The uthors describe the structure nd process used to lign the progrm s competencies nd curricul to the mrket needs by incorporting the helthcre community s input. The first prt of the rticle ddresses the components of the progrm development frmework, including the determintion of progrm vision nd philosophy nd the estblishment of progrm development group nd dvisory bord. The second prt concludes with discussion of the process used in the selection of competency model nd its ppliction to the course curricul. Keywords: Progrm Development, Competency, Mission, Advisory-Bord, CAHME Accredittion INTRODUCTION In 2000 the Institute of Medicine (IOM) published To Err is Humn: Building Sfer Helth System, lndmrk study which focused ttention on the need to increse qulity of cre by reducing medicl errors s thousnds of Americn deths were occurring s the result of preventble medicl mistkes (Institute of Medicine, 2000). The study concluded with clrion cll to ction nd pln centered on the need to improve ptient sfety (Institute of Medicine, 2000). In 2001 the IOM produced follow-on report, Crossing the Qulity Chsm: A New System for the 21st Century, which exmined the reltionship between ptient sfety nd qulity from systems perspective (Institute of Medicine, 2001). In this study reserchers expnded the previous focus on ptient sfety to more comprehensive exmintion of the key fctors ssocited with the qulity of medicl cre. This report noted tht qulity problems were lrgely systemic nd therefore could not be corrected by requiring helthcre providers to work hrder t providing the qulity of cre (Institute of Medicine 2001). The 2001 study identified six fctors believed to be closely relted to the qulity of medicl cre provided ptients on n individul bsis nd clled for ll helth cre orgniztions to pursue sfe, effective, ptient-centered, timely, efficient, nd equitble cre. 34

2 VOLUME 3, ISSUE 1 The Crossing the Qulity Chsm report lso recommended tht n inter-disciplinry summit be convened to develop pln to reform helth professions eduction in order to enhnce ptient cre qulity nd sfety. In the summer of 2002, the IOM convened summit for this purpose. This ntionl meeting, ttended by 150 individuls representing vrious helthcre disciplines nd occuptions, resulted in the IOM report entitled Helth Professions Eduction: A Bridge to Qulity (Institute of Medicine, 2003). This report set forth vision for ll progrms nd institutions engged in clinicl eduction, clling for eductionl reform nd recommending the integrtion of core set of competencies ssocited with the following domins: ptient-centered cre, interdisciplinry tems, outcomes-bsed prctice, qulity improvement, nd informtics. Helth mngement eductors nd prctitioners, whose rnks were well represented t the 2003 IOM Summit, were quick to see the need to increse their focus on importnt eductionl outcomes, i.e., mngeril competencies required by grdutes in order to be effective in the workplce (Clhoun, Dvidson, Sinioris, Vincent, & Griffith, 2002; Griffith, 2001; Wrden & Griffith, 2001). Further, the Joint Commission Whitepper, Helthcre t the Crossrods stted tht ccrediting bodies were lso providing strong support for outcomes-bsed, i.e., competency-bsed, eduction (2005). In 2004 the Commission on Accredittion of Helth Mngement Eduction (CAHME) estblished the requirement for ll CAHME ccredited progrms to identify set of competencies relted to the progrm s mission nd the types of positions their students would enter upon grdution (CAHME, 2004). The ccrediting body further stipulted tht the progrm s competencies would be ligned with specific helthcre mngement content which, together, would be mtched with curriculum (CAHME, 2004). Within this se of chnge in the structure nd process of providing helth services, the field of helth dministrtion eduction hs moved decisively nd concretely from teching-centered model of eduction bsed on the ssumption tht knowledge equls competency to lerning-centered model. The lerning-centered, or studentcentered, model is bsed on the ssumption tht competency is relted to the bility to demonstrte mission- nd mrket-relevnt knowledge, skills, vlues nd ttitudes. The concept of competency-bsed curriculum within mngement relted fields hs been in the literture for more thn four decdes (Boytzis, 1982; Boytzis, Cowen, & Kolb 1995l; Luci & Lepsinger, 1999; McClellnd, 1973). However, it ws not until the cll for common set of competencies by the IOM in 2003 tht helth mngement eduction progrms nd ssocited professionl orgniztions rigorously engged in efforts to identify nd develop comprehensive sets of competencies for the profession of helth dministrtion (Clhoun et l., 2002; Cmpbell, Lomperis, Gillespie, & Arrington 2006; HLA, 2005; NCHL, 2004). This rticle describes nd discusses the structure nd process used to develop