3 1. OPENING INSTRUCTIONS Carefully read the information contained in this RFQ and submit a complete response to all requirements, specifications, and questions as directed. Contact for this RFQ: Catherine Bules Project Manager OneCommunity cbules@onecommunity.org QUESTIONS AND INQUIRIES Respondents who have questions about the RFQ should such questions to the Contact for this RFQ (herein Contact) by the dates noted in the RFQ schedule in Table 1. Responses to questions which involve an interpretation or change to this RFQ will be issued in writing by addendum and ed to all parties recorded by OneCommunity as having replied to the RFQ and have elected to proceed with responding to the RFQ. It is the responsibility of the Respondents to ensure that OneCommunity has the correct contact information. Oral and other interpretations or clarifications will be without legal effect. Companies receiving this proposal are responsible for notifying the Contact that they are in receipt of a proposal package and for providing a name, address and in the event an addendum is issued. All materials submitted in response to this request become the property of OneCommunity and may become a part of any resulting contract. Award or rejection of a proposal response does not affect this right. The Respondent, by submitting a proposal, agrees that any cost incurred in responding to this RFQ, or in support of activities associated with this request, is to be borne by the Respondent and cannot be billed to OneCommunity. OneCommunity will incur no obligation or liability whatsoever to anyone by reason of issuance of this RFQ, or action by anyone relative thereto. EVALUATION PROCESS OneCommunity will review the proposals by assessing the relationship between each proposal and the section of the RFQ that the proposal addresses against the evaluation criteria below. Respondents that address the entire RFQ will be evaluated by each of the evaluation criteria and a composite evaluation score for all sections proposed. OneCommunity will review all submissions (except those rejected as non-compliant) and will compare and rank all proposals, according to the criteria set forth below. Contracting Options and Price OneCommunity is looking for the most cost effective approach to meet the network deployment goals of the HTCN project. The successful Respondent will illustrate creativity in contract approaches and pricing options. Fit to Requirements Conformance to or fit with the items detailed herein. Warranty & Service Respondents solutions will be evaluated on their clear understanding of the federal guidelines and specifications and their ability to meet or exceed the specifications. Respondent viability Respondents will also be evaluated on items such as financial viability/stability, prior history with the company, and flexibility / adaptability in meeting shipment schedule changes if applicable. 2.1 ONECOMMUNITY RIGHTS RESERVED OneCommunity reserves the right to: Request clarification from any Respondent on any or all aspects of its quotation or proposal; Cancel and/or reissue this RFQ at any time; Retain all Quotations or Proposals submitted in response to this RFQ; and, Invite some, all, or none of the Respondent for interviews and further discussion OneCommunity reserves the right to award all or a portion of this RFQ to one or more vendors, where OneCommunity determines that segmenting the order and providing multiple awards may reduce risks to schedule, help ensure resources, or mitigate costs or other risks. In any event Respondents recognize that this RFQ and the resulting Purchase Order or Orders awarded will be funded by the stimulus grant awarded to OneCommunity by EDA under the Public Works program 3. INTRODUCTION OneCommunity is issuing this Request for Quote (RFQ) for network construction services that address the open network infrastructure requirements of the EDAP HTCN project. 3.1 ONECOMMUNITY BACKGROUND Established as a nonprofit in 2003, OneCommunity is working to turn Northeast Ohio into a Smart Region one that directs its resources and investment to support technological innovation, highly efficient and customer-centric public services, and a superb overall quality of life for its citizens.

5 Technology is vital to this transformation, keyed by OneCommunity s ultra-high-speed, fiber-optic broadband network, which covers almost 2,000 miles around Northeast Ohio and provides data service for more than 2,300 public institutions such as hospitals, schools, libraries and government offices. This broadband network serves as a platform for innovation and collaboration, enabling a variety of beneficial programs and applications in health care, education, government and public safety. It is also an ideal test environment for researchers, technology companies and entrepreneurs. The network is also the infrastructure that will uniquely support the shift toward Northeast Ohio as a Smart Region. OneCommunity is eager to play a role in this change and to partner with others to ensure that we use this competitive advantage to improve our region s social and economic prosperity. 3.2 EDAP HTCN PROJECT The purpose of the EDAP HTCN project is to provide an area of Cleveland, Ohio that has high unemployment, a high poverty rate, and below average family income, with high-speed, efficient, competitive broadband infrastructure and service. In effect, the Proposed Action creates the country s first public/private partnership to deliver an ultrahigh speed, 100 gigabit network between the Global Center for Health Innovation and University Circle, through Cleveland s Health-Tech Corridor. The HTC boasts an impressive concentration of world-class healthcare institutions, major academic institutions, cutting-edge business incubators, and thriving high-tech and healthcare companies. It also houses some of the region s strongest community anchor institutions: Case Western Reserve University, Cleveland State University, Cuyahoga Community College, the Cleveland Institute of Art, the Cleveland Clinic Foundation, University Hospitals, the Louis Stokes VA Medical Center, and St. Vincent Charity Medical Center. The close proximity of companies and institutions along the HTC has created a fertile environment in which to facilitate the growth of a globally competitive center of innovation. The potential for marketing, international press, and business attraction a 100 gigabit network will provide for Cleveland and Cuyahoga County is immense. A 100 gigabit network will provide area businesses with a huge competitive advantage for potential innovations. Gigabit connectivity demands are soon to be required as companies move towards the cloud, and video streaming becomes even more prevalent. These ultra-high speed connections will be an incredible asset to the business community. This network will be a huge draw for health and technology companies, providing the potential for advanced R&D and front-line opportunities to corner emerging markets with innovative applications that would not be possible without this network. The primary purpose of the HTCN is to create jobs through providing needed infrastructure to attract businesses that would leverage the new available capacity. Because of the immense health, technology, and educational capital that Cleveland s hospitals and universities provide, Cleveland s Health Tech Corridor has taken off as the place in the region to develop business synergies and partnerships in the health and technology sector. The opportunities for business attraction to Cleveland by partnering with the area s institutions are immense and have already resulted in over 1,800 new jobs since PURPOSE OF REQUEST FOR QUOTE OneCommunity is seeking competitive bids for the following bill of materials for use in its 100 Gig Health Tech Corridor Network project. The following list is a material BOM. The items listed will be needed for the HTCN 100 gigabit network.

Also, responding to this RFQ will not enhance any vendor s chances of receiving future work from OneCommunity. Similarly; not responding to this RFQ will not be a detriment to any vendor when competing for future work.
