The Theater Medical Data Store (TMDS) serves as the authoritative database of the military health system (MHS) for collecting, distributing and viewing the following records: inpatient and/or outpatient medical treatment received by United States Active Duty Military Service Members, United States Military Reserve Members, United States Military National Guard, foreign nationals, retired United States Military Service Members, host nation detainees, NATO personnel and contractors in military treatment facilities (MTFs) outside the continental United States (OCONUS) and in forward operating bases in the theater of -operations, ancillary services records, patient movement records a nd patient tracking records. Also, TMDS collects records of medical treatment patients receive in theater battalion aid stations and from first responders on the battlefield. In addition, TMDS maintains blood inventory records of MTFs OCONUS and at forward operating bases in the theater of operations. The information iri TMDS consists of records of inpatient and/or outpatient medical treatment received by United States Active Duty Military Service Members, United States Military Reserve Members, United States Military National Guard, foreign nationals, retired United States Military Service Members, host nation detainees, NATO personnel and contractors in MTFs OCONUS and in forward operating bases in the theater of operations, ancillary services records, patient movement records and patient tracking records. (VA) TMDS outputs up-to-date patient information to the Combat Injury Pay (CIP) system in support of CIP patient tracking. (DFAS) TMDS outputs patient information for all U.S. Operation Enduring Freedom/Operation Iraqi Freedom (OEF/OIF) patients registered via the TMDS application to the Defense Finance and Accounting System (DFAS) in support of Hazardous Injury Pay (HIP)/Combat Injury Pay (CIP) pay adjustment. (DFAS) TMDS outputs information for ah U.S. patients registered via TMDS to the Medical Operational Data System (MODS) Soldier Patient Tracking (SPT)/Soldier Patient Locator (SPL). (Army) TMDS outputs inpatient and outpatient medical information for all U.S. patients registered via TMDS and other TMIP data sources to the Patient Administration Systems and Biostatistics Activity (PASBA). TMDS outputs longitudinal medical record information to the Medical Situation Awareness in the Theater (MSAT) for viewing by the Secret Internet Protocol Router Network (SIPRNET)-based MSAT. 4 Records Schedule: DAA Records Schedule Items Sequence Number Theater Medical Data Store (TMDS) Master File Disposition Authority Number DAA The Theater Medical Data Store (TMDS) provides web-based access to service member information collected at theater based medical treatment facilities (MTFs) using AHLTA-Theater, Shipboard Automated Medical System (SAMS), Global Expeditionary Medical System (GEMS), and Cache TC2. TMDS master file consists of records of medical treatment patients receive in theater battalion aid stations and from first responders on the battlefield. All TMDS medical records include patient demographic information including but not limited to: age, birth date, social security number (SSN), marital status, personal cell phone number, home telephone number, address, race, pay grade, personnel code, service identification number, mobilization status, unit identification number and unit phone number. In addition, TMDS maintains blood inventory records of MTFs OCONUS and in forward operating bases in the theater of operations. Final Disposition Item Status Is this item media neutral?
