3 l 1.1 ABOUT LAMBTON COLLEGE SECTION 1 INTRODUCTION The Lambton College of Applied Arts and Technology located in Sarnia, Ontario. Established in 1966 by the government of Ontario, Lambton College is accredited by the Ministry of Training, Colleges and Universities (MTCU). Lambton offers postsecondary programming in Technology, Business and Liberal Studies, and Health and Community Services as well as continuing, contract education and apprenticeship as well as numerous other services which provide significant benefit to the local community and the province of Ontario. Lambton College has a proud history of serving the Sarnia- Lambton region its current full- time student population is 3800 with 1050 part- time students, approximately 800 international students and over 700 full and part- time employees. l 1.2 GENERAL a)this Request for Proposals (RFP) is issued by Lambton College of Applied Arts and Technology (Lambton) in respect of the goods/services that are detailed in this proposal b)the purpose of this proposal is to select one or more qualified respondents with whom Lambton intends to enter into negotiations in an effort to reach an agreement to provide the services. c)in this RFP, parties that submit documents in response to this RFP are referred to as Respondents and their submissions are referred to as Proposals. The entity or entities that are selected by Lambton to enter into the final agreement (the Final Agreement) is referred to as the Successful Respondents. d)the procurement to select a successful respondent(s) shall commence with the issuance of this RFP and shall terminate either, l 1.3 COMMUNICATION I.on the cancellation or discontinuation of the RFP Process by Lambton, if such cancellation or discontinuation occurs; or II.on the execution of a Final Agreement by the Successful Respondent(s) and Lambton. SECTION 3 DEFINTIONS Addendum means a written addendum issued under this RFP Business Day means any day other than a Saturday, Sunday, public holiday or other day on which banks in Ontario are authorized or required to by law to be closed College System refers to the 24 Ontario Publicly funded colleges Evaluation Team means a team consisting of a Dean(s), Part- Time Studies and Continuing Education Manager and Project Manager, and the Associate Dean, Learning and Innovation Centre who will perform the evaluation of each of the proposals and make recommendations to award the proposal OntarioLearn is a consortium of the College System to offer online courses throughout Ontario Ontario Online is a consortium of Ontario Colleges and Universities offering course online throughout Ontario Final Agreement refers to the contract arising upon the acceptance of a proposal by the College made in response to this RFP Subject Matter Expert (SME) refers to the content expert of the course and will direct the instructional designer in course content requirements PAGE 4

14 l 5.6 QUALIFICATIONS l Instructional Designer Provide an outline of the general history of the firm and overall type of work completed by the firm. Provide curriculum vitae for the lead individual. In addition please include the curriculum vitae for any offer lead individuals that you may assign to this project. In addition please supply print of electronic (via URL link or USB thumb drive) samples of pervious work from similar projects (including references) within educational environments. l Subject Matter Expert (SME) The College will supply Subject Matter Expert (SME) for each of the courses, however if the firm has an SME that they would like to collaborate with for the purpose of this proposal, they are to provide their curriculum vitae. Unless the tender explicitly states that the services of an SME is also included in the cost, it will be assumed that the services of the SME will be over and above the tendered amount. l 5.7 DELIVERABLES Vendors to Lambton College must provide the following: A full course, loaded into a D2L shell (to be provided). All assessments, exams and tests in the course must be supplied in D2L format and loaded in D2L. Two different sets of every test, exam or assessment must be provided, using at least 50% different questions. The secondary versions of the assessments will be loaded as draft. All learning materials should be provided in Microsoft Word or compatible format. In the case where PDF files are supplied, the original, editable source documents must be provided. All graphics and illustrations must be identified as to source, and permission- to- use from the copyright holder must be provided. All simulations or programmed learning objects must be supplied with source materials for the creating program. Lambton to approve use of each creating program. No materials shall be created with proprietary software without the permission of Lambton. In all cases copies of the creating software must be supplied to Lambton College with a license to use same to modify the materials, as needed. Any edited videos must be supplied in MP3 or QuickTime formats on DVD, even if they are delivered to the students through YouTube or similar platforms. If videos are to be delivered via YouTube, then the contractor is responsible for loading these. Any work posted to Cloud platforms (including YouTube) must be done through an account in which the password is supplied to Lambton College, with the understanding that this password will be changed after the project is completed. 15 An instructor s manual will be provided listing the instructional steps that an instructor must undertake at the start and throughout the course. (Calendar entries, checking links, updating About the Professor, etc.) If the course is not 14 weekly units, then the manual will recommend how the units will be divided into weeks. All materials should be provided in AODA compatible format. All images used in illustrations must be described textually in the HTML alt tag. Videos should have transcriptions. PAGE 15