new competency-bsed grdute progrm in helth dministrtion within n urbn-bsed, privte university locted in the Southwest United Sttes. Similr pproches hve been used successfully to convert existing content-bsed grdute helth mngement progrms to competency-bsed models (Cmpbell, Lomperis, Gillespie, & Arrington, 2006). However, published ccounts of the structure nd process used to crete new competency-bsed progrms re believed to be few, if ny. This pper is detiled, non-prescriptive presenttion of the frmework, process, nd outcomes relted to the development of competency-bsed grdute progrm in helth dministrtion, within School of Business. The first section of this pper ddresses the components of progrm development frmework. Key spects of this frmework involve the determintion of progrm vision nd philosophy, the estblishment of progrm development group, nd the development nd ppliction of n dvisory bord. The second section presents the process used in the selection of the competency model nd development of the curriculum. Progrm Mission, Vision, nd Philosophy PROGRAM DEVELOPMENT FRAMEWORK A foundtionl step in the development of the University of the Incrnte Word (UIW) Mster of Helth Administrtion progrm ws the formultion of progrm vision nd philosophy sttement built upon mission tht centered on the need for competency-bsed curriculum built upon the eductionl requirements of individuls lredy working 35 b

3 in the helthcre industry. The first step in the vision nd progrm philosophy cretion process ws the ppointment of full-time UIW fculty member with experience in cdemic progrm development, dministrtion, nd teching within the field of helthcre dministrtion to led progrm development efforts. This individul lso hd significnt experience s helthcre dministrtor prior to pursuing creer in cdemics nd hd been the director of ntionlly rnked grdute progrm in helth dministrtion. The following ws used s guide to drft the initil progrm vision nd philosophy sttement. Drucker s seminl work on the mngement of non-profit orgniztions (1990) emphsized the importnce of mission clrifiction in order to nswer the following questions: Why does the orgniztion exist, i.e., wht is its purpose? In other words, whom does it seek to serve wht need is it seeking to meet? Wht differentites this prticulr orgniztion from those tht re similr, i.e., wht vlues chrcterize the orgniztion nd wht services will be provided? A multi-dimensionl mission component structure developed by strtegist Fred Dvid provided guide tht helped nswer these nd other importnt mission relted questions (2010). The components of Dvid s model used in this process re discussed in the subsequent prgrphs. Mrket. Who is being served? The proposed progrm ws to be locted in lrge urbn setting of over one million people, with Hispnic popultion of pproximtely 63%. The university in which the progrm is housed lso drws students from geogrphiclly dispersed rurl re to the south of the city. At the time the progrm ws developed, the grdute helth dministrtion eduction mrket ws served by one full-time trditionl Mster of Helth Administrtion (MHA) progrm, which offered courses in trditionl 16 week dytime formt. The mrket ws lso served by two MBA progrms, which offered concentrtions in helthcre dministrtion. During the time this progrm ws being developed, individuls who worked full-time nd wished to pursue n MHA in fce-to-fce formt did not hve the bility to do so. Trgeted Students. The progrm ws designed to meet the eductionl needs of working dults, especilly, those lredy employed within n economiclly significnt nd growing helthcre industry. At the time the progrm ws developed, the helthcre nd bioscience industry hd n economic impct of $14.3 billion on the Sn Antonio MSA (Greter Sn Antonio Chmber of Commerce, 2005). Further, the city hd enjoyed n increse of 20,000 jobs from 1995 to 2005, nd over 108,000 individuls were employed in helthcre nd biosciences orgniztions in 2005 (Greter Sn Antonio Chmber of Commerce, 2005). Progrm Philosophy. Dvid uses the term Philosophy to cpture the vlues, beliefs, spirtions, nd ethicl priorities of the orgniztion (2010). The progrm is locted within fith-bsed, mission-centered university with well-rticulted nd ccepted core vlues. This concept is rticulted in the Progrm s vlues sttement provided in Figure 1. Self-Concept. The term Self-Concept refers to the orgniztion s distinctive competence, i.e., spects of the progrm tht crete competitive dvntge (Dvid, 2010). From its inception, the progrm ws developed to provide vlues-bsed, competency-driven, experientilly-oriented helth mngement eduction. It is this three point prctitioner-focused nd outcomes-bsed combintion tht mkes the progrm distinctive within the Sn Antonio eductionl mrket. As discussed in the next section, this distinctiveness is reinforced by fculty tht my be chrcterized s prctitioner-scholrs. Fculty. It ws envisioned from the outset tht progrm fculty would be prctitioner-scholrs. Specificlly, the primry focus of MHA fculty would be on teching, with secondry focus on reserch. Further, it ws envisioned tht the idel fculty member would hve experience s helthcre dministrtor, s well s being qulified cdemiclly. Services. It ws envisioned tht the progrm would provide working students the bility to pursue competency-bsed, MHA degree delivered in fce-to-fce, executive-like formt, i.e., prt-time, non-working hours, nd ccelerted 8 week mini-semesters. 36

