1 Case Management Options for Older Adults in Larimer County June 2012 Case managers can provide a broad range of services and not all case managers provide the same services. A case manager may be a social worker, nurse, gerontologist, counselor, or other professional. Services can include: Understanding the long term care system; consumer education Accessing public or private programs (Medicare, Medicaid and financial assistance programs) Referrals to local programs, resources and services Coordinating and managing care Assessments and recommendations Crisis intervention Counseling and support Money management and bill paying Transportation for shopping and appointments Assistance and/or education on housing options Power of Attorney and guardianships Regular monitoring and re-assessments as necessary Cost of Case Management can vary based on the services that are provided.
