1 Employer RESUME LEE STEVEN GLASS, MD, JD PRESENT EMPLOYMENT April, 1998 to present: Employed as Associate Medical Director for the State of Washington s Department of Labor and Industries. Duties I function as an executive-level policy advisor for cost-effective, quality health care and prevention services for the workers and employers of the State of Washington. I provide policy development guidance to the department s utilization review program, which is responsible for accurately reviewing, assessing, and making recommendations upon an average of sixty surgical requests per day. I am responsible for working with the department s utilization review vendor to develop policies regarding all aspects of the utilization review process in which the vendor is involved. I assist and advise all divisions of the Department of Labor and Industries on issues involving medical diagnosis and treatment, occupational health and safety, health care policy, crime victim s compensation policy, and related legal and legislative issues. I develop and implement policies, standards, positions, regulations, laws, projects and programs on a wide range of issues, state and nationwide, such as the prevention of long term disability. I coordinate, consult on and develop medical policy related to causality, appropriateness of medical care, impairment rating, disability prevention and other policy areas related to the delivery of quality medical care to injured and ill workers. I coordinate and consult on policy matters related to billing policy and fee schedules, provider review and profiling, utilization review, statutory and regulatory review, review of emerging technologies, and evaluation of controversial or experimental clinical practices. I consult on and direct activities related to provider education and relations. I act to insure that care provided to injured and ill workers meets the unique statutory and regulatory requirements governing the workers compensation system. I seek to resolve disputes that may arise between the Department of Labor and Industries and injured workers or their physicians regarding medical issues in workers compensation cases. I help to create evidence-based and consensus treatment guidelines. I consult on and develop policy related to occupational safety and health in the workplace, and the prevention of work-related injuries and illnesses. I assist the Medical Director, the Director of the Department of Labor and Industries and other agency executives by serving in liaison roles with the Washington State Medical Association, business, labor, the legislature, and other key external stakeholders. I represent the Department of Labor and Industries on the Washington State Committee on Homeland Security. Related Professional Activities 1. American College of Occupational and Environmental Medicine s Representative to the American Medical Association s CPT Editorial Committee Advisory Committee ( ; Reappointed to term) The CPT Editorial Committee is responsible for adopting, modifying and discarding the codes that form the basis for payment for health care services. The CPT Editorial Committee is informed in its decisions by an advisory committee. Members of the advisory committee are appointed by the AMA s Board of Trustees. I served on that committee in 2008 and In 2010 I was re-appointed to a three-ear term, ending in October, Member, APS/ACP Low Back Pain Guideline Project ( ) In May 2005, I was appointed to be a member of a joint project of the American Pain Society and the American College of Physicians. The project is creating evidence-based guidelines for the evaluation and treatment of acute and chronic low back pain. On behalf of the American College of Occupational and Environmental Medicine, I represent the field of occupational medicine in the project. My responsibilities include the review and analysis of high-grade medical literature

2 (randomized and non-randomized controlled trials, other studies of high-quality design and implementation, and meta-analyses of such studies), and the formulation of recommendations regarding the diagnoses and treatment of acute and chronic low back pain, based upon that literature. The director of the project is Roger Chou, MD, of the Evidence-based Practice Center at Oregon Health Sciences University. 3. Chair, Coding and Classification Committee, American College of Occupational & Environmental Medicine ( )) I was appointed chair of the American College of Occupational & Environmental Medicine s Coding and Classification Committee in June The committee has primary responsibility for representing the interests of the college before the AMA s CPT and RUC advisory committee. (Those two committees are responsible for creating procedural codes upon which the reimbursement of physician services is based, and for assigning relative value units to each code that is created.) As chair, I am responsible leading the committee in its various activities, and recommending to the college actions and positions it might consider that will ensure the fair reimbursement of physicians who provide treatment to injured workers. Additionally, I serve as the college s representative to the CPT Advisory Committee. 4. Immediate Past Chair, Guidelines Committee, American College of Occupational & Environmental Medicine ( ) I was appointed chair of the American College of Occupational & Environmental Medicine s Guideline Committee in April 2000, and served in that capacity until June I had primary responsibility for the process of updating and expanding the college s clinical practice guidelines. The committee I chaired consisted of up to twenty-four physicians from across the United States and Canada. The ACOEM clinical practice guidelines are a nationally recognized professional work regarding the treatment of occupationally injured individuals with musculoskeletal and other disorders. The second edition, of which I am editor, was published in December of Editor, Occupational Medicine Practice Guidelines, 2 nd Edition: Evaluation and Management of Common Health Problems and Functional Recovery in Workers. (American College of Occupational and Environmental Medicine, 2004.) A 515-page text published by OEM Press. ISBN ), this text provides physicians who are treating injured workers with evidence-based and consensus recommendations regarding methods of assessment and treatment that are most likely to return injured workers to improved health and increased function as effectively and as efficiently as possible. This treatise has been adopted legislatively by California and Hawaii, and serves as presumptively correct evidence of appropriate medical care in workers compensation cases in those states. I was principally responsible for the chapters on Pain, Suffering and Restoration of Function; Elbow Complaints; Knee Complaints; and Stress-related Conditions. As editor, I had substantial responsibility for the content of all other chapters, as well. 6. Editor/Associate Editor, ACOEM Practice Guidelines Insights ( ). I was the initial editor of APG Insights, published by the American College of Occupational and Environmental Medicine. This publication addresses various guidelines-related issues, and answers questions from readers regarding the ACOEM Practice Guidelines. I continue to be an Associate Editor of that publication. 7. Member, Council on Occupational and Environmental Medicine Practice and Managed Care (2000 Present) I serve on the American College of Occupational & Environmental Medicine s Council on Occupational and Environmental Medicine Practice and Managed Care. The council provides oversight of a number of committees of the college, and makes recommendations to the college s board of directors on matters involving clinical practice issues, managed care, and related matters. -2-

