Dr. Joshi has a Doctorate in Public Health and a Master of Health Services Administration from the University of Michigan and a Bachelor of Science in Mathematics from Lafayette College. Dr. Joshi is editor-in-chief for the Journal for Healthcare Quality. He also co-edited The Healthcare Quality Book: Vision, Strategy and Tools (Health Administration Press, third edition published in 2013), and authored Healthcare Transformation: A Guide for the Hospital Board Member (CRC Press and AHA Press, 2009). Keynote Session: Becoming a Highly Reliable Organization Time: 1:15 2:15 PM PT (all times listed in Pacific Time) Plenary: Gary Yates, MD President: Sentara Quality Care Network (SQCN) Prior to his current role, Dr. Yates served as the former Senior Vice President and Chief Medical Officer for Sentara Healthcare. In those roles, Dr. Yates focused on the clinical effectiveness of patient safety programs, physician integration efforts and medical management initiatives for Sentara s hospital system and health plan. Dr. Yates is a board-certified family physician and fellow of the American Academy of Family Physicians (AAFP). Dr. Yates is also a member of the clinical faculty at the Eastern Virginia Medical School. Prior to joining Sentara in 1996, Dr. Yates served as the Chairman for the Department of Family and Community Medicine and Chief Quality Officer for Maricopa Health System in Phoenix, Arizona. Dr. Yates also served as cochair of the Institute for Healthcare Improvement s (IHI) ninth annual National Forum on Quality Improvement in Health Care. Dr. Yates currently serves as Chair of the AHA/McKesson Quest for Quality Prize Selection Committee, is a member of the Board of Directors for VHA and a part of the Editorial Board for the American Journal of Medical Quality (AJMQ). 2 Session 1: The Role of Patient and Family Engagement (PFE) in a High Reliability Organization Time: 2:15 3:00 PM Moderator: Beverly Johnson President and Chief Executive Officer (CEO): Institute for Patient and Family Centered Care (IPFCC) Bev is the President and CEO of IPFCC in Bethesda, Maryland. She has provided technical assistance to over 250 hospitals, health systems, primary care practices and federal, state and provincial agencies. Bev has authored and co-authored many publications on patient and family-centered practice. She recently served as Project Director for a multi-year initiative to develop resource materials for senior leaders in hospital, ambulatory and long-term care settings on how to partner with patients, residents and families to enhance the quality, safety and experience of care. Bev serves on the Selection Committee for the AHA/ McKesson Quest for Quality and is a member of the Board of Directors for the Patient-Centered Primary Care Collaborative (PCPCC), Premier s QUEST/PACT Advisory Panel, the American College of Physicians (ACP) Advisory Board for Patient Partnership in Healthcare and the Advisory Group for the World Innovation Summit for Health. Bev was presented with the Humanitarian Award by Pediatric Nursing in 1990, and the Lloyd Bentsen Award in In 2007, she was honored with The Changemaker Award by the Board for the Center for Health Care Design and The Gravens Award for leadership in promoting optimal environments and developmental care for high-risk infants and their families. Most recently, she was a recipient of a Dorland Health 2011 People Award. Panelist: Mark Rumans, MD Chief Medical Officer (CMO): Vidant Health Dr. Rumans became CMO at Vidant Health in June Prior to joining Vidant, Dr. Rumans was an executive leader at the Billings Clinic. While under his leadership, Consumer Reports ranked the Billings Clinic number one in the nation for patient safety in The organization also achieved a 66 percent reduction in health care-acquired methicillin-resistant staphylococcus aureu (MRSA) infections. As the CMO of Vidant Health, Dr. Rumans chairs the Clinical Governance Committee, which is responsible for coordinating clinical care and policies across the system. Dr. Rumans earned his Bachelor s in Pre-Medicine from Montana State University and his Doctor of Medicine from the University of Washington (UW) School of Medicine. He completed his residency in UW-affiliated hospitals in Seattle and his fellowship in gastroenterology at Oregon Health Sciences University in Portland. Dr. Rumans also earned a Master s in Medical Management from the University of Southern California at Los Angeles. He is board certified in internal medicine and gastroenterology. Panelist: Dorothea Handron (not pictured) Patient Advisor: Vidant Health Dorothea has been working as a patient and family advisor with Vidant Health Systems since That year, she suffered complications from a routine surgical procedure which resulted in acute respiratory distress syndrome (ARDS) and several other hospital-acquired infections. Dorothea was in a medically induced coma for six weeks and spent 91 days in the hospital. Her background as professor emerita in nursing provided a false sense of understanding health care delivery. When she experienced being a critically ill patient, her perspective dramatically changed. Today, Dorothea shares her story nationally with a new vision and energy concerning the importance of PFE and patient-centered care. SPEAKERS 2 AHA SLHQ
