Are you a GP who wants do research in general practice? Your practice manager can help A Wood 1,2, J Hocking 2, M Temple Smith 1 1 Department of General Practice, University of Melbourne, Australia 2 Melbourne School of Population and Global Health, University of Melbourne, Australia Aims Presentation of the results of a qualitative study of practice managers (PM) that aims: To determine the role of the PM in the uptake and implementation of a research intervention in Australian general practice Conducted as a sub study of a clinical trial of a chlamydia testing intervention in general practice ACCEPt 1

ACCEPt Australia Chlamydia Control Effectiveness Pilot 1 Aims to assess the feasibility, acceptability, efficacy and costeffectiveness of chlamydia testing for year olds in general practice Cluster randomised controlled trial Intervention implemented in 65 rural general practices 3 4 year intervention period 1. Hocking JS, et al. 12 PRT 09010: Australian Chlamydia Control Effectiveness Pilot (ACCEPt): a cluster randomised controlled trial of chlamydia testing in general practice. Lancet protocol. May ACCEPt intervention Intervention designed to facilitate increased chlamydia testing Intervention components Research Officer visits practice 4 times per year Quarterly feedback on testing rates and positivity Incentive payments for each test done by GPs and PNs Ongoing education and support 2

PMs role in ACCEPt Keep the research on practice agenda Data collection (IT) Arrange meetings Invoicing for incentive payments Recruit new GPs Liaise with Research Officers Support ACCEPt to make chlamydia testing routine Methods recruitment PMs from practices (65) in ACCEPt intervention group 23 PM recruited stratified by size of practice (no. of GPs) Recruited by direct contact Semi structured interviews between April and October 2014 Telephone interview recorded, transcribed verbatim Thematic analysis 3

Results PM interviews 23 Interviews PMs from rural practices Median age Time in role Qualifications & experience 19 female & 4 male NSW, VIC, QLD & SA years 12 years 3 nursing, 13 business, 7 clerical 7 diploma of practice management 21/23 PMs interviewed had no research training or experience Results: PMs views on general practice-based research I think it is a good place to do research the information comes back to our clinicians; it encourages them to think about the way they prescribe or deal with people. It creates awareness. PM 12 I think it improves the care for our patients it makes doctors more aware of their clinical judgements, what they are doing and tests they are ordering. PM 5 4

Results PM role in research 22/23 PMs believe their role is to manage research in the practice Implementing the research and also making sure that it is done correctly and on time. PM 20 If it hasn t got the PM on board you are really not going to get anything done PMs can run with these programs. PM 1.. Results PM role in ACCEPt PMs role supporting ACCEPt The PM creates enthusiasm...if the PM was not interested it is not going to be driven It has got to have someone driving it. PM 1 I take a leadership role I have got a clinical background and a strong rapport with all GPs I think that that enables me to easily lead or keep people on focus, on track and enthused. PM 17 5

Results: PMs supporting GPs to do research PMs out of the group probably have the most amount of time doctors are always with patients. Receptionists are always at the front, nurses helping doctors your PMs are the ones that can implement it. PM 15 Ensure that a project keeps moving forward work with staff manage the process. PM 20 You need that background planning and I think that is an important role for the PM. PM 13 Discussion PMs have identified a role for themselves in research PMs created a role for themselves in the ACCEPt trial The non clinical tasks associated with research in general practice PMs are enthusiastic and supportive of research in general practice 6

Discussion Invest time explaining the research to the PM Explain to the PM the tasks you require then to do Investigate how the PM can implement and drive the research Support research training courses for PMs Acknowledgements All general practices participating in ACCEPt (145) All practice managers ACCEPt staff: Eris Smyth, Alaina Vaisey, Michelle King, Sam Sukkel, Sam Fox, Belinda Ford, Rebecca Lorch & Suzanne Cranny ACCEPt was commissioned by the Australian Government Department of Health. Additional funding has been received from the Victorian Department of Health, NSW Health and the National Health and Medical Research Council. No conflicts of interest 7
