4 2006 Survey of Needs: Poverty Reduction Initiative 14,000 surveys of low income indiv. in 5 counties Questions asked re: wants, needs Results: 44% needed dental care Prompted Traverse Health Clinic to act Applied for HRSA Dental Outreach Grant Awarded May year funding Committees formed, plans set in motion

5 Hire The Hygienist RDH began work in October 2006 Created Dental Access Committee: DDS s, RDH, Dental Clinics North, TBAISD, Board member, Lab representative 2 major areas: 1. School age programs 2. Adult programs 1. ORAL HEALTH for all children 2. VOLUNTEER DDS & RDH program 3. ALL persons have Dental Home 4. AFFORDABLE dental services for low income 5. SUSTAINABILITY

16 PA 161 Program People enroll to access services through Traverse Health Clinic Patients are seen at Traverse Health Clinic, or one of many other volunteer medical offices, for medical assessment and referred to Dental Access Program Hygienist assesses patient and refers to either a community volunteer DDS or Collaborative Clinic Soft tissue management is scheduled with RDH Oral surgery, endo & restorative services are donated Patient is asked to assist in payment of crowns & dentures When all treatment completed, the patient may have access to re-care with RDH All patients are asked to volunteer at a local non-profit they choose, a number of hours based on their treatment needs

20 Collaborative Clinic University of Michigan School of Dentistry Students Collaborative Clinics were held about 1 x month for 3 days 2-4 dental students were scheduled through U of M Dental assistant students were scheduled through NMC or volunteer RDA s and the 2 RDA s at Garfield are hired Day 1= oral surgery at Grand Traverse Oral Surgery Days 2 & 3= restorative at Garfield Dental Group