American College of Occupational and Environmental Medicine s Representative to the American Medical Association s Relative Value System Update Committee (RUC) ( ) Commencing in 1992, the federal government established a standardized physician payment schedule that is based on a resource-based relative value scale. Nationally, that system is now used not only for Medicare payments, but for reimbursement of most physician services. CPT codes are used to identify medical services that have been performed. The relative value of each such code and, by extension, each such service is established and updated by the AMA s Relative Value System Update Committee, commonly abbreviated RUC. The RUC is composed, in substantial part, of representatives of approximately twenty-four areas of medical specialty. The American College of Occupational and Environmental Medicine represents one specialty area, and has designated me its representative to the RUC. 9. I served on that committee in 2008 and Committee on Homeland Security, State of Washington Department of Emergency Management (2000 to Present) I serve on the Washington State Homeland Security Committee, and on the Intelligence/Prevention Subcommittee of that committee. My principal interest regards bioterrorism, and more specifically on matters related to both prevention of, and response to, a bioterrorism attack in the State of Washington. 11. Disaster Preparedness Task Force, Washington State Medical Association (2001) In January, 2002, the Washington I was appointed by the Washington State Medical Association to be one of the initial members of its newly-formed Disaster Preparedness Task Force. 12. Bioterrorism Preparedness and Response Program Advisory Committee (2001 to Present) In March, 2002, Washington State s Department of Health created the Bioterrorism Preparedness and Response Program Advisory Committee in response to certain federal requirements. I was designated by the Director of the Department of Labor and Industries as the department s representative in March, Member, Expert Panel, Substance Abuse and Mental Health Services Administration (SAMHSA), Prevention of Prescription Abuse in the Workplace (2012 to present) SAMHSA has created a Prevention of Prescription Abuse in the Workplace Technical Assistance Center. A group of eight subject matter experts has been empaneled by the Center to advise it on matters related to the prevention of improper substance use in, or related to, the workplace. I have been designated one of the eight subject matter experts to advise the Center. -3-

5 Glendale, CA (The parent company of Fremont Health Corporation purchased Industrial Indemnity, of which I was medical director at the time of the purchase. Following the purchase, I was asked to become medical director of Fremont Health Corporation.) Responsibilities included medical training of approximately 120 nurses, nationwide. I was a major participant in the development of guideline training materials, and the person principally responsible for training nurses and claims examiners in how to integrate the medical and legal aspects of complex workers compensation cases. I served as a consultant to nurses nationwide, and assisted them in the evaluation of complex workers compensation claims. I regularly made determinations as to whether recommended denials of payment requests should be approved, and in so doing made necessarily made determinations as to whether requested medical care was a reasonable and necessary result of a work-related injury. I credentialed health care providers who applied to be part of Fremont Health s preferred provider network. I also credentialed health care providers who sought to provide secondlevel utilization review services for Fremont Health. I interfaced between Fremont Health and physicians when disagreements arose between Fremont Health and a health care provider. I recommended policy to more senior management at Fremont Health, and testified when appropriate at legislative or administrative hearings. I had extensive responsibilities with regard to the review of billings for medical procedures. I participated in utilization review activities, both in resolving individual cases that were brought to me, and trained others in the proper means of conducting utilization review activities. I participated in the development of guidelines and protocols which formed the basis for the management of care of injured and ill workers. During 1997, I spent hundreds of hours developing evidence-based guidelines for clinical care. I regularly taught classes on the clinical guidelines published by the American College of Occupational and Environmental Medicine. On a regular basis, I was responsible for deciding issues of impairment and disability. 6 the physician demonstrates the level of quality expected of such providers by Industrial Indemnity; Interfacing with PPO and other physicians on behalf of Industrial Indemnity in a variety of circumstances, including dispute resolution; Developing Procedure Authorization Guidelines for use by the company s nurses; Participating in the development of what are now the American College of Occupational and Environmental Medicine s Occupational Medicine Practice Guidelines; Training the company s nursing and other case management staff in the use of the Occupational Medicine Practice Guidelines; Russell, Tesche & Wagg Medical-legal consultations, principally in Alaska and Washington, principally to workers' compensation insurance companies.
