2 Submitting Health Applicatins All Individuals Submitting Health Applicatins: All cnsumers can nw use the MAhealthcnnectr.rg website t enter an applicatin fr cverage n their wn, r with the help f a Navigatr r Certified Applicatin Cunselr (CAC). Certified Applicatin Cunselrs can guide applicants under 65 thrugh the MAhealthcnnectr.rg Individuals and Families prtal Apply using a paper applicatin: ACA-2 fr under 65, SMBR fr age 65 and ver. Individuals can call Custmer Service and cmplete an applicatin by phne. 2

3 Submitting Health Applicatins Online Online Applicatins: Applicants can submit thrugh MAhealthcnnectr.rg website Health Assistance cmpnent f Virtual Gateway Cmmn Intake was remved n 12/30/13 (SNAP, WIC, SNP, and ther prgrams remain in Cmmn Intake) Until the Navigatr/CAC prtal is available: CACs can guide patients/clients thrugh the nline Ind./Families Prtal Until trained/certified, remaining Virtual Gateway users can: Direct them t cmplete nline applicatin at MAhealthcnnectr.rg: Can cmplete at hme, in library, r n a PC at yur facility, if set up fr this Prvide them with paper applicatin (ACA-2) t cmplete and submit Refer t Custmer Service t cmplete phne applicatin Refer t a CAC rganizatin, r a certified ACA Navigatr rganizatin. Lists f these rganizatins are available n the MAhealthcnnectr.rg website. Lists cntinue t be updated as rganizatins becme certified. 3

4 Submitting Health Applicatins Paper / Phne Paper applicatins: ACA-2 Fr age under 65 Use the new ACA-2 applicatin the MBR is nw bslete. If the ld MBR is received after January 1st, will require fllw-up fr MAGI and ther infrmatin in rder t determine eligibility. As sn as the individual receives their training invitatin, he/she can lg int the LMS and take their training it s all thrugh nline curses Hw lng it will take depends n each individual s experience with eligibility, their learning style/pace, whether learner wants t print sme f the cntent r g back ver smething, etc. As sn as the individual cmpletes their last curse, a link t print their certificate displays
