5 On top of this, I have taught approximate 470 hours of undergraduate teaching and approximate 691 hours of graduate studies in subjects that are in my area of expertise in Medicine and in other degrees. Approximate 350 hours of those 691 hours of graduate studies have corresponded to Masters and 305 hours to PhD courses including one with an honorable mention (MCD ). This teaching experience has also been in my area of expertise according to the implementing law 1417/1990 of October 26 by which proper guidelines of the subjects in Medicine have been stated. I have taught Continued Training Professional Courses (approx. 110 hours). Jokin De Irala Estévez Born June 9, 1958 in St. Jean de luz (France). I finished elementary school in Ateneo de Manila (Philippines) where I lived for eight years and finished secondary school in St. Jean de Luz where I graduated from Baccalaureat C (Science and Physics). As I continued to improve in my career in Medicine at the University of Navarra (Pamplona), my enthusiasm for helping directly in educational and structural aspects related with the health of the population grew strongly. This is why I started my formal training in the area of Preventive Medicine and Public Health as well as activities carried out in Medicine at the University. This Résumé presented includes my education and my 24 years of teaching and research experience at national and international universities since 1985, year I obtained my Medical Degree, until now. Vice Dean of Investigation and Undergraduate studies of the Faculty of Medicine at the University of Navarra (2010-to present). I ve been in charge of two core subjects in the Degree of Medicine. Initially worked as a gynecologist and obstetrician achieving the first doctoral degree of specialization and after making a decision about changing the specialization he obtained the second degree in pediatrics and later in neonatology working at the Neonatology Department of the Institute of Obstetrics and Gynecology. In 1993 he became the Head of the Neonatology Department at the Karol Marcinkowski University of Medical Sciences in Poznan and in 1998 he was appointed the Chair of the Neonatology Department. In July 2001 he additionally took on the position of the Head of the Neonatology Department at the Polish Mother s Memorial Hospital Research Institute. He was granted scholarships at the University of California in Los Angeles, University of Southern California in Los Angeles, University of Irvine in the U.S. and several times at the University of Turku in Finland. He organized a program for training Polish neonatologists at the University of California. In years he was a Polish coordinator of the Children s Medical Care Foundation in Los Angeles for which he was granted a reward of that Foundation. In 1982 he obtained the title of Philosophy Doctor and in 1990 he became Habilitated Doctor. In 1993 he obtained the title of Habilitated Doctor Professor and in 2000 the title of Professor. He was a member of two Senate Committees and the Head of the University Council on Basic Sciences and for three terms the Head of the University Committee on the Student s Recruitment as well as the person in charge of cooperation with the University in Turku within a governmental agreement. The President of Finland in recognition of his contribution awarded him with the Cross of the White Rose of Finland. The President of Poland awarded him with the Gold Cross of Merit. He was granted a medal of the voivod for the Kujowsko-Pomorski region for his contribution in the development of medicine. He was also a plenipotentiary to the President of He was the Vice President of the Karol Marcinkowski University of Medical Sciences in Poznan in years and later on the President of that University during two terms in years as well as the Vice President of the Conference of the Presidents of Universities of Medical Sciences in Poland in years In years he was a member of the President of the Republic of Poland s Council on Healthcare as well as for one term a member of the Scientific Council of the Minister of Health. He was a member of the Scientific Board of the Child Health Center in Miedzylesie ( ) and is a member of the Scientific Board of the Polish Mother s Memorial Hospital Research Institute in Lodz. For three years he acted as a regional consultant in neonatology for the Szczecin region. For 2 years he has been a regional consultant in neonatology for the Wielkopolska region. He was the state consultant in neonatology in years He was a member of the Supervisory Board for the Wielkopolska Healthcare Union as well as a leader of the team on the reform on the care over the mother and the child in Wielkopolska. In 1993 he became the head of the task force group responsible for implementation of the Improvement of Perinatal Care Program in Poland. The program