Captain A.G. Lamplugh spoke these words as part of an aviation safety statistics presentation made to the Royal Aeronautical Society in The quote, reproduced countless times on posters and plaques, is famous but few realize Captain Lamplugh s direct connection to the aviation insurance industry. He is the person who began the company that would evolve over the course Now, in the 21st century, through our Vista Elite and SM4 programs, we are expanding our support for improving safety for all operators and service providers. It is tempting to say that he would find that aviation has advanced way beyond what he could have envisioned in 1931, but that, sadly, our war on human error has not advanced nearly as far. Therefore, our work continues. This report provides a snapshot of what we are accomplishing with our newest aviation safety initiatives. Human factors, emergency response, in-flight medical assistance, adherence to standards and modern safety management are combined in our groundbreaking SM4 program.