This document served s the bsis for recruiting progrm development group nd dvisory bord members, s well s for the eventul cretion of mission, vision, nd vlues sttements for the progrm. Working closely with n industry chmpion, the progrm s vision philosophy sttement ws used s guide in the recruitment of Progrm Development Group (PDG) nd n Executive Advisory Bord membership. Industry Chmpion. In the cse of the UIW MHA development effort, it ws the university president who selected the individul who would led the progrm development effort. It ws lso the president who introduced this individul to locl helth industry CEO who would serve in the criticl role of industry chmpion. The locl CEO possessed four chrcteristics tht proved criticl to the success of the progrm development effort. First, the individul held strong belief tht Sn Antonio nd South Texs were in dire need of dditionl helthcre mngers in the growing helthcre industry. Further, he believed tht high-potentil cndidtes for such positions were lredy working in the industry but the lcked cdemic preprtion necessry to serve in importnt mngement positions. He lso noted tht these individuls did not hve n MHA progrm vilble to them, where they could pursue degree while still remining employed on full-time bsis. Second, the industry chmpion hd extensive helthcre ledership experience, with over 20 yers in the multi-site helthcre services field, to include serving s CEO of four different mnged cre pyer orgniztions nd s CEO of lrge Multi-Specilty Physicin Group. At the time he joined the UIW MHA progrm development effort, the individul ws the founding CEO of highly successful clinicl trils compny nd ws lso serving s Chir of the Helth nd Wellness committee for one of Sn Antonio s Chmbers of Commerce. Third, he ws very well connected within the Sn Antonio helthcre community, significntly contributing to the PDG s bility to ttrct well-respected, eductionlly oriented, helthcre executives to the progrms Executive Advisory Bord. Fourth, he held doctorte of science in biosttistics nd hd experience teching t the Ph.D. level for mjor university. 37 b

UIW MHA Vision nd Philosophy Sttement This unique combintion of chrcteristics, unified by pssion to meet the needs of the locl helthcre industry through trgeted eduction, llowed our industry chmpion to significntly influence nd contribute to the development of robust, relevnt, nd loclly supported grdute progrm in helthcre dministrtion. The most significnt contributions of our industry chmpion, who ws formlly designted s the progrm s executive in residence, were in the recruitment of executive dvisory bord members nd in the pproch to engging the bord in the identifiction nd development of progrm competencies. Progrm Development Group. Guided by the progrm vision nd philosophy sttement, Progrm Development Group (PDG) comprised of five individuls ws formed. In ddition to the executive in residence discussed bove, the PDG consisted of four more experienced helth dministrtors, three of whom lso possessed doctortes in helth services mngement or reserch. The fourth ws senior executive nd hd significnt experience teching helth dministrtion t the grdute level. As group, these individuls represented n verge of over 17 yers of diverse opertionl experience s helthcre dministrtors nd totl of 26 yers of teching experience, lmost exclusively t the grdute level. This group included sitting CEO, CIO, nd CFO nd Deputy Director for Strtegy nd Progrms for ntionl medicl eduction group. Also in the PDG ws n individul with extensive CAHME ccredittion site visit experience, s well s experience s member of the CAHME Accredittion Commission. Executive Advisory Bord. Grdute progrms in helth dministrtion hve significnt nd necessrily focused emphsis on the business of helthcre. To further emphsize the connection between helth dministrtion nd business, the progrm under study in this pper is locted within school of business. Business progrm stkeholders, to include fculty, employers, students, nd ccrediting gencies, provide criticl input towrd the development of relevnt progrm curriculum directly relted to expected progrm outcomes. Hmmond nd Moser (2009) suggest tht it is especilly importnt to include employers of business school grdutes nd representtives of locl nd regionl firms with n interest in the success of the business school on dvisory bords. Others focused on the development of helth dministrtion progrm curriculum suggest tht potentil employers bring criticl experientil insights to the curriculum development process (de los Sntos, Dominguez, & LFrnce, 2011), including the identifiction of criticl environmentl trends leding to relevnt curriculum (Irelnd & Rmsower, 1994). Finlly, there is evidence tht n dvisory bord ble to provide diverse stkeholder perspective, my enble cdemic 38

