Refractive Surgery Refractive eye surgery is any eye surgery used to improve the refractive state of the eye and decrease or eliminate dependency on glasses or contact lenses. The flap is lifted like a hinged door, targeted tissue is removed from the corneal stroma, again with the microkeratome, and then the flap is replaced. Radial Keratotomy (RK) 3

Developed by Refractec, the process received FDA approval for reduction of hyperopia and in March 2004 received first and FDA approval for vision technology that improves presbyopia. 4

January 2010 PresbyLASIK (Multifocal LASIK, Bifocal LASIK or LASIK for Presbyopia) Multifocal LASIK (presbylasik) is an advanced type of laser vision correction surgery that changes the shape of the eye's cornea to create different power zones for seeing at varying distances Unclear when FDA approval may occur, multifocal LASIK eventually could be a new option in the United States for correcting presbyopia 5

6 PresbyLASIK PresbyLASIK In a Canadian study reported at the 2006 American Academy of Ophthalmology conference by W. Bruce Jackson, MD, 49 farsighted and presbyopic patients were involved, and 82 eyes were treated. One year after surgery, all patients were able to see 20/25 or better in the distance and at a level of "J3" (ability to read newspaper print) or better at near. In a study published in the May 2008 issue of Journal of Refractive Surgery, 44 eyes of 22 mildly farsighted patients with presbyopia were treated with a distance-center peripheral multifocal LASIK (PML) ablation. Six months after surgery, the mean binocular (both eyes) uncorrected visual acuity (UCVA) in the distance was 20/20, and the mean binocular UCVA up close was approximately 20/25. PresbyLASIK In another 2008 study published in the Journal of Refractive Surgery, Dr. Epstein found that, about two years after having monofocal distance LASIK in the dominant eye and presbylasik in the non-dominant eye, 91 percent of 103 myopic and hyperopic patients reported being completely free of eyeglasses. The average age of patients was 53, and all were wearing bifocals or reading glasses prior to the procedure. PresbyLASIK During recent clinical trials using modified monovision with PresbyLASIK, 91.3 percent of all patients no longer needed to wear eyeglasses or contact lenses. Among nearsighted (myopic) patients, 92 percent no longer needed glasses or contact lenses. Of those who were farsighted (hyperopic), 89 percent were spectacle-free. This study involved 75 patients with myopia and 28 with hyperopia; average age was 53.3 years. Standard wavefront-guided LASIK provided monocular (single vision) distance vision in the dominant eye, and presbylasik for vision at multiple distances was used to correct the nondominant eye. Results are after 1.2 years to 3.9 years of followup. Statistics apply to use of both eyes together, or binocular vision. (Reported July 2008 in Journal of Refractive Surgery.) PresbyLASIK (Multifocal LASIK, Bifocal LASIK or LASIK for Presbyopia) Currently undergoing U.S. clinical trials, the procedure has been performed in other countries for the past several years 10,000+ IntraCor IntraCor (Technolas Perfect Vision) Outside the United States Uses a femtosecond laser to create precisely placed gas bubbles, which then reshape the interior of the cornea without penetrating the outer surface less chance of infection, inflammation or other complications? 6

One femtosecond is equal to one millionth of one billionth of a second, or one millionth of a nanosecond. (Yes, really.) The procedure Traditional Custom Types of Bladeless LASIK IntraLase FS-150 The first femtosecond laser approved for bladeless LASIK in the United States FDA approval in IntraLase, Inc. has introduced several new models of this laser with advanced features. Advanced Medical Optics (now Abbott Medical Optics) in 2007 acquired IntraLase, which was incorporated into the company's CustomVue excimer laser platform. This new, integrated system is marketed now as ilasik. Types of Bladeless LASIK Other bladeless laser systems Ziemer Femto LDV Ziemer Ophthalmic Systems received FDA approval in March 2008 for its portable femtosecond laser, known as the Ziemer Femto LDV. This femtosecond laser attaches to a movable arm and can be combined with any approved excimer laser system to create a flap for bladeless LASIK called zlasik 7

Zeiss VisuMax.