3 Panelist: Mary Cooper, MD Vice President and Chief Quality Officer (CQO): Connecticut Hospital Association (CHA) Dr. Cooper is CQO for CHA, a position she began in December In this capacity she is responsible for overseeing the implementation of high reliability science throughout Connecticut hospitals, creating value for the hospitals with their quality and safety strategies, and working closely with state government and quality organizations. Prior to joining CHA, Dr. Cooper was Senior Vice President and CQO for Lifespan Corporation, a four-hospital health system based in Providence. She was an Assistant Professor in Medicine (Research) at The Warren Alpert Medical School of Brown University. Before joining Lifespan in 2007, Dr. Cooper spent 12 years as CQO at New York Presbyterian Hospital. She also practiced internal medicine for 10 years prior to moving into administration. Her interests encompass health policy and shaping the quality agenda on a variety of topics including quality methodologies, patient safety and operational efficiencies. Dr. Cooper s achievements and honors include grants and fellowships in outcomes research, patient safety, information technology, case management and cardiac medicine totaling over eight million dollars; publication of book chapters, abstracts and journal articles including Circulation, Pediatric Pulmonology, Journal of the American Medical Association, Academic Medicine and Association of Perioperative Registered Nurses (AORN) Journal, among others; presentations at numerous conferences and symposia; and membership on the Rhode Island State Board of Medical Licensure and Discipline and Federation of State Medical Boards (FSMB) sub-committees. Dr. Cooper received her undergraduate degree from Duke University, her medical degree from Temple University School of Medicine, where she was elected to American Optometric Association (AOA), and her degree in Health Law from Pace University School of Law. 4 Panelist: Tammy Dye, MSN, RNC Vice President, Clinical Services and CQO: Schneck Medical Center Tammy is Vice President of Clinical Services and CQO at Schneck Medical Center in Seymour, Indiana. In that role, she is responsible for building and supporting a patient first culture that maintains clinical experience, develops an infrastructure for managing clinical quality and cost improvement, and implements a measurement system to assure patient and operation outcomes are consistently measured against national norms. Tammy joined Schneck in 1998 as the Director of Women and Children s Services and has been in her current position since She started her nursing career as a Certified Nursing Assistant over 25 years ago and currently holds a Bachelor of Science in Nursing from Southwest Baptist University and a Master of Science in Nursing Administration from Indiana University. Tammy is a National Baldrige Examiner and is the Executive Leader and Process Owner for the Malcolm Baldrige National Quality Award at Schneck Medical Center. Panelist: Fred Goldberg, MD, MS HCM Vice President, Medical Affairs and CMO: Nathan Littauer Hospital Dr. Goldberg, a board certified pediatrician, enjoyed an 18-year career in pediatric clinical practice in Syracuse, New York. He received numerous honors and awards, including those recognizing his contributions to the medical education of students and residents affiliated with pediatric and family practice training programs at Upstate Medical University and at St. Joseph s Hospital in Syracuse. Dr. Goldberg has held leadership positions with two health plans in upstate New York, and served as CMO at Faxton - St. Luke s Healthcare in Utica, NY ( ) and Community General Hospital in Syracuse, NY ( ). He has been in his current role at Nathan Littauer since Dr. Goldberg has been a leader in promoting quality, patient safety, superior patient experiences and cost-effective care. His skills in change management and leadership development have led to significant improvements in clinical outcomes at the hospitals where he has worked. Dr. Goldberg founded and organized annual healthcare symposia in Utica and Syracuse, New York ( ) featuring the country s foremost experts in health care quality and safety. Dr. Goldberg is a graduate of Franklin and Marshall College and received his medical degree from the University of Pennsylvania School of Medicine. He holds a Master s Degree in Healthcare Management from the Harvard University School of Public Health and is a member of the American College of Physician Executives (ACPE), the National Association for Healthcare Quality (NAHQ), the American College of Healthcare Executives (ACHE) and the Healthcare Association of New York State Quality Steering Committee. Panelist: Beth