10 NHSC Sites NHSC approved sites enable NHSC providers to fulfill their service obligations.

11 Eligible Types of Sites ü ü ü ü ü ü ü ü Federally qualified health center (FQHC) ü Includes 330 Healthcare for Homeless sites and Migrant Health Centers Federally qualified health center look-alike Certified rural health clinic Hospital-affiliated primary care out-patient clinic Indian Health Service, Tribal Clinic, and Urban Indian Health Clinic (ITU) State or Federal Correctional Facility Private Practice (Solo/Group) Other Health Facility

12 NHSC Site Requirements ü Located in a Federally-designated HPSA ü Provides primary care medical, dental, or mental and behavioral health services ü Ensures access to ancillary, inpatient, and specialty referrals ü Provides services regardless of a patient s ability to pay ü Accepts patients covered by Medicare, Medicaid, and Children s Health Insurance Program (CHIP). ü Does not discriminate in the provision of services ü Agrees not to reduce a provider s salary due to NHSC support and provides a supportive environment ü Provides sound fiscal management ü Provide services on a free or reduced fee schedule basis to individuals at or below 200% of the federal poverty level. ü View the National Health Service Corps Site Reference Guide for complete information on eligibility and applying to the NHSC. 12

13 Health Professional Shortage Areas A Health Professional Shortage Area (HPSA) is a designation, determined by the HHS Secretary, of a health workforce or provider shortage. A HPSA designation can be made for a geographic area, population group or health care facility. There are 3 types of HPSAs: Primary Care Dental Mental Health HPSA scores are used in determining priorities for assignment of NHSC clinicians. National Health Service Corps and NURSE Corps clinicians must complete their service obligation at an approved site with a HPSA. Priority funding is given to clinicians working at a site with a HPSA score of 14 or higher, which corresponds to greater need.

14 HPSA Scoring for Healthcare for Homeless Program Grantees HCH Section 330 are granted an automatic facility HPSA designation = Auto-Facility HPSA Section 330 sites DO NOT have to apply to the NHSC. They are considered approved NHSC sites and based on their HRSA grantee status. Auto-Facility HPSA sites (Section 330 HCH sites) DO have to obtain a HPSA score. 14

15 HPSA Scoring for HCH Sites Auto-Facility HPSA sites are scored by the Shortage Designation Branch in HRSA at the time of the initial grant award & based on national available data sources. However, it is important that the site check their HPSA designation (score) and request a re-score if the information 1) does not look accurate or 2) if the facility has updated or more current data. Shortage Designation scores range 1-25 for primary care and mental health and from 1-26 for dental. 15

16 HPSA Scoring Methodology Scoring Methodology for primary care Ø Population to Primary care physician ratio Ø Percent of population with incomes below 100% of poverty Ø Infant mortality rate or low birth weight (whichever score is higher) and Ø Travel time or distance to the nearest available sources of care Scoring Methodology for dental Ø Population to Dentist ratio Ø Percent of population with incomes below 100% of poverty Ø Access to Fluoridated Water < 50% of the population Ø Travel time or distance to the nearest available sources of care 16

18 HPSA Scoring Data can be obtained from following sources: Ø UDS Ø PCO Ø US Census Ø Local level data Ø County level data Ø State data Ø GIS road classification data/google maps Scoring templates are available by ing Norma Campbell at the Shortage Designation 18

19 NHSC and Use of HPSAs HPSAs determine funding priorities LRP makes awards by descending HPSA score o Applicants working in sites with HPSA scores of 14 and higher receive $20K more than other applicants ($60,000 v. $40,000) HPSAs guide placement opportunities S2S participants serve in sites with HPSAs of 14 or higher Scholars have a limited number of placement opportunities & the statute directs them to serve in areas of greatest shortage o FY 2012 placement score is a HPSA of 16 o FY 2013 placement score is going through internal clearance 19

20 HPSA Scoring and Assistance For all Auto-HPSA scoring information, re-scoring requests and technical assistance contact Norma NHSC is using HPSA data as of January 1, 2013; new HPSA scores will not be used until January Requests for rescores should be submitted no later than September 30, 2013 for the January 2014 updates. 20

Is a point of contact listed? ü Log-Into/Create a Site Portal Account https://programportal.hrsa.gov Ø Update Site Profile (http://nhsc.hrsa.gov/sites/siteprofile/) Ø Upload pictures of your site into your site profile Ø Add at least 2 points of contact Ø Add any open clinical vacancies (these will be reflected on Jobs Center) ü ü ü ü Post sign indicating services will not be denied due to a patient s inability to pay Update sliding fee schedule and sliding fee patient application Review site HPSA score(s) and contact Regional Office or PCO with updates Develop a recruitment and retention plan 21

29 NHSC Virtual Job Fairs This platform provides a cost-effective solution for promoting job vacancies by connecting sites and providers virtually using Adobe Connect technology. BCRS held three Virtual Job Fairs in 2012 which enabled 206 NHSC-approved sites from 27 states to promote more than 370 job opportunities to 575 providers from across the country. First NHSC Virtual Job Fair in March 2013 focused exclusively on Indian Health Service and Tribal sites Four additional Virtual Job Fairs are planned for 2013 Ø July 2013 and August 2013 job fairs will focus on NHSC Scholars and Scholar eligible sites 29

30 NHSC Health Care for the Homeless Sites As of June 2013, there are 56 National Health Service Corps clinicians working in Health Care for the Homeless Sites. There are 435 active NHSC Health Care for Homeless Sites Ø 54 of these have site profiles on the NHSC Job Center Ø 381 do not have profiles NHSC Job Center Important Recruitment Tool for Sites and Providers Ø More than 5,795 NHSC sites across the country are using the tool Ø Health Care for Homeless sites are under-represented 257 Healthcare for Homeless sites do not have a point of contact with a portal account. 30
