2 The Scott County Senior Resource Directory will provide you with descriptions of services designed to meet the continuing needs of our senior population. Following each description is a list of service providers as well as resource numbers and/or web sites for each option. Scott County does not assume responsibility for endorsing the providers listed. This is not an all-inclusive list as the depth of senior services is vast and constantly changing. Consumers may consult the Yellow Pages Directory, United Way 211 or Scott County Health and Human Services at for additional resources. Scott County Health and Human Services is committed to assisting the seniors of Scott County to remain active and well-informed in making lifestyle choices and changes. 3 Adult Day Services Adult day services are community-based centers designed to meet the needs of adults who may be experiencing a decrease in physical, mental and/or social functioning. Adult day services are comprehensive, nonresidential programs that provide a variety of health, social and related support services in a structured setting. Supported services provided by each adult day program may vary. Services may include: skilled nursing, liaison with physicians, medication administration, incontinence management, personal care assistance, bathing, transportation, beauty shop, breakfast and lunch, rehabilitative therapies, foot care. Participants attend on a planned basis. While at the center, adults will experience a comprehensive, therapeutic social recreation program designed to meet their individual needs. Participants find purpose and develop friendships with peers, allowing them to remain more independent and be a vital part of the larger community. Adult day centers also provide the much-needed respite for the caregiver. The cost of these services varies from center to center. Financial assistance is available for those who qualify. 8 MN State Services for Blind and Visually Handicapped Minnesota Stroke Association Moving (planning, coordinating and packing/unpacking) Gentle Transitions Rose s Daughters National MS Society, MN Chapter (Multiple Sclerosis) Parkinson s Association of MN Struthers Parkinson s Center Scott County Mental Health Center Senior LinkAge Line Shakopee Community Education Southern Valley Alliance for Battered Women Social Security Administration Veterans Assistance Scott County Veterans Service (assistance with government forms, medical care, financial, adult day, skilled nursing, etc.) Veterans Linkage Line Telephone Equipment Distribution Program (Loans phone equipment on a long-term basis to people who are hard of hearing, deaf, speech or mobility impaired. Income based eligibility.) Vision Loss Resources Home and Community Based Waiver Programs The Elderly Waiver (EW) and Alternative Care (AC) programs fund certain home and community-based services for eligible persons age 65 and older. Both programs require that eligible persons meet the level of care provided in a nursing home but choose to reside in the community. The Elderly Waiver program requires that the person be on medical assistance and the Alternative Care is a cost sharing program that helps seniors make the best use of the resources they have. To determine if you qualify for these programs, you must request a Long Term Care Consultation by contacting: Scott County Home and Community Care
9 Home Care (Non-medical) Non-Medical Home Care focuses on assisting the elderly individual to remain independent. Services may include assistance with personal cares such as bathing, dressing, grooming, toileting and/or housekeeping, laundry, linen changes, companionship and incidental transportation. Non-Medical home care services can also help seniors adjust to life at home after hospitalization, often assisting with the maintenance of a prescribed exercise/rehabilitation program. Services from a nonmedical home care agency are generally provided on an hourly basis. Home Care (Medical) Home Health Care services cover a broad range of services including: skilled professional and paraprofessional services (nursing and rehabilitation therapies), hightech pharmacy services provided and/or delivered to your home. In order to be covered by health insurance, most services must be ordered by your physician and must be medically necessary to maintain or improve your health condition. Generally, home health services are initiated when an elder is no longer able to care for him/herself due to recent changes in their health status. Often a physician, nurse, hospital discharge planner, or case manager suggests obtaining professional help at home to assist with your health care needs. Medical home health care services are generally provided on a visit basis rather than an hourly basis. 11 Housing Independent Living Facilities The first decision to be made is do I want rental or ownership? The decision properly made should be based on health, assets and needs. Elders and their families need to be aware of the variety of options available. Rental options Market Rate Rental offers properties at the prevailing price in the community. A lease is signed that states the monthly rent for a period of time. These units fall into two categories: independent and independent with services. Services may include meals, laundry and cleaning. Ask what is included in the rent? Be very specific with your questions (example: Is there a full time activities director?) Always ask the costs. Campus style complexes offer a Continuum of Care that may include assisted living and memory care located on the campus. This choice offers convenient access to added services as health care needs change. Subsidized Apartment Rental is based on the gross income of the prospective tenant. There are many different programs offered for elders. It can take time for apartments to open up and waiting lists are long. Plan ahead to get qualified, especially for county programs. Ownership: Condominiums and Cooperatives Condominium ownership allows the resident to hold a title to the property they live in while the common property is held jointly. Association fees are collected to cover maintenance costs for the common property. Cooperative ownership involves residents purchasing shares of the housing corporation who own and operate the property. The property is not owned outright. Both condominiums and cooperatives have association fees. Ask all questions on costs. What is and is not included in these fees? Are there any services available at an additional cost? For more information on the follow programs for Seniors, and other CAP Agency programs, contact CAP Agency at and/or visit our web site at Senior Dining Home Delivered Meals CHORE Weatherization Thrift Store Emergency Services Energy Assistance Food Shelf Information and Referral Housing Volunteer Opportunities Assisted Living Facilities Assisted Living or Housing with Services providers are senior developments that offer services and support with daily living activities. Providers vary greatly in their costs, what services they offer, and level of supervision they provide. When searching for assisted living services it is best to compare prices, staffing, and features along with their ability to allow the resident to age in place. Services and level of care features may include personal assistance, meals, laundry/linen service, 24-hour emergency response system, transportation and organized social/ recreational activities. Services may or may not be included with rent. Because prices and features vary, it is important to check with the provider and be aware of what is included and what extra fees may be. 11

Nursing homes provide skilled care and custodial care to seniors who are no longer appropriate to remain living in their home independently. The number of beds in a nursing home can range from approximately 20 to 500. 16 Memory Care Many providers of senior care address the needs of people with dementia and Alzheimers. However, there are facilities and agencies that specialize in meeting the needs of those with memory loss. Specialized programming, security systems to safeguard residents, dementia specific employee training are some features found with these providers. Check with the provider about what is included in the daily rate and what services require additional fees. Hospice care is appropriate when the focus of care is on comfort and no longer on cure. This is an approach of care that emphasizes quality of life with a limited life expectancy. Hospice neither hastens nor postpones death; it affirms life and regards dying as a normal process. The hospice program stresses human values that go beyond the physical needs, which also includes, psycho-social and spiritual care to the individual and their family.
