1 JUNIOR GOLF FOUNDATION OF AMERICA, INC Non Profit 501c(3) Foundation Brooks Koepka JGFA College Scholarship INFORMATION FOR SCHOLARSHIP APPLICANTS The Brooks Koepka Scholarship was established in 2014 by a joint partnership between Brooks Koepka and the Junior Golf Foundation of America, Inc. The Brooks Koepka College Scholarship will be granted in 2014 to a boy or girl high school senior who is pursuing a college education, but will not be playing collegiate golf. One scholarship in the amount of $ will be awarded in 2014 BACKGROUND Palm Beach County s Brooks Koepka is ranked among the best young golfers in the world. In 2012, after leaving Florida State University as a three-time All American, he immediately joined the European Challenge Tour where he quickly stormed through the ranks with three historic wins earning a promotion to the European Tour. During the PGA Tour season, Brooks also rewrote the history books by earning enough Fed Ex Cup Points to receive the PGA s Special Temporary Membership on this side of the Atlantic Ocean. While Brooks meteoric rise is still in-process, his beginnings remain an important part of his story. No matter where in the world Brooks plays, we know he found his love for the game at Okeeheelee Golf Course. Brooks was a member of the Junior Golf Foundation of America where he actively participated in developmental programs, lessons, camps and tournaments. He credits the opportunities he received at Okeeheelee Golf Course with providing him with the foundational support necessary to play golf at the highest levels.

2 Brooks Koepka JGFA College Scholarship Page 2 SCHOLARSHIP CRITERIA 1. Applicants must be a U.S. citizen or legal resident 2. Applicants must be high school seniors accepted to attend a full-time course of study at an accredited College or University in the United States. 3. Applicants must have played a minimum of 50% of a High School golf team's scheduled events or has played golf "regularly" for the past three years. 4. Applicants must have a minimum unweighted GPA of 3.2 on a 4.0 scale. 5. Applicants will not be playing on a competitive collegiate golf team. 6. Applicants must have been a member of the JGFA for a minimum of 2 years prior to graduation. APPLICATION CRITERIA 1. Complete the Brooks Koepka College Scholarship Application form 2. Official High School Transcript (including completed Fall semester of senior year) 3. An autobiographical essay written by the applicant must accompany each application, including the applicant's background and career objectives, and describing how golf has been an integral part of his/her life. This letter should also include applicant's personal, academic and professional goals, chosen discipline of study, and how this scholarship will be of assistance. (Please limit to no more than two pages) 4. Three letters of recommendation for the following individuals are required and must accompany the completed form: a. High School coach or golf course junior golf instructor b. High School teacher, counselor or principal c.