was approved as a governmental project; he was the head of the program until the end of The program led to implementation of a three level regional system of perinatal care in, at that time, 14 voivodeships of Poland. In 2007 he was appointed by the Minister of Health a member of the Expert Team working on a Central Database of Healthcare Services in the area of neonatology. He acted as the Vice Chairman in one term and the Chairman in another term for the Management Board of the Polish Society of Perinatal Medicine; presently he is a member of the Management Board of the Polish Society of Perinatal Medicine and a member of the Board of the Polish Society of Neonatology as well as the Polish representative in the Boards of the European and World Association of Perinatal Medicine. He was a member of the Scientific Award Team for the city of Poznan. He is a member of the Social Council of the Archbishop of Poznan. He is a co-editor of one periodical and a member of editorial committees in 7 other periodicals including one in Germany and two in the U.S., e.g. International Board Journal of Perinatology the official periodical of the Perinatal Section of the American Academy of Pediatrics. He was granted the second prize of the European Society of the Perinatal Medicine, three times the award of the University s President and four times individual awards of the Minister of Health as well as a prize for a paper awarded during the Congress of the World Society of Perinatal Medicine. 7 terms he was a member of the Education Committee and later European Scientific Society of Perinatal Medicine, and in years he was a co-chairman of the European Study Group on Perinatal Nutrition for the European Society of Perinatal Medicine. For several years he has acted as the chairman of one of three Examination Committees in Poland for the second degree in neonatology. During the years of the Marshal Law in Poland he was the coordinator of the Hope Foundation in the U.S. at that time the University of Medical Sciences Hospital received equipment worth hundred of thousands of dollars. He was the originator and the supervisor of the modernization work of the Neonatology Department in Poznan and after many years of use thanks to his endeavors new rooms were built and equipped by the governmental program and by the Wielkopolska Union of Healthcare. This is presently the most modern object of this kind in Poland. Thanks to his petition, the first Neonatology Department along with the Neonatal Infectious Diseases Department was opened in Poland. In recognition of his contribution in the development of the student scientific movement and his activities for the Student Scientific Society of the University of Medical Sciences in Poznan he became the honorary member of the Student Scientific Society. He was a member of several committees reviewing doctor theses, Ph.D. theses as well as professorship applications. Charles Chapron Born on July 21, 1959 and graduated from Lille University of Medicine in 1991, Charles Chapron was named a University Professor in Paris in Since 2005 Professor Chapron is the head of department of Obstetrics and Gynecology II and Reproductive Medicine at Cochin University Hospital (Paris, France). Coordinator of the teaching program of Obstetrics and Gynecology at Cochin Port-Royal Faculty of Medicine, Chairman of one University diplomas, Director of 20 medical thesis, Professor Charles Chapron has authored and co-authored about 308 peer-reviewed papers in national (103 publications) and international (205 publications) scientific journals concerning surgical laparoscopy in gynecology, reproductive medicine and endometriosis, and near than 700 scientific presentations on national (300) and international (398) meetings. He is the coordinator of 11 scientific books and the author of more than 80 chapters in other scientific books. Since 2013 Professor Chapron is Associate Editor of Human Reproduction Update. He is also a reviewer for a number of journals including Human Reproduction, Fertility and Sterility, British Journal of Obstetrics and Gynaecology, The Journal of Minimally Invasive Gynecology, European Journal of Obstetrics & Gynecology and Reproductive Biology, Gynecologic and Obstetric Investigation, and Gynécologie Obstétrique et Fertilité. Previously he was the Secretary General of the French Society of Endoscopy in Gynecology (SFEG), the Associate Secretary General of the Francophonic International Society of Pelvic Surgery (SIFCP), Co-ordinator of the Special Interest Group (SIG) of Reproductive Surgery of the European Society of Human Reproduction and Embryology (ESHRE), and the Secretary General of the French 12 13

