OF FORT BEND COUNTY, TEXAS SITTING AS A JUVENILE COURT CONDITIONS OF PROBATION In accordance with the provisions of Title III of the Texas Family Code, the Court has placed you on probation. You are hereby advised that under the laws of Texas, the Court determines the terms and conditions of your probation. In fact, if you do not follow rules set forth by this department and Juvenile Probation Officer, the court can modify and/or revoke your probation. IT IS THE ORDER OF THE COURT that beginning day of, 20, and for a period of months or until the child s eighteenth birthday, you shall strictly comply with and obey the following conditions of Probation: 1. I will not violate any laws of the United States or any City, County or State. 2. I will report to the Juvenile Probation Department at 122 Golfview, Richmond, Texas, in person once per month during the normal working hours, or as instructed by the Probation Officer for the entire term of my probation beginning day of, 20, and I will obey all rules and regulations of the probation department. 3. I will attend school, be regular in attendance, and obey school rules and regulations; or I will attend an accredited G.E.D. course until successful completion if approved by the Probation Department and the Juvenile Court. 4. I will be in my home at all times except when attending school and can only leave my home when accompanied by a parent or guardian, except for church functions or school activities such as an athletic contest and then only with the consent of parent or guardian. 5. I will reside within Fort Bend County, Texas, and will advise the Juvenile Probation Department in writing forty-eight (48) hours before any change of address.

2 6. I will not associate in any way with persons whom I have violated the law, including:. Also, I will not have any contact with the victim, or his family in this cause, either in person, in writing, by phone or any other method. 7. I will avoid injurious or vicious habits, including use or possession of narcotics or habit forming drugs, paraphernalia, inhalants, alcoholic beverages and cigarettes. I will not place nor have another place a tattoo on my person. 8. I will participate in counseling with Fort Bend Juvenile Probation Contract Services and remain in said program until properly discharged by the authorities of said program, this Court and Juvenile Probation. 9. Parent(s)/Step Parent/Guardian are to participate in parenting education, if deemed appropriate by the Juvenile Probation Department. Parent(s)/Step-Parent/Guardian is additionally responsible for providing transportation for their child to and from counseling services. 10. I will submit myself to random urine surveillance as directed to wit: a minimum of 1 TIME per month as instructed by my Probation Officer. 11. I will permit the Probation Officer to visit me at home or elsewhere upon the Probation Officer s discretion. 12. I will complete hours of Community Service Restitution, starting on the day of, 20 ; all such service shall be completed by the day of,. 13. (a). I will participate in my field supervision plan as instructed by my Field Probation Officer. (b) MUST ATTEND, PARTICIPATE, COOPERATE, AND SUCCESSFULLY COMPLETE THE TDC OUTREACH PROGRAM. MUST ATTEND A MOTIVATIONAL BOOTCAMP DAY AS RECOMMENDED BY THE JUVENILE PROBATION DEPARTMENT.