6 VOLUME 3, ISSUE 1 progrms to better lign eductionl offerings with prctitioner expecttions (Leisen, Tippins, & Lilly, 2004). Understnding the vlue of Advisory Bord enggement, the PFG sought to lign progrm competencies with industry-wide opertionl expecttions. Therefore, they formed n executive dvisory bord to obtin input from locl helthcre executives in the progrm development process. Bord membership ws comprised of diverse cross-section of senior helthcre industry executives, including CEO s nd Senior Vice Presidents from Sn Antonio metropolitn re helth system, hospitl, group prctice, insurnce, nd helth reserch settings. Argubly, the PDG nd Executive Advisory Bord members combined to form very knowledgeble, eductionlly focused, group of helthcre dministrtion professionls well positioned nd suited for the work of developing competency-bsed progrm in helthcre dministrtion. Once the development tem ws formed, the next step in the progrm development process ws to dopt competency model nd then identify mrket specific competencies for entry level helthcre dministrtors. Competency nd Curriculum Development Review of existing competency models. Severl competency models specific to the field of helthcre dministrtion hve been developed, in recent yers (Cmpbell et l., 2006; HLA, 2005; NCHL, 2004). These models vry significntly in the number of identified competencies, rnging from s few s 21 in the Ntionl Center for Helthcre Ledership (NCHL) model to s mny s 300 in the Helthcre Ledership Allince (HLA) model (HLA, 2005; NCHL, 2004). The NCHL nd HLA frmeworks re comprehensive collections of competencies required of helthcre dministrtors over the course of creer, i.e., from entry-level mngement to CEO, nd were developed by helth dministrtion scholrs nd prctitioners working in collbortion. A third competency model developed by the helth dministrtion fculty t Sint Louis University (Cmpbell et l., 2006) ws bsed upon the knowledge, skills, vlues nd ttitudes required of entry-level helthcre dministrtors. Becuse the Sint Louis University model ws focused on the knowledge, skills, vlues nd ttitudes, required of entry level dministrtors, nd becuse of the perceived comprehensiveness nd intuitive ppel of its six domins Ledership, Criticl Thinking, Mngement, Science/Anlysis, Politicl nd Community Development nd Communiction the Sint Louis University model ws selected s the guiding frmework in the UIW MHA competency development process. Still, s described in our discussion of competency development, components of ech of the three models influenced nd becme prt of the UIW MHA competency model. Mngeril Competence. Competence hs been defined s the underlying chrcteristics of n individul tht relte to effective or superior performnce in job (Golemn, Boytzis, & McKee, 2002). Competence is lso referred to s knowledge, skill, bility, or ttitude tht positions someone to effectively operte within given business environment nd is generlly ccepted s the stndrd for mesuring pproprite behvior (Chyung, Stepich & Cox, 2006; Jckson et l., 2007). As previously noted, competency-bsed curriculum hs well recognized nd sustined support in the literture (Boytzis, 1982; Boytzis, Cowen, & Kolb, 1995; Luci & Lepsinger, 1999; McClellnd, 1973). Further, competencies provide n importnt link between clssroom-bsed lerning nd outcomes produced on the job (Chyung et l., 2006; Moon, 2007). Finlly, they re required to be ligned with curriculum for progrms seeking CAHME ccredittion (CAHME 2011). Leders in the field of helth dministrtion eduction hve responded to the IOM cll for eductionl reform by developing competency-bsed curriculum built upon the demonstrble knowledge, skills, behviors, nd ttitudes believed to be criticl to mngeril effectiveness (Clhoun, Dvidson, Sinioris, Vincent, & Griffith, 2002; Cmpbell et l., 2006). Linking progrm competencies to importnt professionl criteri requires tht clssroom experience be ligned with specific competency knowledge, skills, vlues nd ttitudes. This then llows progrms to quntify nd mesure the level t which identified eductionl outcomes, i.e., competencies, hve been chieved (Dominguez, Techout, & LFrnce, 2009). UIW MHA Competency Model. As previously discussed, developing competency-bsed curriculum tht reflects contemporry, mrket specific, opertionl expecttions in terms of the knowledge, skills, vlues, nd ttitudes of entry level mngers requires input from competent eductionlly oriented helthcre executives. Using focus group methodology described in previous work (de los Sntos, Dominguez, & LFrnce, 2011), the PDG sought 39 b

