1 Congratulations! It s a pleasure to welcome you as a member of our first ever Connect Wireless President s Club. For the past several years we have been working on a concept to celebrate the Top Performers on our team who consistently deliver on our Mission and Key Metrics. As we all know CELEBRATON is part of our Culture and we are so proud to be able to recognize you all in this way. Each of you has worked hard over the last six months in the hope of being part of this special group and you deserve something special for your efforts. 5 About your stay The famous Mexican beach town of Los Cabos is located at the southern tip of Mexico's Baja California Peninsula, in the state of Baja California Sur. It encompasses the towns of Cabo San Lucas and San José del Cabo, as well as the hotel zone (the corridor) that lies between the two. Like many Mexican coastal towns Cabo thrives on fishing and tourism, with the main drawcards being sports fishing and golf. Los Cabos is a coastal town with a desert backdrop, in the warmer months temperatures reach 90 degree F with occasional showers during the day and during winter temps range from 80 degree during the day and around 60 degree at night. About the Area Los Cabos sits on the Sea of Cortez and is surrounded by low lying mountains with a dry landscape, which is the first thing you see when flying into the Los Cabos International Airport. The airport has flights arriving daily from all parts of the world, on the short drive to your hotel you will see a transformation from the desert to tropical palm trees. The Los Cabos corridor spans across 32 km and is home to hotels where the rich and famous often visit, as you make your way to your destination you will pass through San Jose del Cabo a traditional Mexican town. Whatever kind of vacation you are looking for you will find it in Los Cabos, it is the perfect destination with the main tourist area called the Golden Zone in the San Lucas Harbor. Be sure to go and explore. The Details The hardest thing to decide on when you arrive or before you arrive is what to do, for the fishing enthusiast you are in the right location with sports fishing and big game fishing tours daily. Sailing, swimming with dolphins, zip-lining, ATV expeditions through the desert are just some of many adventures waiting for you, one famous Cabo landmark that you can t miss getting up close to is the El Archo (The Arch) which is a haven for marine life and a great location to see the passing whale migration. Aside from the beautiful landscapes and adventure wonderland Los Cabo is also famous for its night light and party atmosphere, by day and night the town is alive with music and entertainment. Be sure to try the local cuisines and the yummy fish tacos.

6 PASSPORT INFO: All passport cost are the responsibility of the employee & their guest. Please be sure you and your guest have passports or have already applied for them. In the event that someone can t attend, an alternate will be chosen, time permitting, (enough time for a passport). Once we reach our deadline, I will need confirmation from each person and their guest in order to make the travel arrangement. Please understand. If you don t have a passport for you and your guest you will not be able to make the trip. ALL PASSPORTS THAT HAVE NOT BEEN APPLIED FOR YET - NEED TO BE EXPIDITED!! Please do not use a standard letter- size #10 (4 1/8" x 9 1/2") envelopes. This will help to protect the contents of your mailing from the elements throughout the delivery process. We strongly encourage you to mail your passport application and any personal documents using a trackable delivery method. Please Note: Renewing Applicants will receive their old passport back in the mail. It may be sent separately from the new passport. 8 RELEASE AND WAIVER OF LIABILITY th This Release and Waiver of Liability (the Waiver ) is made and entered into on the 5 day of March, 2015, by and among T3 Wireless LLC DBA Connect Wireless ( T3") and its Employee (defined below). WHEREAS Company (defined below) is sponsoring a trip in April of 2015 to Cabo San Lucas, Mexico for its Employee (defined below). WHEREAS Employee (defined below) desires to attend the trip and participate in all activities and events therein (collectively, the Trip ). Those activities and events may involve strenuous mental and/or physical conditions risking death, illness, injury and/or property loss. Those risks may include but are not limited to those caused by actions, acts of God, conditions, equipment, events, facilities, nature, political issues, temperature, terrain, travel, vehicles, war and/or weather. Furthermore, those risks cannot be eliminated without jeopardizing the essential activities and qualities of the Trip itself. Company (defined below) has taken into account the enforceability and validity of this Waiver into the costs and feasibility of the Trip and otherwise would not be able to sponsor the Trip. WHEREFORE in consideration of attending and participating in the Trip, the recitals above which are a material part of this Waiver and the provisions below, Employee hereby takes action for himself or herself and all administrators, assigns, beneficiaries, devisees, family, executors, grantees, heirs, legatees, next of kin, parties, persons, representatives, spouses, successors and all other parties claiming through him or her (collectively, Employee ) as follows: A. DISCHARGE, RELEASE AND WAIVER. 