1 Cibus procedure for the "Eat&Go by Directly Charging a Personal Credit Card" service 1. The Purpose of the Procedure The purpose of the procedure is to establish rules and conditions for the "Eat&Go by Charging a Personal Credit Card" service, for the population that is entitled to benefit from this service. The responsibility for providing the service, and all that it involves, is exclusively assigned to Cibus, and the cardholder's contract is with Cibus ("Cibus"). The Eat&Go service only gives discounts on the regular price lists in the campus restaurants at Weizmann Institute of Science ("The Institute") for meals, and not for the purchase of items such as refreshments and / or entertainment. 2. Definitions: 2.1 Eat&Go service a service provided to a defined population, in the framework of which it is possible to purchase food from restaurants around the Institute campus for a discount at variable rates, (the "Service"), charged to the personal credit card of the diner. 2.2 Restaurants Restaurants on the Institute campus: Café Mada, San Martin, Chez Charlie and the Stone Snack Bar. 2.3 The "Eat&Go by Charging a Personal Credit Card" service card a card issued by Cibus for personal use only (the "Card"), which can be used to pay for meals in restaurants as defined above, while accepting an agreed discount, where the charge will be made by means of the credit card details saved in the system. 2.4 A holder of the "Eat&Go by Charging a Personal Credit Card" service card (the "Cardholder") is a person who meets the following conditions: He/she has asked to join the Eat&Go service by filling out an application form requesting that a computerized or manual card be issued (Appendix A) He/she has registered for the Eat&Go service on the Cibus website, including providing credit card information with which the service charges are to be collected.

2 3 Service Registration Process&&& 3.1 Online registration at the Cibus service provider's website It is necessary to register for the service at the Cibus website The registration will be made by the applicant, under his/her responsibility, and in accordance with the commercial terms stated on the Cibus site When entering the Cibus site, the authorized party will enter his/her ID number in the "User Name" field and also the "password" field, as a single value The "Company name" defined for this service on the Cibus website is: "29 Weizmann Institute." When signing up for the service, you must enter the amount of NIS 120 as the daily limit for card charges When signing up for the service, the card will be charged with the amount specified above, and the cardholder's personal credit card will be charged accordingly. Charging and billing will be made on an ongoing regular daily basis up, to the daily limit stated in paragraph above For assistance with registration problems, you can call a Cibus representative at Receiving the service card After the above instructions are carried out, an automatic request will be sent by the Cibus system for issuing the "Eat&Go by Charging a Personal Credit Card" service card. The diner will receive the card within some 10 days of delivery of the above request. 4. Using the card 4.1 The holder of the Eat&Go by Charging a Personal Credit Card" service card will be able to use the card to pay for his/her meals in restaurants, by passing it through a card reader located at the cashier station. 4.2 The payment with this card is limited to 120 NIS per day; this amount is after the discount. 4.3 The use of the card grants the diner a discount as described in section 4.4 below, minus a 3% fee for credit card clearance transferred by Cibus to the credit card company. 4.4 Paying with the card grants discounts as follows (net, after deduction of credit card clearing fees):

3 Chez Charlie cafeteria and the Stone snack bar San Martin restaurant Café Mada 12% off the regular price in the price list 7% off the regular price in the price list 15% off the regular price in the price list By entering the Cibus website (identification as stated in Section 3.1.3), you can at any time see the details of the transactions carried out with the card as part of this service. 4.5 In case of loss or theft of a card, the holder must immediately report to Cibus by at cards@cibus.co.il in order for the card to be canceled and a new card issued. 4.6 All the charges made with the card are the responsibility of the cardholder. 4.7 In addition to the service specified above, the card also confers the right to enjoy the special deals and discounts granted by Cibus and under its responsibility at restaurants and cafes around the country, according to the information published on the Cibus website and updates sent by . 4 Appendix A Declaration of a holder of the "Eat&Go by Charging a Personal Credit Card" service card I hereby declare that I have read this procedure and the Cibus service regulations I understand them well, and I undertake to use the card in accordance with the procedure and these regulations. I accept the terms of the procedure and the regulations and I would like to join the service according to these terms. I understand that the responsibility for providing the service, and all that it involves, is exclusively assigned to Cibus, and I hereby release the Weizmann Institute of Science from any liability of any kind, in connection with the provision of the service and/or with charging my personal credit card. 1. I understand that in order to disconnect from the service, for any reason, I must follow the procedure for "Disconnection from service" on the Cibus website, in accordance with the conditions specified on the Cibus website at: cibus.co.il. 2. It has been made clear to me that, if I choose to disconnect from the service as stated in paragraph 1 above, I can use the card with the amount charged to it within 60 calendar days from the date of "disconnection from service" on the Cibus website: cibus.co.il. 3. I acknowledge and agree that the password I will be using to enter the Cibus website is my ID number and that I cannot replace it with any other password. 4. I understand, and I authorized the Weizmann Institute to give my name and my ID number as a card user to Cibus as part of its current update requirements.