Daley Ullem Board of Directors: ThedaCare Beth is a nationally-recognized governance expert for patient safety and quality. Beth works with hospital leadership teams, boards and health care industry leaders to develop and fund programs that improve the quality of patient care. Beth also assists these leaders to use best practices to provide oversight of quality and safety and make patient-centric outcomes transparent and accessible to consumers. Beth serves on the system-wide Board and the Quality Committee of ThedaCare in Appleton, Wisconsin, and formerly served on the systemwide Board and on the Quality committee of the Children s Hospital of Wisconsin. Beth is on the Board of Governors for NPSF and also on the Board of the ThedaCare Center for Health Care Value. Beth has worked extensively with the network of 81 pediatric hospitals called Solutions for Patient Safety on their board education efforts. Beth has been a guest speaker at Harvard Business School, Georgetown University s Kennedy School of Ethics, Kaiser Permanente, the American Medical Association (AMA) and the Centers for Medicare & Medicaid Services (CMS). Beth is passionate about reducing harm and errors, improving the transparency of health care outcomes and better enabling hospital boards to understand and lead on quality issues. Beth is a former McKinsey consultant, DePaul University Professor of Strategy and Management, an MBA graduate from Northwestern and a School of Foreign Service graduate from Georgetown University. Sunday, July Welcome and Recap Time: 8:00 8:15 AM Speaker: Charisse Coulombe, MS, MBA Vice President, Clinical Quality: HRET Charisse is Vice President of Clinical Quality for HRET. Her primary responsibilities include the oversight of all areas of the AHA/HRET Hospital Engagement Network (HEN) project, the Comprehensive Patient Safety Leadership Fellowship (CPSLF) in collaboration with NPSF and the Symposium for Leaders in Healthcare Quality (SLHQ). Before HRET, Charisse spent 16 years at Resurrection Health Care (RHC). Her last position with RCH was System Director of Informatics where she focused on covering acute and non-acute entities. Charisse oversaw the system-wide quality scorecard development, CMS core measure data submission and managed several large measurement databases. Reporting to the CMO, she played an active role in the integration of the quality data and measurement systems when RHC merged with Provena Health. Charisse has a Master s in Business Administration from University of Phoenix, a Masters in Zoology from Western Illinois University and a Bachelor s of Science in Biology from Northeastern Illinois University. She is a Six Sigma Black Belt and a certified professional in health care quality. Session 3: Hospital and Patient and Family Advisory Councils (PFAC) PFE Best Practices Time: 8:15 9:15 AM Moderator: Sue Collier, MSN Performance Improvement Specialist: North Carolina Quality Center (NCQC) Sue joined NCQC in 2013 as a Performance Improvement Specialist in PFE. Prior to NCQC, Sue was employed by Vidant Health for over 32 years. She began her career with Vidant Health as a staff nurse, head nurse and clinical specialist in trauma and surgical clinical services. Sue led an innovative hospital-wide initiative designed to improve patient experiences across inpatient and outpatient settings in the early 1990 s. She also served as a corporate leader in the system s planning, marketing and strategic development division for over 14 years. Sue developed the corporation s first office of patient and family experience and led the system s nationally recognized portfolio of change initiatives designed to create meaningful partnerships with patients and families. Her vision is to create and sustain patient-centric models of care delivery to improve the quality of life for people in North Carolina. 6 Panelist: Evan Benjamin, MD, FACP Senior Vice President, Quality and Population Health and CQO: Baystate Health Dr. Benjamin, a board-certified internist, is Professor of Medicine at Tufts University School of Medicine and Sackler School of Graduate Biomedical Sciences, as well as Senior Vice President, Quality and Population Health & CQO of Baystate Health in Springfield, Massachusetts. Dr. Benjamin founded the Division of Healthcare Quality at Baystate in 2000 and helped to create the Center for Quality of Care Research in His work has helped to transform health care at the three hospitals within Baystate s system, garnering a number of national awards acting as a model for other health care organizations. Nationally recognized for his work in outcomes management and quality-of-care improvement, Dr. Benjamin s publications have appeared in over 40 articles in major journals. His research in measuring quality, patient safety and health policy has been published in the New England Journal of Medicine and the Journal of the American Medical Association (JAMA). He has contributed to five books including Healthcare Quality: The Clinician s Primer (ACPE 2012). Dr. Benjamin