8 society of gynaecological surgery, Société de Chirurgie Gynécologique et pelvienne (SCGP). In 2011 Professor Chapron was elected President of the French Society of Gynaecologic Surgery (SCGP). In 2013 Professeur Chapron was elected President of the Special Interest Group (SIG) on Reproductive surgery and endometriosis of the American Association for Gynecologists Laparoscopists (AAGL). In 2013 Professor Chapron was elected President of the European Endometriosis League (EEL). Professor Charles Chapron is also a Member of several national and international Societies including the French National Academy of Surgery, the International Society for Gynaecologic Endoscopy (ISGE), the American Association of Gynecologist Laparoscopic (AAGL) the European Society of Human Reproduction and Embryology (ESHRE), the European Society for Gynecological Endoscopy (ESGE), the French National Society of Obstetrics and Gynecology, and the French National College of Obstetrics and Gynecology. During the fifteen years, Professor Chapron has been intensively working in Port-Royal University Clinic, Cochin University Hospital, together with a highly specialized team in endoscopic surgery on techniques and results of operative laparoscopy in gynecology and on the pathogenesis, the assessment and the surgical and medical management of endometriosis. 14 Ponnezhathu Sebastian John Born November 11, 1953 in Sherthallia Kerala, India. Dr. P S John is the Head of the Department of Regenerative Medicine, St.Thomas Hospital, Changanacherry, Kottayam, Director of Spinal cord regeneration centre, Fathima Matha hospital, Kottayam and Former Professor and Head of the Department of Orthopaedics, Government Medical College, Kottayam, Kerala. He has vast administrative experience in professional associations. He is the founder president of Asian Society for Computer Assisted Orthopaedic Surgery (CAOS-Asia) and the Indian Society for Computer Assisted Orthopaedic Surgery (CAOS-India). He was the regional representative for Asia and programme committee member of the International Society for Computer Assisted Orthopaedic surgery ( CAOS- International) and the executive committee member of CAOS-Asia. He was the vice president of Indian Orthopaedic Association (IOA), President of Kerala Orthopaedic Association(KOA), Chairman Trauma committee and Academic committees of IOA. Posts held in academic committees include Dean, Cochin University of Science and Technology (CUSAT), Cochin,Kerala, Dean Mahatma Gandhi University(MG university), Kottayam, Kerala, Syndicate member of CUSAT, Chairman of the Board of studies in MG University, Member of Academic council CUSAT, and Visiting Professor The Tamil Nadu Dr. MGR Medical University, Chennai. Basic science research on fracture healing in head injury won him the Silver Jubilee award of Indian Orthopaedic Association (IOA) and golden jubilee award of IOA was received for the work on spinal cord regeneration. He has also received IOA Panacea Biotec Research award, Sri Shyam Lal Saxena Memorial Award from National Academy of Medical Sciences, Best paper award in Asia Pacific Orthopaedic Association (APOA) conference held in, Seoul, Korea and Silver Jubilee award of Kerala Orthopaedic Association. He has given many orations and lectures including the one in the staff college of US FDA. He has many international and national publications and is in the review board of many journals. He is a member of AO Spine 15

9 International, ISMIS, CAOS-International, CAOS-ASIA, Indian Biotechnology Society, Stem cell Voice of India and faculty for AO Trauma Course. Dr. John has received many International recognitions for his outstanding research in the field of Computer Assisted Orthopaedic Surgery. He has started his work on developing a new and less expensive software for computer assisted spine surgery as early as 1996, in association with many computer experts and engineers. The software was developed in 2002 and computer assisted surgery on spine was done for the first time in India on 01/11/2002 by Dr.P.S.John in Medical College Hospital, Kottayam, Kerala using this less expensive software. The work was presented in 6 international conferences and published in 5 international publications. He was selected as the regional representative of the International Society for Computer Assisted Orthopaedic Surgery (CAOS International) and the Programme Committee Member of CAOS-International in 2003 and was continuing this position until He was one among the jury to select the best scientific paper presented during the 5th Annual Conference of CAOS-International held in Helsinki, Finland from June 19th 21st He has chaired many sessions on spine in the international conference on computer assisted orthopaedic surgery. He is the founder president of Asian Society for Computer Assisted Orthopaedic Surgery (CAOS-Asia) and is considered to be the pioneer of computer assisted orthopaedic surgery in India. He was the organizing chairman for the First Asian Meet for CAOS on 27 th & 28 th of