1. ASSUMPTION OF RISK. Employee hereby assumes all risks in attending, participating and/or volunteering in the Trip including but not limited to any risks that may arise from (i) carelessness, failure to act, fault, gross negligence, insufficient insurance, medical treatment, no fault, omissions, ordinary negligence and/or recklessness of Company (defined below); and (ii) dangerous and/or defective equipment or property whether they same is controlled, maintained, operated and/or owned by Company (defined below). 2. DISCHARGE, RELEASE AND WAIVER. Employee hereby discharges, releases and waives from any and all liability arising out of, connected to or related to carelessness, failure to act, fault, gross negligence, insufficient insurance, medical treatment, no fault, omissions, ordinary negligence and/or recklessness of T3 and its administrators, affiliates, agents, assigns, associates, attorneys, board members, directors, employees, holding companies, indemnities, insurers, legal representatives, managers, members, officers, representatives, successors, sureties and volunteers (collectively, Company ) from any and all liability arising out of, connected to or related to the Trip including but not limited to death, disability, illness, injury, personal injury, property damage, property loss and/or property theft. 9 B. INDEMNIFICATION. 1. HOLD HARMLESS AND INDEMNIFICATION. Employee holds harmless, indemnifies and promises not to sue Company from any and all liability arising out of, connected to or related to carelessness, fault, gross negligence, insufficient insurance, medical treatment, no fault, failure to act, omissions, ordinary negligence and/or recklessness of Company from any and all liability arising out of, connected to or related to the Trip including but not limited to actions, arbitration, attorneys fees and costs, causes of action, claims, contributions, costs, damages, demands, expenses, fees, mediation and/or suits of any kind whatsoever, asserted or unasserted, contingent or mature, disclosed or undisclosed, disputed or doubtful, known or unknown, or prior or present or in the future. 2. ACTS OF OTHERS. Employee acknowledges and agrees that Company is not responsible for the acts, errors, failures to act and/or omissions of any hosts, organizers, others and/or sponsors whom Company hires or uses to facilitate any activities or events during the Trip. C. MEDICAL. 1. CONDITIONS. Employee certifies, covenants, represents and warrants that he or she (i) has not been advised to not participate in the Trip by a qualified medical professional; (ii) is physically fit; and (iii) has sufficiently prepared and/or trained for participation in the Trip. In certifying, covenanting, representing and warranting that he or she is physically fit Employee acknowledges and agrees that he or she has no health related problems or reasons which may preclude participation in any activity or event during the Trip except for:. 2. TREATMENT. Employee hereby consents to receive medical treatment which is deemed advisable in the event of accident, illness and/or injury during the Trip. D. MISCELLANEOUS. 1. CONSTRUCTION. Employee acknowledges and agrees that this Waiver shall be construed as broadly as possible to discharge, release and/or waive Company to the maximum extent permissible under applicable law. 2. GOVERNING LAW. Employee acknowledges and agrees that this Waiver shall be construed according to and governed by the laws of the State of Idaho. 3. LIKENESS. Employee acknowledges and agrees that Company may use his or her film, image, likeness, photo and/or video from the Trip for any legitimate business purpose. 4. SEVERABILITY. Employee acknowledges and agrees that if any provision of this Waiver is held to be invalid or unenforceable for any reason, the remaining provisions shall continue to be valid and enforceable. FURTHERMORE, EMPLOYEE CERTIFIES THAT HE OR SHE WAS ADVISED TO RETAIN INDEPENDENT COUNSEL TO ASSIST HIM OR HER IN INTERPRETING THIS WAIVER AND WAS PROVIDED A REASONABLE TIME PERIOD TO DO SO. IN DOING SO, THE GUARDIAN OR PARENT HEREBY AGREES TO ALSO INDIVIDUALLY BE BOUND TO THE DISCHARGE, RELEASE AND WAIVER; INDEMNIFICATION AND MISCELLANEOUS PROVISIONS.
