JOB ANNOUNCEMENTS October 10, 2014 Clinical Nurse Specialist-Psychiatry Req. #130 Primary Location: Gardner & Fitchburg Under the general direction of the Behavioral Health Director and Director of Psychiatry Coordinates the comprehensive health care needs of the specific patient population and provides appropriate direct patient care through assessment, diagnosis, and treatment of specific conditions Participates in educational and research activities of the department. Massachusetts Licensed MLP-NP Certified in Massachusetts to practice in the expanded role as a Clinical Specialist in Psychiatry & MH Nursing Demonstrated mastery of a specialty area or other related field normally acquired by 3-5 years progressively responsible nursing experience Dental Assistant Req. #57 Category: 40 hrs. (Non-Exempt) Prepare patient for dental treatment. Take blood pressure and pulse as required. Maintain dental instruments, materials and medications in the quantities to be used for general and specialized scheduled treatment. Keep oral operating areas clean before and after dental procedures. Prepare restorative materials and dental cements. Sterilize instruments using autoclaves Perform routine maintenance described as follows: lubricate equipment, replace expendable parts, and clean operatories. Help patients with appointments, payments etc. when required. Take, develop and mount x-rays (must be State Certified see below). Works as part of team and works at the Community Health Center as required. Prepare work to be sent to dental laboratories. High school diploma required. (Non-Exempt) Responsibilities Prepare Print, and Mail all Record Requests in compliance with HIPAA standards Maintains orderly and accurate filing of medical records in Medi Notes. Sorts and files to correct medical record, correspondence, lab results, and/or dictation Copy, Mail and/or Fax completed forms and/or appropriate supporting portions of the medical record Receive and log all requests for copies of medical records Managing patient care is a team effort that involves clinical and non-clinical staff interacting with patients. High School Diploma or GED plus required Electronic Medical Record Training required or an Associate Degree in secretarial science or equivalent preferred. Experience may be substituted for formal training Work experience similar to a medical office environment required Bilingual English/Spanish REQUIRED. 4 LICSW Req. #15 Primary: Fitchburg (20%) Leominster (80%) Under supervision, provides a wide range of psychosocial evaluation/assessment, diagnostic, counseling therapy, crisis intervention, and/or case management services in an integrated primary care setting located in a community health center Works collaboratively with other mental health and medical providers and trainees engaged in related therapeutic/patient care activities Master's degree in Counseling or Social Work Licensed Independent Social Worker (LICSW) or equivalent or licensure pending, as documented by temporary license Ability to make administrative/procedural decisions and judgments Ability to observe, assess, and record symptoms, reactions, and progress Knowledge and understanding of clinical social work and mental health counseling principles, methods, procedures, and standards Familiarity with psychological testing. Interviewing and psychological/developmental evaluation skills. Explain health care benefits, coverage, and CHC services to applicants and the general community at large. Provide post enrollment support to enrollees who experience barriers to care to reduce these barriers. Have a working knowledge and make referrals to outside helping resources for CHC patients as needed. Attend related community meetings, trainings, and workshops as necessary. Outreach Work with patient service representatives to develop and implement a coordinated outreach plan to educate individuals and the broader community about CHC as well as available insurance options Assist with developing linguistically and culturally appropriate outreach materials. Conduct trainings, presentations, as well as community outreach and enrollment events. Attend related community events, fairs, meetings, trainings, and workshops as necessary.

7 General administrative duties Assist Health Center staff with duties relating to reception, appointment setting, enrollment, and general patient services for Spanish speaking patients. Assist CHC patients, staff and clinical providers by serving as an interpreter as appropriate and providing other translation services as needed. Track enrollment data of community partners and reporting statistics. Foster a positive, professional, effective working relationship with CHC staff, Spanish American Center clinical providers and other staff members at the outreach site. Ensure all clients, staff, and service providers are treated in a professional and courteous manner. Qualifications: High School Diploma or GED, Associate Degree in secretarial science or equivalent preferred One (1) years experience in a customer service or health care related field preferred. 9 Physician Assistant - Req. #43 Primary Location: Leominster Category: Per Diem Benefited: No Under the general direction of the supervising physician, the Medical Director of CHC Family Health Center, the Physician Assistant coordinates the comprehensive health care needs of the specific patient population and provides appropriate direct patient care through assessment, diagnosis, and treatment of specific conditions Massachusetts Licensed Physician Assistant Certified in Massachusetts to practice in the expanded role as a Physician Assistant Demonstrated mastery of a specialty area or other related field normally acquired by 3-5 years progressively responsible nursing experience Physical requirements include near visual acuity to observe changes in health/illness status of patients Podiatrist Req. #56 Provides comprehensive and continuing podiatric care to patients requiring routine and emergency treatment through the diagnosis of disease processes and through the provision of medical treatment of patients at clinics, nursing and private homes, and shelters. Always utilizes best practice as designed by the national standards set by APMA, CDC, and credentialing agencies of CHC. Refers patients to other providers as appropriate Provides services in support of health promotion and preventive medicine Maintains appropriate medical records on assigned patients Participates in patient care audits for quality assurance and utilization review to maintain and improve patient care standard of excellence Maintains medical professional standards and knowledge by regularly attending medical conferences, seminars and continuing education programs Ability to deal with sensitive information in a highly confidential manner. Effective communication skills, both oral and written, with patients, families, visitors, coworkers, health care professionals, funders, and other regulatory agencies. Works as part of team. Awareness of resources available within the health center to accommodate the needs of patients. Demonstrates understanding and commitment of the mission of the health center.
