TIRADO, D.C. : CONSENT ORDER License No. 38M TO PRACTICE CHIROPRACTIC IN THE STATE OF NEW JERSEY This matter was opened to the New Jersey State Board of Chiropractic Examiners upon receipt of information which the Board has reviewed, and the parties wishing to resolve this matter without further formal proceedings, hereby stipulate to the following findings of facts and conclusion of law: FINDINGS OF FACT 1. Respondenț Jason C. Tirado, D.C., is a chiropractic physician in the State of New Jersey and has been a licensee at all times relevant hereto.

On March 26, 2012, Respondent pleaded guilty in the United States District Court, District of New Jersey, and was convicted of the crime of Bank Fraud in violation of 18 U.S.C (Copy of the "Judgment In A Criminal Case" attached hereto and incorporated herein as "Exhibit A"). 3. Respondent admitted to the Court that in mid to late 2003 he conspired with another to obtain a student loan from Key Bank in the amount of approximately $100, by falsely representing that he would use the proceeds of the loan to attend a flight school in Florida, when in fact he intended to invest the money with his co-conspirator instead. He then misrepresented to the bank that he had enrolled in the flight school, but the tuition money was not returned to the bank by the flight school when he decided not to attend. 4. On March 26, 2012, Respondent was sentenced to serve one (1) day in custody followed by two (2) years of supervised release and pay $99, in restitution. CONCLUSION OF LAW Respondent's conviction provides grounds for the suspension of his license to practice chiropractic in New Jersey, pursuant ton.j.s.a 45:1-21(f), in that Respondent has been convicted of a crime involving moral turpitude or relating adversely to the practice of chiropractic. 2

3 ACCORDINGLY, IT IS on this day of 'arr a 201,'3 ORDERED AND AGREED that: 1. Respondent's license to practice chiropractic in the State of New Jersey shall be suspended for a period of two (2) years; except that the initial three hundred sixty-four(364) day period shall be an active suspension, commencing thirty (30) days after the date on which this Order is issued ; and the remaining one (1) year and one (1) day period of suspension shall be stayed, and become a period of probation. The stayed period of suspension shall be activated upon a showing of Respondent's -compliance non with any of the terms and conditions set forth herein. Respondent shall be required to appear before the Board(or a committee thereof ) prior to reinstatement of his license on probation to demonstrate his fitness to be released from his active suspension. 2. During the period of active suspension, Respondent shall fully attend, successfully complete and unconditionally pass the ProBE (Professional Problem Based Ethics) course offered by: The Ethics Group, 89 Summit Avenue, Suite 185, Summit, New Jersey 07901; the PRIME (Professional Renewal in Medicine through Ethics) course offered by the Center for Continuing Education in the Health Professions at UMDNJ-Robert Wood Johnson Medical School, 97 Paterson Street, Room 124, New Brunswick, New Jersey ; or an equivalent ethics course pre-approvedby the Board, and shall provide documentation thereof to the Board. 3