7 competency input from its executive dvisory bord, using the Sint Louis University model s working frmework. Through n itertive process of informtion gthering, refinement, review nd pprovl, Advisory Bord input ws first collected using the six domin Sint Louis University (SLU) frmework during fcilitted focus group process. Input ws ctegorized within the SLU frmework nd then compred nd ligned with the competencies from the NCHL, HLA nd SLU models to ssess the comprehensiveness of the input. The input ws found to be most similr with the highly specified NCHL competency model, though severl competencies described within the HLA model were lso included in the finl itertion of the UIW model. A mtrix of the Bord s input nd ssocited NCHL/ HLA competencies nd mesures ws fed-bck, reviewed, nd modified by the Advisory Bord nd PDG to ensure relevncy nd ppropriteness of both the competencies nd their ssocited mesures. The finl itertion of the UIW MHA Model (see Tble 1) consisted of 24 competencies tht were trgeted to the needs of Sn Antonio metropolitn re entry-level helth dministrtors nd distributed cross six domin frmework similr to tht used by SLU. Curriculum Development. The first step in the curriculum development process ws to lign the 24 UIW MHA specific competencies nd ssocited mesures nd dimensions with the 19 helthcre mngement content res required by the CAHME t the time the progrm ws developed (CAHME, 2007). This ws ccomplished by the PDG nd reviewed nd endorsed by the Executive Advisory Bord. Once the Executive Advisory Bord pproved the competency-lignment mtrix, the PDG dded column to the mtrix nd mtched competencies nd required content with specific courses. An exmple of the outcome of the competency, content re, course lignment process is provided in Tble 2. (A full list of the criteri required in 2007 is vilble t OfficilCAHMECriteriFll2008ndBeyond.pdf.) This effort culminted in 15 course, 45 hour grdute degree in helth dministrtion. The finl step in the curriculum development process ws to sequence the courses. Consistent with the progrm mission, courses were to be offered in the evening, in 8-week mini-semesters cross 21 month period. Students would be enrolled in cohort ech fll nd tke one to two courses per mini-semester. The progrm ws pproved by the university in the spring of 2009, nd the inugurl clss mtriculted tht fll. At the time this pper ws written, the progrm hd grduted its second clss. A third cohort of MHA students ws one semester wy from grdution, with fourth completing its first yer of didctics. Further, the progrm hd prticipted in very successful CAHME initil ccredittion site visit in October CONCLUSION This pper describes non-prescriptive structure nd process used to develop n integrted competency-bsed grdute progrm in helthcre dministrtion. The progrm focuses on developing the knowledge, skills, vlues, nd ttitudes tht entry level mngers require in order to perform t consistently high level within South Texs helth industry orgniztions. Criticl to the development effort ws the use of mission-driven frmework grounded in clerly rticulted vision nd philosophy sttement. This frmework fcilitted the effective prtnership of progrm development group comprised of experienced prctitioner-scholrs nd diverse, eductionlly focused, executive dvisory bord, in the identifiction nd development of mrket relevnt competencies. Building upon the six domin, entry-level mngement focus of the Sint Louis University competency model, the University of the Incrnte Word grdute Progrm Development Group ws ble to crete mrket-relevnt competency model tht incorported spects of both the Ntionl Center for Helthcre Ledership nd the Helthcre Ledership Allince models. Guided from its inception by the CAHME ccredittion process, the Progrm Development Group ws ble to use CAHME requirements to lign the progrm s competencies first to required content, nd then to curriculum. The trnsformtion of helthcre is well underwy nd, long with it, the trnsformtion of helthcre mngement eduction. Mny hve resisted the powerful forces t ply in the helthcre industry, preferring to mintin the sttus quo for s long s possible. At the University of the Incrnte Word, the Mster of Helth Administrtion Progrm Development Group elected, insted, to embrce the trnsformtion, believing tht it offers pthwy to success to mking difference in the lives of the students nd the communities we serve. Results to dte re very encourging, 40
