Sr n OCI APPLICATION FM Dcuments / Required 1 Cmplete the OCI Online Applicatin Frm n the Gvernment Site ( Once the OCI Online Applicatin Frm is cmpleted and printed, the fllwing shuld be submitted alng with the applicatin package Step 1 :- One Part A AND Step 2 :- One Part B Print ut PART A and PART B. Submit BOTH parts fr the OCI Applicatin. MIN SIGNATURE/THUMB 2 Fr child f 5 years and abve, signature / name f the minr child shuld be in the signature bx under their pht n the first page f the applicatin. Fr child less than 5 years f age thumb impressin f the minr child shuld be in the signature bx under their pht using nly a BLACK r BLUE inkpad. (Use Left Thumb fr Bys & Right Thumb fr girls). PARENT CANNOT SIGN ON BEHALF OF THE CHILD UNDER ANY CIRCUMSTANCES ON THE PART A. Applicatins shuld cntain BOTH PARENTS signature n the PART B ONLY AND DULY NOTARIZED. MINS DO NOT SIGNPART B OF THE APPLICATION. Wherever parent s sign, the signature must match with the passprt signature. Applicatins will be put n hld and a new applicatin will be asked fr if parent s signature des nt match with the passprt signature. NOTE - If the signature is OUTSIDE f the bxes prvided, the applicatin will be REJECTED.

Usually applicants are able t submit a certificate f residence r place f birth f self / parents / grandparents frm First Class Magistrate / District Magistrate (DM) f the cncerned place. Ntarized cpy f educatinal certificate issued by an institutin f India and land wnership certificate by which eligibility may be reasnably ascertained A duly ntarized affidavit t that effect A cpy f an Ancestral Nativity Certificate issued by an Indian fficial r magistrate with sufficient authrity t cnfirm Indian Origin f the applicant. The decisin t whether accept this type f dcument as prf f Indian Citizenship will be made by the Embassy/Cnsulate Officer and will be FINAL in such cases IMPTANT NOTE: PARENTS/GRANDPARENTS PROOF OF IGIN 16 If the applicant des NOT HAVE an Indian Passprt and is applying fr OCI n the basis that his parents and / r grandparents were Indian Natinals, then the applicant must enclse the fllwing dcuments:- f Prf f bth parents current natinality and previus natinality Applicants birth certificate (in which yur parent's name is mentined) If the applicant is applying as a grandchild f an Indian Natinal, then the applicant must submit his birth certificate as well as his parent s birth certificate. All submitted birth certificates must shw the parent s names clearly s that it establishes the relatinship with the applicants parents and grandparents AND Cpies f first three and last tw pages f BOTH parent/grandparent's Indian passprt, f BOTH parent/grandparent's dmicile certificate issued by the cmpetent authrity, f parents / grandparents OCI cards including BOTH infrmatin

AFFIDAVIT F CHILD S NATURALIZATION BY PARENTS 18 ONLY IF MIN APPLICANT CANNOT PROVIDE NATURALISATION CERTIFICATE AS HAD ACQUIRED US CITIZENSHIP ON THE BASIS OF PARENTS: Applicant must prvide DULY Signed and Ntarized Jint Statement by Parents (Visit Frm sectin t dwnlad the frmat). NATURALIZATION CERTIFICATE 19 If minr gained naturalizatin thrugh his/her parents and pssesses n separate naturalizatin certificate, then cpies f the parents naturalizatin certificates and a Ntarized statement signed by BOTH parents is required stating the circumstances that their child received his/her naturalizatin status and why the child des nt pssess his/her wn certificate. If yu acquired US Natinality thrugh Naturalizatin, a cpy f yur Naturalizatin Certificate is required. If ne has btained mre than ne natinality after having Indian Citizenship then prvide ALL NATURALIZATION COPIES PROOF OF NATIONALITY FROM ALL COUNTRIES that yu have held citizenship. If yu d nt have an Indian passprt and is applying fr OCI n the basis that the parents and/r grandparents were Indian natinals then PARENTS AND/ GRANDPARENTS NATURALIZATION CERTIFICATE IS NEEDED The US naturalizatin Certificate shuld state FMER NATIONALITY AS INDIA. Certificates that state Cuntry f Birth as India CANNOT be accepted. IMPTANT NOTE: NAME CHANGE DOCUMENTATION 20 If yur name in any f the supprting dcuments prvided is different in any way frm the name in the US Passprt, a cpy f ONE f the fllwing dcuments must be prvided Official Curt Order fr the name change Endrsement f name change during naturalizatin prcess If in case the middle name was changed t an initial then a ntarized statement letter is required.
