1 GOLD LEVEL PROPOSED LIST OF SERVICES AND PRICING AZNHA Members and Associate Members SERVICES DHR is pleased to offer the following services to members and associate members of AZNHA Payroll Services, including: Providing complete new hire packets with updated tax forms and Form I-9s New hire set-up Web-based payroll input and processing Electronic access to payroll information (at client and employee level)* Payroll compliance assistance (exempt/nonexempt, minimum wage, overtime) Payment of withholding taxes Assumption of federal, state and unemployment tax liabilities Processing and distribution of year end W-2 s Direct deposit set-up Administer all unemployment administration, paperwork, and claims management Provide income and employment verifications Support and administration of all child support, state tax levy, IRS levy, student loans, and writs of garnishment Access to DHR s client and employee web portal, allowing access to payroll forms and new hire packets Vacation/sick/personal time off tracking Access to our innovative Time & Attendance program for tracking employee s service/work hours. Ensure compliance with FT/PT mandates for (ACA) Affordable Care Act. **(Additional cost for Time & Attendance program) Human Resources Services, including: Access to HR Specialists via toll free HR hotline and dedicated address Provide updated state and federal compliance posters Option of allowing Employment Verification Services (EVS) to administer E-Verify on your behalf at no additional cost
2 Insurance and Benefits Offerings, including: Assistance with acquisition of workers compensation insurance, including calculation and payment of premiums (If DHR provides your workers compensation insurance) Manage and track claims between carrier and health care providers Access to group benefit plans DHR Group Plans Dental, Vision, Section 125/FSA accounts, LTD, STD, Life Insurance, AFLAC and Transamerica 401K retirement plans. (These are large group DHR voluntary plans ALL of these plans are guaranteed issue no underwriting for employees and employers are not required to financially participate with no minimum participation requirements for the employer) Access to Major Group Health Plans Fully Insured, partially funded and self insured group health plans plan designs will meet ALL requirements of the Affordable Care Act The plans will provide flexible plan designs, wellness programs, ancillary benefits and affordable rates. (ALL groups are fully underwritten to determine the best plan designs, deductibles and final rates for each group) Employee/employer benefits administration, including calculating and inputting deductions and remitting payments directly to benefit providers* COBRA and benefits continuation administration, including initial notifications and notifications upon termination (if applicable, depending on size of company)* * Additional charges may apply. PLUS the following services: Additional HR Consulting, including: A template employee manual including relevant state and federal law language (binding and copies available for additional cost) On-line HR trainings for staff and managers (two per year) Examples of available trainings: Preventing Harassment in the Workplace Effective Recruiting and Hiring HIPAA Time Management Skills HR Do s and Don ts Progressive Discipline Risk and Safety Services, including: Guidance and training regarding OSHA reporting requirements (including electronic tracking tools) Accident investigation and injury prevention assistance Loss Analysis / Return to Work programs Access to approved safety vendor list
3 Recruiting Services (Included) Four annual postings (one per quarter) on DHR-designated posting sites. DHR can post on various job sites *Additional fees may apply for alternative sites, such as (i.e., Craigslist). Postings and recruiting services available at rates that is substantially lower than current market rates. DHR s rates for these services are as follows: o Standard posting of the job - $150 o Resume scrub - $50 o Prescreening and interview scheduling - $300 PRICING DHR will include all of the services listed above for the fee of $8.00 $8.50 per check (depending on whether the business runs its payroll on a semi-monthly or bi-weekly basis). Pricing is subject to change based on market factors.