16 l 6.1 SUBMISSIONS SECTION 6 INSTRUCTIONS TO RESPONDERS All Proponents must submit one (1) original and two (2) copies of their submission. The copies must be clearly marked copy. It must be in a sealed envelope with the company name, contact name and means of contact, (such as address), and reference Instructional Design Services for Online Course Development. To be considered in the RFP process, a Proponent s proposal must be received on or prior to the Proposal Submission deadline, as set out in Section 3 of this RFP,, in a sealed package and should bear the Proponent s name, contact person, return address, RFP number, and the RFP Coordinator s name, in this case, Kelly Hirlehey Procurement Specialist. Proposals received after the Proposal Submission Deadline shall not be considered and shall be returned to the Proponent unopened. Regardless of the method of delivery chosen by the Proponent (such as courier, delivery service, Canada Post,) each Proponent is responsible for the actual delivery of its Proposal to the address set out below. Proposals transmitted by facsimile or sent by any other electronic means shall not be considered. Notwithstanding anything to the contrary contained in any applicable statute relating to electronic document transactions, including the Electronic commerce Act, 2000, S.O. c.17, any notice, submission, statement, or other instrument provided in respect of the RFP may not be validly delivered by way of electronic communication, unless otherwise provided in this RFP. The Proponent should identify and mark any trade secret or proprietary intellectual property in its Proposal. Proposals should be completed without delineations, alterations, or erasures. In the event of a discrepancy between the original paper copy of a Proposal and any other copies, the original shall prevail. l 6.2 ADDENDUM Lambton College will issue any changes/additions/deletions to Scope of Services and/or terms and conditions in the form of numbered addendums. Any and all addenda issued prior to the closing date shall form part of the document. PAGE 16

Upon award, the successful proponent(s) will be issued a final agreement confirming the fee for services and the contractual obligations as outlined in this RFP. l 7.2 CLARIFICATION AND ADDENDA It is the responsibility of the Respondent to seek clarification on any matter they consider unclear before submitting a proposal. If the College, for any reason, determines that it is necessary to provide additional information relating to this RFP, such information will be communicated to all Proponents by way on MERX, or by the same manner in which the Proponent received the RFP. All addenda issued shall become part of the Contract(s) and will be considered in determining the bid price. Any amendment or supplement to this RFP made in any other manner will not be binding on the College. PAGE 18
19 Such addenda may contain important information including significant changes to this RFP. Proponents are responsible for obtaining all addenda issued by the College. In the space provided in the Form of Offer, Proponents shall confirm its receipt of all addenda by setting out the number of addenda in the space provided in the Form of Offer. In the event that a Proponent chooses to cancel the receipt of addenda or amendments, its Proposal may be rejected. l 8.1 INTELLECTUAL PROPERTY SECTION 8 GENERAL CONDITIONS Course outlines and other documentation supplied by Lambton to the Respondents are provided for the intent of this work only. Intellectual property created, for the purpose of this project shall be the sole and exclusive property of Lambton College. Upon completion of the project, Lambton College will retain the source files and the vendor retains no rights to future use the College s intellectual property. l 8.2 PROJECT ADMINSTRATION Administration of this project will be managed by one primary contact by the vendor. l 8.3 CONFIDENTIAL INFORMATION The vendor agrees not to directly or indirectly disclose or use any material or information belonging to Lambton College without first obtaining written consent of Lambton College. The vendor also agrees to return such materials or information to Lambton College upon request without retaining any copies. l 8.4 WARRANTY The vendor warrants that the services will be performed in a professional and competent manner, in accordance the applicable standards of the trade. The vendor shall review and correct any portion of the services or product which is identified by Lambton College during the 120 day period following the completion of the projects as failing to meet said standards. l 8.5 OWNERSHIP OF MATERIAL The successful respondent agrees that all materials developed in the production of these courses will become the sole property of Lambton College including but not limited to all written and electronic materials, source code, all rights to use the name and trade- mark in connection with these courses and all other trademarks, trade mark registrations, trade names and registered user and other trade mark rights, copyright, copyright registration and applications. l 8.6 AUTHORITY The Successful Respondent(s) will ensure that all materials used in conjunction with eh development of the course(s) are transferable to Lambton College and proper authority for use and transfer is obtained. 20 SECTION 9 OPTION PRICING The Responder will submit a proposal for exclusive and/or individual and/or bulk course pricing for instructional design services for all 12 courses listed on page 5. Lambton College will provide the subject matter expertise/content expert to the instructional design service. Lambton College reserves the right to award all courses or individual courses to one or more proponents. In addition, the Responder can also submit a proposal for exclusive and/or individual and/or bulk course pricing for instructional design services plus providing subject matter expertise, for the following courses: Math 1 for PreHealth Sciences Math 1 for PreHealth Sciences Environmental Management Systems and Audits Restoration Ecology Fundamentals in Patient Safety Patient Safety Best Practices Patient and Family Centred Care Cultural Competence for Health Care Providers l 9.1 For Instructional Design Services The Responder can submit pricing for exclusive and/or individual and/or bulk course pricing for instructional design services. l a) Exclusive Course Price Provide the price to design all 12 of the listed courses in Section 4.1 of this RFP. l b) Individual Course Price Provide the price for each course.
