategies and/or tools. Student Attendance & Timeliness Attendance Standards Students are prepared for a career in the healthcare field; we expect the same attendance standards as employers. Any missed classes or portions of classes are tracked in minutes. CGTI Course Catalog Page 20

21 Expectations Students are expected to attend every class, skill lab, and clinical assignment. The objective is to learn all information and skills required for the job and to meet state guidelines for training time in each setting. Missed time will not allow these requirements to be met. All students are expected to be present for all classes, labs and clinical at least five minutes prior to the sessions scheduled start time. Just like a job, students must be there and ready to work at the beginning of the shift. All students are expected to remain and actively participate in the class, lab or clinical for the entire scheduled time. Absenteeism Being absent from 20% of the class hours, excluding clinical hours, will result in dismissal from the program and the necessity of repeating the program. Absences less than 20% of total class hours, excluding clinical hours, will result in the student having to make up those specific class hours (modules) in a succeeding class. Courses under 100 hours, attendance is mandatory. No absences are allowed. If the student misses a lab, he/she is required to learn and pass the skills from that lab, prior to the next clinical, or he/she may be denied participation in the clinical because it may threaten the safety of the clients. If the student misses a clinical, he/she is required to make up the clinical before receiving his/her certificate of completion which is necessary to take the State CNA examination; if the student cannot be scheduled into an existing clinical, a charge may be assessed to set up a make-up clinical. If the student has a situation beyond their control that will force him/her to miss a class, lab or clinical, CGTI requires advance notice. To insure the student s message is received, he/she must do the following: o Call the CGTI office ( ) and talk to a staff member or leave a message that explains the situation why the student cannot attend the class or lab. o If a clinical is to be missed, call the clinical instructor at the number provided or call the CGTI office. Leave a message or talk to someone to inform them that you cannot make your assigned time. Students who have an absence are required to learn all information that was presented at the missed class or lab. Any missed clinical time is expected to be made up. Upon request from the student, the CGTI Director of Education will reschedule the clinical. A student will not receive a certificate of completion until the session is made up. Timeliness If the student is late for the beginning of the class, the time will be recorded in minutes. If the student leaves early, the time will be recorded in minutes. CGTI Course Catalog Page 21

22 Attendance Probation Probation: This will be based on the student s attendance and timeliness. No Call, No Show: Any no call and no show will automatically place you on attendance warning. If Absences and/or Tardiness increase, students will be placed on probation. Program Dismissal A student will be dismissed from the program after missing 20% of the required class hours. This is separate from graduation requirements. Class Cancellation Normally this does not happen; however, CGTI has the right to cancel or change the dates of the class if there is insufficient enrollment. Change in Student Status CareGiver Training Institute does not have a Leave of Absence policy. If you miss more than 20% of the hours, you will be forced to drop the class as you will not be able to catch up and have the necessary hours and information required for the certifications. However, we will give the student priority in registering for classes that begin at a later time Credit for Previous Education, Work or Life Experience CGTI does not provide tuition credit for previous education, training, work or life experience. Dress Code CGTI students are expected to dress appropriately and professionally for the class in which they are enrolled. Please see section 12 of the student handbook for more specific guidelines. Harassment/Bullying/Sexual Harassment Policy CGTI is committed to providing an environment free of harassment, bullying and sexual harassment. Harassment, bullying and sexual harassment are defined as conduct that substantially interferes with another s performance or creates an intimidating, hostile or offensive environment. This would include but not be limited to harassment and/or threatening or offensive conduct directed toward a person s gender, race, age, disability, religion, national origin, sexual orientation or veteran status. Sexual harassment can differ from other types of harassment. It is defined as any unwelcome or sexual advances, requests for sexual favors, and other verbal or physical conduct of a sexual nature. Section 14 of the Student handbook provides additional guidance. Nondiscriminatory Policy CGTI admits students of any race, color, national and ethnic origin to all the rights, privileges, programs and activities generally accorded or made available to students at the school. It does not discriminate on CGTI Course Catalog Page 22

23 the basis of race, color, national and ethnic origin in administration of its educational policies, admissions policies, scholarship and loan programs, and other school-administered programs. CGTI endeavors to provide an environment wherein human dignity prevails and all students and all student applicants receive equal consideration and treatment. No person is granted special privilege or consideration nor is it withheld because of race, creed, color, religion, national origin, age, gender, sexual orientation, veteran or reservist status, handicap/disability, or any other protected classification. CGTI will assess whether any reasonable accommodation would allow a person with a mental or physical disability to perform the required tasks of a student. An accommodation which creates an undue financial hardship on CGTI or which endangers health or safety is not a reasonable accommodation. If you have a handicap/disability that requires special accommodations you are encouraged to notify CGTI. You are expected to conduct yourself in a manner that is not offensive to the gender, sexual orientation, race, color, age, creed, religion, national origin, handicap/disability, veteran or reservist status, or any other protected classification, of anyone in our community. Pregnant Students Pregnant students must have a written medical release from a licensed health care provider (M.D., D.O., N.P.) stating that the student may participate in the skills lab and clinical sessions. If they cannot lift or move at least 50 pounds by the clinical sessions, they may have to take a leave of absence from the program. Probation/Suspension Students have to meet strict attendance and timeliness standards as well as other expectations as listed in the Student Handbook. Students who violate those issues can be given a warning, placed on probation or terminated depending on the seriousness of the incident and the rule that was violated. Rules that impact client rights, confidentiality or safety and rules impacting relationships with faculty, staff and students are also serious. Depending on the action and the result, a student could be suspended if the violation is of a serious nature. Requirements for Registration In order to register for the class, the student must complete all requirements as to meetings and seeing agencies to which they are referred. In addition, prior to the first day of class, each student must have a TB test and CPR Training. The student must provide the counselor with these two documents so copies can be made. Failure to comply with these requirements allows CGTI to refuse registration in the course. Satisfactory Progress Every student s ongoing performance is evaluated by his/her Instructor throughout the course. A written evaluation/grade is given for every test/quiz, each skill on the final skills test, and for clinical performance. The Instructor will evaluate the student s ability to follow directions and work as a team member and his or her ability to demonstrate ethical and legal conduct. The Instructor will also evaluate CGTI Course Catalog Page 23