is an active faculty member for IHI and sits on the Board of Trustees for Catholic Health Partners, a 26-hospital system in Ohio. Dr. Benjamin received his Bachelor s in Chemistry from Williams College, his Medical degree from Case Western Reserve University School of Medicine in Cleveland, completed post-graduate education in Internal Medicine at Yale-New Haven Hospital and the Yale University School of Medicine, and has training in epidemiology with the US Public Health Service. Panelist: Linda McShane (not pictured) Patient Family Advisor (PFA): Baystate Health Linda is the Chair of the PFAC at Baystate Health, and focuses her work on educating the health care community on how PFACs are a valuable resource in supporting the patient and family experience. David Mayer, MD Vice President, Quality and Safety: MedStar Health As the Vice President of Quality and Safety for MedStar Health, Dr. Mayer is responsible for overseeing the infrastructure for clinical quality and its operational efficiency for MedStar and each of its entities. Dr. Mayer also designs and directs system-wide activity for patient safety and risk reduction programs. Over the course of 10 years, he held numerous roles at the University of Illinois Medical (UIC) Center, including acting as the Co-Executive Director of UIC Institute for Patient Safety Excellence, Director of UIC Masters of Science Patient Safety Leadership Program, Associate Dean for Education and Associate CMO for Quality and Safety Graduate Medical Education. Additionally, Dr. Mayer has significant experience in the private sector as Director of Medical Affairs of the Hospital Products Division of Abbott Laboratories. He regularly presents and writes on topics related to quality and patient safety and recently was presented with the 2013 Founders Award from the American College of Medical Quality (ACMQ) for his outstanding contributions to quality management and improvement. Dr. Mayer attended UIC Chicago for both his undergraduate and medical degrees. Session 4: Accelerating Improvement to Eliminate Patient Harm Time: 7:15 8:15 AM Tanya Lord, MPH, PhD Patient and Family Engagement (PFE) Consultant: New Hampshire Foundation for Healthy Communities and AHA SLHQ Tanya was a special education teacher when her son, Noah, died as a result of medical error. After his death, Tanya read The Institute of Medicine s (IOMs) report To Err Is Human and realized that the errors responsible for her son s death were not unique. This created a desire to work towards improving health care quality. While completing a Doctorate in Clinical and Population Health Research, Tanya took note of the lack of patient and family involvement in quality improvement and patient safety initiatives. Having the patient voice represented at all levels of the health care system became a new passion. Currently, Tanya coaches 25 New Hampshire AHA/ HRET HEN hospitals to develop and enhance PFE programs. She is also an active consultant on multiple state and national improvement projects, including the Quality & Patient Safety Roadmap and Health Forum and AHA Leadership Summit. Speaker: Knitasha Washington, PhD, DHA, FACHE Advisor, PfP Campaign; Executive Director: CAPS Knitasha is a professionally trained master change agent in health care and business administration. Shaped by her passion and belief in social justice, Knitasha has earned recognition nationally as a health care leader, policy contributor and patient activist. With more than 15 years of experience as a health care consultant, Knitasha primarily focuses in the areas of systems performance improvement, patient safety, PFE, quality and health equity. After committing several years advocating for the elimination of health disparities, Knitasha s perspective on the U.S. health care system changed dramatically when losing her father to a medical error in Today, Knitasha serves in multiple roles driving change and innovation nationally. As an advisor to the PfP Campaign, she has been commended for her work to improve equality of health outcomes. Knitasha is credited for being the thought-leader that forged a pathway to consider health disparities among vulnerable populations as a strategic imperative in the nations fight to achieve improvement in quality and patient safety. Accordingly, she continues to advocate through the National Association of Health Services Executives (NAHSE), Chicago Midwest Chapter and was recently appointed the Executive Director to CAPS. Knitasha is a fellow of the ACHE and received her Doctorate of Health Administration degree from Central Michigan University, Masters of Heath Administration from Governors State University, and Bachelor of Arts with a focus in Business Administration from Chicago State University. Nominations and awards received include the 2014 Governors State University Luminary Pioneer, 2014 Illinois Department of Public Health Champions Award, 2013 ACHE Regents Health Care Leadership Award, and 2009 NAHSE National Young Healthcare Executive Award.