March 2004 in Cochin. He was a faculty for the 2 nd Asian Meet held in Singapore on 14 th & 15 th of March 2005, faculty for the 3 rd Asian Meet at Taiwan on 22 nd & 23 rd of April 2006 and faculty for the 4 th annual meet in Seoul, Korea 29 th -31 st March He is credited with popularizing the CAOS technology in India. He was the president of Indian Society for Computer Assisted Orthopaedic Surgery (CAOS India) from 2004 to He has given invited lectures in the subject in various conferences and seminars throughout India. In recognition of his efforts to popularize the technology in India, His excellency the President of India Dr.A.P.J.Abdul Kalam and the Central Minister for Health and Family Welfare Mr.Shatrukhan Sinha have congratulated Prof. P S John. Prof. P S John has chaired more than 14 international conferences, attended more than 20 international conferences and presented scientific papers in 16 international conferences. Stephan Sahm Born January 13, 1959 in Offenbach, Germany. Studied medicine and philosophy at Frankfurt University. He graduated from Medical School in 1884 and had been assistant researcher and assistant physician at Frankfurt University Medical School and an affiliated academic teaching hospital. In his medical dissertation he worked on controlling cell growth of leucemia cells using nucelotides. He completed internship and passed board examinations in Internal Medicine and Gastroenterology, in addition he has taken ESMO-certification in Oncology and graduated in Palliative Care Medicine. He had been chief of the Oncological outpatient unit at the German Clinic of Diagnosis in Wiesbaden. Since 2005 he has been Director of the Medical Clinic I at Ketteler Hospital, Offenbach. Ketteler Hospital is catholic academic teaching hospital. Under his direction the hospital has emerged as a part of the Comprehensive Cancer Center of Frankfurt University. Stephan Sahm is head of Offenbacher DarmCentrum which is a certified high volume centre for treatment of patients with gastrointestinal tumours. Based on his training as a philosopher he has published in the field of medical and bioethical ethics since His research into ethics in medicine and bioethics is focussed on end-of-life issues, pallative care, stem-cell-research and foundational issues of ethics in medicine. In 2006 he passed the postdoctoral lecture qualification on Medical Ethics at Frankfurt University Medical School (Habilitation according to the German academic system). In addition to his appointment as Medical Director at Ketteler Hospital he is an assistant professor with the Institute for History and Ethics in Medicine at Frankfurt University Medical School (Senckenbergisches Institut fuer Geschichte und Ethik in der Medizin, Franfkurt). Stephan Sahm has published in peer reviewed journals on end-of life- issues, advance directives, theory of medicine, palliative care and others more. His book 16 17

10 Care of the Dying and Advance Directives appeared in 2006 (Campus 2006). He is co-author of books about gastrointestinal cancer, palliative care and has contributed book chapters to various volumes on biomedical ethics. He had been invited as an expert in hearings of committees of the German Parliament (Deutscher Bundestag) and the Parliament of the State of Hassia. Recently he had been one of the speakers at a symposium of the German Parliamentary Society in Berlin to comment on a draft law banning commercial assistance in suicide. He is a member of several academic societies, e.g. European Society for Philosophy of Medicine and Health Care, German Academy of Ethics in Medicine a.s.f. He is member of the editorial board of the journal Zeitschrift für Medizinische Ethik. For more than 20 years he is a regular writer on Medical Ethics and Bioethics for the Frankfurt Allgemeine Zeitung, which is the leading daily newspaper in Germany. For more than ten years he had run a periodic column on bioethical issues in that paper. In addition to his academic training he had been educated as an organist, choir director and composer at the Institute of Sacred Music of the Diocese Mainz, Germany. He is currently serving as one of the organists of his local parish. Together with the Franciscan Fra. Helmut Schlegel (lyrics) he has composed (amongst others) chants for stage plays about St. Clara and St. Elisabeth. In 2004 they published a mass for the international youth pilgrimage to Vezelay in France (Vers de horizon nouveaux). Several hymns can be found in contemporary choral collections in Germany, e.g. Junges Gotteslob, Lauterborner Psalmen, Durch Hohes und Tiefes (Young Praise of God, Lauterborn Psaltery, Through the Highest and Deepest). He is founder of the Offenbacher Vokalensemble Prophet, a distinguished chamber choir specializing in Sacred Music. Since 1983 Stephan Sahm is married to Angela Ritter-Sahm who is a lawyer.