24 the student s time management skills, and his or her participation level and cooperation during class, skills lab, and clinical assignments if any. In the judgment of the faculty and staff, if a student is considered unsafe or unethical, they will be subject to dismissal from the program. Would they have this student take care of them or a family member? Would they work with the person as a colleague? Student Re-enrollment A student who drops the course is eligible to reapply for admission to another class unless they were dismissed for reasons that would endanger clients in the future. All students requesting reinstatement into any program offered by CGTI will be required to go through a portion of the admissions process again. They will be required to pay a $25 reinstatement fee. Student Probation, Suspension or Expulsion Failure to meet academic, skills lab, and clinical performance expectations and/or to follow CGTI policies, procedures, and Code of Integrity may result in probation, disciplinary action, and/or dismissal from the program. CGTI will use probation, disciplinary action, and dismissal when it is deemed appropriate and at its sole discretion. In some instances, probation, disciplinary, and/or dismissal will not be used at all. Probation may be for academic, attendance, and/or clinical issues. Disciplinary action may include: Warning - a verbal warning or written warning for the first offense. Probation a written probation statement Suspension Dropped from the certificate program One action may or may not immediately follow another action. When the student behavior is intolerable, the first action taken may be dismissal from the program without a verbal or written warning. See the student handbook, section 12, to see examples of unacceptable behavior. (These are examples and are not limited.) Student Records Student records are collected and maintained to help in the instruction, guidance and educational progress of students. Records are maintained in the Administrative offices. Students have the right to review their records and follow procedures to modify records they believe to be inaccurate. However, records will not be released to any other agency without an authorized signature unless they paid for your tuition partially or fully. CareGiver Training Institute does not assign grades or provide transcripts. The grades on the student transcript will be Pass or Fail or Withdrew. Contact the office for a copy of the transcript. 25 Student Grievance Procedure When people come together, problems and complaints occasionally arise. Whatever the problem, CGTI is concerned that the student be given the opportunity to resolve it. CGTI will not tolerate unfair treatment or discrimination of a student by another student, instructor, or staff member. If a student feels that he/she has been harassed or discriminated against, the student may utilize the Student Grievance process. To obtain a fair review of actions that may appear to be harassing, discriminatory or unfair in any way, the student should first discuss the issue with the Instructor. If discussing the issue with the instructor does not satisfy the concern, the student is encouraged to use the following formal Student Grievance process: 1. Submit a written description of the concern, the events leading to the concern, witness names, if any, and actions that have been taken in an attempt to resolve the concern to the CGTI Manager of the School. 2. Submit a written description within five (5) days of the event causing the concern. 3. The CGTI CEO, or designee, will meet with the student within two (2) working days of receipt of the written concern. 4. Any unresolved grievances may be independently addressed as the situation warrants. If the complaint about a CNA issue cannot be resolved after exhausting the Institution s grievance procedure, the student may file a complaint on nursing assistant issues with the Arizona State Board of Nursing. The student must contact the Arizona State Board of Nursing for further details. The Arizona State Board of Nursing address is 4747 N. 7 th Street, Suite 200, Phoenix, AZ Transportation (You will also find this statement on page 23 of the Student Handbook) Every student is responsible for his or her own transportation to and from classes, skills lab, clinical locations, cooking classes and test locations. Tuition Requirements (Adult Classes Only) Students are responsible for prompt payment of tuition, fees or any money owed to CGTI. Failure to maintain payments current may result in not being eligible to attend classes, skills labs, clinicals, be scheduled for testing and/or receive certifications. Violence CGTI strives to maintain an environment free from intimidation, threats or violent acts. This includes, but is not limited to, intimidating, threatening or hostile behaviors, physical abuse, vandalism, arson, sabotage, use of weapons, carrying weapons of any kind onto CGTI property or our associate s property, or any other act, which is inappropriate in the academic environment. Students who observe or have knowledge of any violation of this policy should immediately report it to their Instructor and CGTI. 26 Job Opportunities CareGiver Training Institute provides training to enable the student to work as a caregiver or CNA. Depending on the program, the student will earn various certificates at the time of graduation or upon successful passing of the state exam. The Caregiver certificate you will get after passing the state caregiver exam, allow you to work in homecare, large assisted living centers and smaller assisted living homes, commonly referred to as adult care homes. The Certified Nursing Assistant (CNA) certificate you will get after passing the state CNA exam, allows you to work in home care, hospice, skilled nursing facilities and hospitals. Assisted Living Manager Certificate allows you to take the state exam and if you pass hold a position as a manager in an assisted living center or assisted living home.
