The policy endows at 95 with coverage to age 95. ELOP is available to all eligible employees ages who are actively at work. You decide what is right for you and your family and the premiums would be conveniently payroll deducted from your paycheck. You own the policy so you can keep it at the same rates, even if you leave the company. 2 voluntary Benefits 41 Supplemental Disability Insurance Full-Time Employees budgeted to work 40 hours or more per pay period can supplement their employer-provided short term disability by purchasing an additional guaranteed issue* short term disability benefit that offers a tax free benefit of 30% of salary up to $4,000 per month. Benefits provided for both partial and total disability. The plan can be set up in a variety of plan designs with benefit durations of 6 or 12 months. Elimination periods, the length of time you must be out of work before benefits are payable to you, may be 14 or 30 days. You cannot receive any more than 100% of your salary in disability benefits between your FMH plan and this voluntary plan. Benefits are paid to the employee monthly. You can continue this policy should you no longer be employed at FMH. Coverage is guaranteed renewable until age 70. 3 42 frederick memorial hospital Critical Illness Insurance Critical illness insurance is available for employees who are actively at work and working a minimum of 40 hours per pay period. Critical illness insurance pays a lump sum benefit if you are diagnosed (while this coverage is in force) with one of the covered Specified Critical Illnesses (cancer, myocardial infarction, stroke, major organ transplant, end stage renal disease, Alzheimer s disease, coma, paralysis, severe burns, ALS (Lou Gehrig s disease), loss of sight, speech or hearing and benign brain tumor). A partial benefit (30%) is payable for carcinoma in situ, coronary artery bypass and angioplasty and stent insertion. A one-time $300 benefit is payable for skin cancer. A $50 annual health screening benefit is included if you have any one or more of the defined screening tests. Children are automatically included in this coverage at 25% of the employee amount at no additional premium. You may also apply for coverage for your spouse (age 69 or under) at 50% of your amount. Once you are continuously covered for at least one month, you can continue this coverage should you no longer be employed at FMH. 4 Accidental Death and Dismemberment Insurance In addition to accident insurance for healthcare expenses, protection in the case of a loss of life is also important. That is why FMH offers a voluntary accidental death policy to employees and their families. This policy supplements the basic AD&D policy that FMH offers to you free of charge. Employees can choose amounts in increments of $25,000 up to $500,000. Benefit amounts over $250,000 are limited to 10 times Annual Compensation. Your family Your spouse s benefit amount will be 40% of yours, or 50% if you have no dependent children. Each of your covered childrens benefit amount will be 10% of yours, or 15% if you have no eligible spouse. Your spouse s benefit cannot exceed $250,000. The benefit amount per child cannot exceed $25,000.
