Ebenezer has provided services for older adults for close to 100 years, so we understand the many challenges that go along with finding a new home. We re here to help you and your family in this new chapter of life. The guide will serve as a reference to help answer questions such as: When is it the right time to move? What is available? What should I be looking for in a new community? How can I afford this new lifestyle? We hope this guide will be a useful tool for you as you begin, or continue your search for a new home. 4 Independent Senior Living People who wish to live on their own often describe themselves as living independently. This can broadly describe a lifestyle of living in an apartment, condominium or cooperative building designed with the needs of adults age 55 and older in mind. Extra security, wider doorways and other adapted-design elements are part of the overall features of the buildings. These communities are designed to provide options that create a lifestyle of convenience while offering interaction with other residents to help you avoid isolation. Independent living communities may also have amenities such as underground parking, beautician/barber shops, convenience stores and gift shops. If you find that you need extra help, you can contract services such as housekeeping, personal care, or wellness services that make it possible to age in place without the need to move as your assistance needs grow. See the Additional Resources on Page 22 for Fairview and Ebenezer partners. 6 Assisted Living Assisted Living is rental apartment living for older adults. Minimum age requirements vary. Residents enjoy full apartments with access to services. A basic service package is included (typically meals, light housekeeping and other services) in monthly rent and additional care services such as personal care, wellness services and medication administration, can be added for a fee as needed. Assisted Living is usually less expensive than skilled nursing in a care center, and is suited to people who do not require complicated full time medical assistance. A nurse is on site during business hours and on call 24/7. Night hours are covered by care attendants. To summarize: Assisted Living apartments are typically for those age 55+. 8 Memory Care Like Assisted Living, Memory Care provides choices for personal care and defined medical services. The primary difference is that memory care communities are specifically designed for those with memory loss; they feature specially designed apartments, community areas and controlled exits to prevent residents from wandering off-property. Staff at Ebenezer memory care communities are trained to work with people with dementia who often require a higher level of care and attention throughout the day. Residents needs are assessed by an RN to offer the most suitable level of care. To summarize: Similar to Assisted Living, Memory Care is a secure living apartment and community for persons 55+ who in addition to personal care needs have memory loss which could cause them to become disoriented, wander away or be a danger to themselves or to others. 8 Making Informed Decisions for Vibrant Senior Living

10 Adult Day Programs Adult Day programs serve adults age 55+ who live at home but would benefit from scheduled activities, personal care and supervision during the work week. Many programs host clients from one to five days a week providing social stimulation, fun activities and support such as medication administration and bathing assistance. Transportation can often be arranged by the adult day program. Care givers benefit from the program by knowing that their loved one is well taken care of in a stimulating and nurturing environment. 12 Skilled Nursing Facilities or Care Centers Skilled nursing facilities, commonly referred to as care centers or nursing homes, are licensed healthcare facilities that are inspected and regulated by the state department of health. Skilled nursing facilities offer long- and short-term care for people who need rehabilitation services after illness or injury. Short-term care is also commonly known as transitional care. Skilled nursing facilities also offer longterm care for those who suffer from serious or persistent health issues that are too complicated to be managed at home or in an assisted living community. 18 Senior Living Funding Options When considering a new senior living community, location and ability to afford the costs are often top concerns. At Ebenezer owned and managed communities we know that everyone s finances are different. Please explore the following funding sources with an estate planner, financial advisor, attorney, or other qualified person as you consider your move to an Ebenezer setting. SOCIAL SECURITY INCOME: Many people receive a monthly social security check. This monthly amount gives you an excellent starting point for determining your monthly income. PENSIONS: Some people receive pensions from former places of employment, unions, or other membership organizations. Add this to your list of monthly income sources. LONG TERM CARE INSURANCE: You may have purchased long-term care insurance years ago through an employer sponsored plan or perhaps you paid premiums on your own in preparation for a time when you might need the insurance. Check with your insurance agent to find out which care settings are eligible for coverage. Your agent will also be able to tell you the daily rate provided and how to access the funds. People are often surprised to learn that Medicare does not cover the cost of senior living. However, you may be eligible for Public Assistance (Medicaid) if you meet income and asset tests. Contact your county case manager to apply or determine eligibility. LIFE INSURANCE POLICIES: Check with your insurance agent to find out whether your life insurance policies can be applied to senior living expenses. 18 Making Informed Decisions for Vibrant Senior Living

19 Senior Living Funding Options continued SAVINGS, CHECKING ACCOUNTS, CD S, BONDS, AND OTHER INVESTMENTS: These may generate interest, gains, and dividends that you can spend on your expenses. Or you may need to use your savings and sell your investments to pay for your living expenses. PERSONAL LOANS AND LINES OF CREDIT: You may need to borrow money to help with your short term financial needs. REAL ESTATE: You may have equity in your home, cabin, or investment property. Selling your real estate may be an option for you. ANTIQUES, COLLECTIBLES, JEWELRY, AND OTHER PERSONAL PROPERTY: You may want to consider selling valuables as another source of income. ASSISTANCE FROM LOVED ONES: Your children or other relatives may choose to help with your living expenses. SHARE AN APARTMENT: Living with a roommate means you are responsible for half of the housing expenses. You might enjoy the extra companionship this arrangement offers. VETERANS BENEFITS: If you served during wartime, you and your spouse may be eligible for Aid and Attendance benefits. Contact your county veterans office for more information. TAX DEDUCTIONS: Speak to your tax advisor about deductions for medical expenses and assisted living services. 21 Tips for a Successful Move Whether you are moving to a senior living community or relocating an aging or elderly family member to a nursing home or assisted living community, it is never too early to start sorting and downsizing to prepare for your move. Here are some of our favorite moving tips: Consider hiring a move manager. Start with organizing closets and file cabinets, attics, and basements. Prepare for your downsize bit by bit. Begin to think about items that you would like to take to your new home, and start putting some things not in everyday use in boxes. In preparation for your move, start distributing things to family and friends including family mementos, photos and books. Save only those keepsakes that mean the most to you. Don t ever throw away something that has special memories, is valuable or a family heirloom. Get rid of anything that is not fixable, not wearable, out of date, or just taking up space. Begin to use up canned and frozen foods; you don t want to have to pack and unpack these items. Have draperies, rugs, and other linens you are taking with you cleaned before the move. Develop a customized floor plan. Remember, the more items you discard before the move the less your moving bill will be. Go through medicine cabinet and organize your meds. Throw out anything that is expired. Don t pack them, take them with you. Arrange for overnight accommodations if needed for the night of the move. Ask for assistance! 23 Additional Resources continued Fairview Hospice Hospice is for patients with a terminal illness or who have a life expectancy of six months or less, and who choose not to seek or continue treatment to cure the illness. Hospice care is provided wherever the patient lives. Hospice care helps to manage pain to the comfort level of the patient. Hospice nurses are experts in pain and symptom management. They work closely with the patient, family and physician to assure optimal comfort by using medications, counseling and therapies such as music, massage and energy work. Palliative care can begin at any stage of your illness and it has no time restrictions. Generally, you can receive palliative care where you live, where you receive care, or in the hospital. Designed to reduce symptoms and address your spiritual and emotional needs, palliative care helps to manage suffering and improve the quality of your life. If you find yourself in need of assistance, after your Medicare no longer pays for Home Care, private duty care can help.
