September 17, 2012 Continuing Education Committee Meeting minutes Ms. King stated that the minutes are being revised and will be presented for ratification during the October 5, 2012 General Business meeting. APPLICANTS FROM APA ACCREDITED PROGRAMS FOR EXAMINATION AND LICENSURE AS NOTED 21 Tab 3. List of Applicants Dr. Rafael Rivas-Vazquez moved to approve the addendum list of applicants distributed to the Board on September 19, Dr. Neda Koehnemann seconded the motion, which carried 4/0. Dr. Orta was recused from the vote due to technical issues with downloading the addendum list. PETITIONS FOR WAIVER OR VARIANCE 26 Tab 4. Hibbert, Tasha re: 64B , F.A.C., Examination Dr. Hibbert was not present on the call. On May 19, 2012, Dr. Hibbert s application was scheduled to expire pursuant to Section (3)(a), F.S., which requires all licensure requirements to be complete within 24 months of September 21, 2012 Board Quorum Meeting Minutes Page 1

2 board approval. During the June 2012 meeting, Dr. Hibbert petitioned for, and was granted, an extension to May 19, Dr. Hibbert completed the required supervised experience and passed the laws and rules examination on November 3, 2010; however, pursuant to Rule 64B , F.A.C., the score expired on May 18, Rule 64B (4)(b), F.A.C., Examination, provides as follows: A passing score on the Florida laws and rules examination shall cease to be valid eighteen (18) months after the Board s letter to the applicant advising that the applicant has passed the Florida laws and rules examination. Dr. Hibbert filed a petition for waiver of Rule 64B , F.A.C., Examination, to be allowed to proceed with the licensure process without having to re-take the laws and rules examination. Following discussion, Dr. Luis Orta moved to deny the request for waiver of the rule. Dr. Neda Koehnemann seconded the motion, which carried 5/0. RECONSIDERATION OF LICENSURE APPLICATION 14 Tab 5. Fernandez, Mary Jo Dr. Fernandez was present on the conference call. Dr. Fernandez applied for licensure on February 20, Her application was approved by the Board during the April 2012 meeting. Subsequently, Dr. Fernandez became aware that a case which she thought was dismissed was still pending. She contacted the Board office to inform staff that she was the subject of a complaint pending before the Massachusetts Board of Psychology. During the June 2012 quorum call, the Board required a personal appearance from Dr. Fernandez following resolution of the case in Massachusetts. Following discussion, Dr. Rafael Rivas-Vazquez moved to allow the applicant to withdraw her application. Dr. Dean Aufderheide seconded the motion, which carried 5/0. Dr. Fernandez decided to withdraw her application. ADJOURNMENT Dr. Dean Aufderheide moved to adjourn the meeting. Dr. Neda Koehnemann seconded the motion, which carried 5/0.
